Mycobacterium tuberculosis cough aerosol culture status associates with host characteristics and inflammatory profiles.

Interrupting transmission events is critical to tuberculosis control. Cough-generated aerosol cultures predict tuberculosis transmission better than microbiological or clinical markers. We hypothesize that highly infectious individuals with pulmonary tuberculosis (positive for cough aerosol cultures) have elevated inflammatory markers and unique transcriptional profiles compared to less infectious individuals. We performed a prospective, longitudinal study using cough aerosol sampling system. We enrolled 142 participants with treatment-naïve pulmonary tuberculosis in Kenya and assessed the ass

Clinical profiles and related factors in tuberculosis patients with positive sputum smear mycobacterium tuberculosis tests.

The aim of this study was to explore the related factors linked to the development and infectivity of tuberculosis. This was achieved by comparing the clinical characteristics of patients with pulmonary tuberculosis (TB) who tested positive in smear Mycobacterium tuberculosis tests with this who tested negative in smear mycobacterium tests but positive in sputum Gene Xpert tests. We gathered clinical data of 1612 recently hospitalized patients diagnosed with pulmonary tuberculosis who tested positive either in sputum Gene-Xpert test or sputum smear Mycobacterium tuberculosis tests. The data wa

Challenges in Managing Newly Diagnosed Granulomatosis With Polyangiitis and Concurrent Respiratory Infections: A Retrospective Case Series.

INTRODUCTION: Granulomatosis with polyangiitis (GPA), formerly termed Wegener's granulomatosis, is an autoimmune disease marked by necrotizing granulomatous inflammation and vasculitis affecting small-sized vessels. It commonly impacts the renal and respiratory systems. MATERIALS AND METHODS: This retrospective case series sampling conducted in a tertiary care hospital between May 2023 and April 2024 examined six newly diagnosed GPA patients who were proteinase 3 cytoplasmic-antinuclear cytoplasmic antibody (PR3 c-ANCA) positive and had concurrent respiratory infections. None of them had any

Assessing the impact of COVID-19 non-pharmaceutical interventions and relaxation policies on Class B respiratory infectious diseases transmission in China.

This study investigates the incidence of Class B respiratory infectious diseases (RIDs) in China under the Coronavirus disease 2019 (COVID-19) epidemic and examines variations post-epidemic, following the relaxation of non-pharmaceutical interventions (NPIs). Two-stage evaluation was used in our study. In the first stage evaluation, we established counterfactual models for the pre-COVID-19 period to estimate expected incidences of Class B RIDs without the onset of the epidemic. In the second stage evaluation, we constructed seasonal autoregressive integrated moving average intervention (SARIMA

Coexistence of pulmonary thromboembolism and pulmonary tuberculosis: a diagnostic dilemma.

A man in his early 50s with previously treated pulmonary tuberculosis (TB) presented with a 3-month history of cough, expectoration and progressive breathlessness, accompanied by significant weight loss. Examination revealed tachycardia, tachypnoea, hypoxaemia and unilateral diminished breath sounds. Investigations showed anaemia, leucocytosis and a homogeneous opacity on the left side of the CXR. ECG and echocardiography suggested pulmonary thromboembolism (PTE). A CT pulmonary angiogram confirmed a chronic embolus and a severely fibrosed left lung. A comprehensive thrombophilia workup yielde

Bacterial pneumonia patients with elevated globulin levels did not get infected with SARS-CoV-2: two case reports.

BACKGROUND: COVID-19 began in December 2019, rapidly spreading worldwide. China implemented a dynamic zero-COVID strategy and strict control measures after the outbreak. However, Guangzhou city ended closed-off management by the end of November 2022, leading to exposure to SARS-CoV-2. Despite most hospitalized patients being infected or co-infected with SARS-CoV-2, some remained uninfected. We report two cases of bacterial pneumonia with elevated globulin levels not infected with SARS-CoV-2, aiming to identify protection factors of SARS-CoV-2 infection and provide a scientific basis for SARS-C

Very High Warfarin Dosing Requirements in a Patient With Tuberculosis and a Pulmonary Embolism: A Case Report.

Purpose: To describe a case of significantly increased warfarin requirements in a patient receiving rifampin for the management of tuberculosis. Summary: A 76-year-old male was admitted due to altered mentation, cough, and weight loss. He was diagnosed concurrently with tuberculosis and a pulmonary embolism. Given the profound effect of rifampin on CYP450 enzymes, direct oral anticoagulants were avoided and warfarin therapy was selected. Management was further complicated by a gastrointestinal bleed during admission, history of cancer, and low body weight. After several weeks of daily internat

[Emergence of a multidrug-resistant tuberculosis through inadequate treatment of isoniazid monoresistance].

HISTORY: We admitted a 65-year-old patient with suspected reactivation of a pulmonary tuberculosis for further diagnosis. FINDINGS AND DIAGNOSIS: 14 months after completing a standard treatment course against pulmonary tuberculosis, the patient presented with cough and night sweat. A CT-scan revealed signs of a bipulmonary progress. Microbiological results proved multi-drug resistant tuberculosis (resistances against isoniazid and rifampicin). Reviewing the patient's old records uncovered a previous isoniazid-resistance at the start of the first treatment course, which had not been appropriat

Clinical and Radiological Presentation of Patients with Smear Positive Pulmonary Tuberculosis.

Chest X-ray is an important diagnostic aid frequently used alongside microscopic smear of sputum for the confirmation of pulmonary tuberculosis (TB). However, there is a dearth of literature investigating the clinical and radiological pattern of sputum positive pulmonary TB among adults in Bangladesh. The current study explored these patterns in presentation. This descriptive cross-sectional study was conducted at outpatients in department of medicine of a tertiary care hospital. A total of 50 newly diagnosed adult cases of smear positive pulmonary TB attending at the Directly Observed Treatme

Bilateral chronic cavitary pulmonary aspergillomas in an adult patient with recurrent tuberculosis: a case report and literature review.

BACKGROUND: Aspergillