Severe community-acquired pneumonia caused by Chlamydia abortus in China: a case report.

BACKGROUND: Chlamydia abortus causes abortions in ruminants; it can also cause miscarriages and stillbirths in pregnant women. However, it rarely causes pneumonia in humans. Here, we report a case of severe community-acquired pneumonia caused by C. abortus. CASE PRESENTATION: On admission to our hospital, a 74-year-old woman reported that she had had a fever, cough, phlegm in her throat, and shortness of breath for 10 days. In the local hospital, she was initially diagnosed with community-acquired pneumonia and treated with piperacillin-tazobactam for 4 days. However, her condition worsened,

Chronicles from the Nest: A Case of Psittacosis in India.

Psittacosis is a rare zoonotic disease caused by a gram-negative obligate intracellular bacterium Chlamydia psittaci, which is transmitted through contact with infected birds. It comprises approximately 1% of all community-acquired pneumonia cases. However, this can be just the tip of the iceberg pertaining to the lack of routine testing and awareness of this disease entity, thereby requiring a high index of suspicion for its diagnosis. We report a case of a 37-year-old male presenting with high-grade fever with chills, acute onset of dyspnea, dry cough, arthralgia, and myalgia which was not r

Early and rapid diagnosis of Chlamydia psittaci pneumonia by tNGS in six patients: a case series.

BACKGROUND: Psittacosis is a zoonotic infectious disease caused by Chlamydia psittaci (C. psittaci) infection, which can be transmitted by birds, poultry and wild animals. The symptoms and imaging findings of C. psittaci pneumonia are atypical and primarily rely on etiological diagnosis. The incidence of C. psittaci infection has been significantly underestimated because of the low sensitivity and poor timeliness of traditional diagnostic methods. Therefore, early and accurate diagnosis of psittacosis remains a challenge. CASE PRESENTATION: A case series with six pneumonia patients who were a

Psittacosis Pneumonia Features, Distinguishing Characteristics, and Outcomes: A Retrospective Study.

PURPOSE: Psittacosis is an often-neglected cause of community acquired pneumonia (CAP). The limited diagnostic methods for psittacosis pneumonia invariably result in an unfavourable prognosis. Consequently, the early detection of psittacosis pneumonia is crucial. This study aimed to analyse the characteristics, clinical features and treatments of the patients to improve early diagnosis and outcomes. PATIENTS AND METHODS: We retrospectively analyzed the clinical features and outcomes of 52 cases of psittacosis pneumonia diagnosed with next-generation sequencing (NGS) from January 2022 to Augus

Acute respiratory distress syndrome caused by Chlamydia psittaci: a case report and literature review.

BACKGROUND: Psittacosis is a zoonotic disease with a low incidence rate and a lack of specificity in clinical manifestations, making it prone to be missed, misdiagnosed, and even cause delayed treatment for patients. Metagenomic next-generation sequencing (mNGS) was successfully performed for the diagnosis of a young patient with psittacosis progressing to acute respiratory distress syndrome (ARDS), and precisely targeted antibiotic treatment was promptly administered. Additionally, a comprehensive review was conducted on 68 cases of psittacosis complicated with ARDS, with the goal of improvin

A novel variant of Chlamydia psittaci causing human psittacosis in China.

From January 2022 to November 2022, sporadic psittacosis occurred in Lishui city, China. The patients were presented with fever, cough, and pulmonary infiltration. Their clinical symptoms were not relieved after receiving cephalosporin, penicillin, beta-lactamase inhibitors, and quinolones. Metagenomic next-generation sequencing of bronchoalveolar lavage fluid samples from the patients revealed Chlamydia psittaci infection. Then, three C. psittaci strains were isolated from the patients. Their whole genome sequences (WGSs) were obtained, and a core genome multilocus sequence typing (cgMLST) me

SARS-CoV-2 Symptoms during the Omicron Surge Differ between Boosted and Vaccinated Non-Boosted Persons.

Purpose: To determine the impact of booster COVID-19 vaccination on SARS-CoV-2 symptoms. Background: The Omicron surge of infections provided an opportunity to evaluate symptoms in relation to booster receipt. Methods: At a US medical college, the number, type, and duration of symptoms were evaluated for 476 students or employees, factoring in days between last vaccination and SARS-CoV-2 diagnosis. Results: Compared with vaccinated non-boosted individuals, boosted individuals reported a significantly higher frequency of nasal congestion (57.9% vs. 44.4%, p = 0.018) and nasal congestion and/or

Exacerbation of Osteoarthritic Joint Pain by Lyme Disease.

Lyme disease progresses through three distinct clinical phases: early localized, early disseminated, and late disseminated. Lyme arthritis is characterized by attacks of joint swelling lasting for weeks to months, potentially causing permanent joint damage in late disseminated disease. Our case focuses on a 63-year-old, obese, type 2 diabetic, wheelchair-bound, Caucasian male with severe bilateral knee pain. Our patient had previously undergone bilateral knee arthroscopies for meniscal tears and also had knee injections performed previously without the desired level of pain alleviation. He ind

Takotsubo Syndrome in Patients With COVID-19: A Systematic Review.

BACKGROUND: Respiratory conditions are major physical triggers of takotsubo syndrome (TTS) and portend worse outcomes. However, data on TTS in patients with coronavirus disease-2019 infection (COVID-19) are limited. METHODS: We searched PubMed, Embase, and Cochrane Library databases for case reports for the period 2019-2022 describing TTS in patients with COVID-19 pneumonia (TTS-COVID). We summarized the clinical data and outcomes and compared them to those in patients with TTS with an acute respiratory disease other than COVID-19 as a trigger (TTS-acute respiratory disease) and those with TT

Blastomycosis Awareness: A Crucial Reminder Amidst the Fifth Pneumonia Episode.

Blastomycosis is a rare fungal infection endemic to North America and parts of Africa. It can be challenging to diagnose until it reaches a critical stage. We present a blastomycosis case in Alabama, emphasizing the importance of early recognition and management. A 67-year-old man had a month of worsening respiratory symptoms, which included malaise, cough, shortness of breath, and fever. Despite multiple urgent care visits and treatments with antibiotics and steroids, his condition continued to deteriorate. He was hospitalized with persistent fever and hypoxia. The imaging revealed patchy lun

Pulmonary infection with an unusual microorganism.

Pulmonary trichomoniasis is an underdiagnosed disease. In most cases, there is an underlying clinical condition related to immunosuppression. The results of molecular biology techniques indicate that trichomonad infections have been significantly underestimated. A 7-year-old girl with a medical history of suspected juvenile rheumatoid arthritis presented with a fever, chills, and a productive cough. Her chest computed tomography scan indicated a pericardial effusion and consolidation in the left lower lobe. In direct microscopy of the bronchoalveolar lavage fluid, we identified a motile and fl

Symptoms predictive of Fusobacterium necrophorum pharyngotonsillitis - an observational study of cases presenting to hospitals in Southern Sweden.

OBJECTIVES: Fusobacterium necrophorum is a common cause of pharyngotonsillitis. However, no guidelines exist on when to diagnose or treat it. We aimed to investigate associations between clinical criteria and F. necrophorum-positivity in pharyngotonsillitis and assess the predictive potential of a simple scoring system. METHODS: Pharyngotonsillitis patients who were tested for F. necrophorum (PCR) and presented to hospitals in the Skåne Region, Sweden, between 2013-2020 were eligible. Data were retrieved from electronic chart reviews and registries. By logistic regression we investigated asso

An Atypical Presentation of Legionnaires' Disease.

Legionnaires' disease is an atypical pneumonia caused by Legionella pneumophila. Legionella species are found in freshwater sources and are transmitted through inhalation of contaminated aerosols. Patients commonly present with fever, chills, and cough. However, in immunosuppressed patients or severe cases, the disease can lead to multiorgan failure. In recent years, the incidence of Legionnaires' disease has drastically increased and unfortunately is commonly underdiagnosed. Gold-standard diagnosis is made through sputum cultures; however, urine Legionella antigen remains the most common test
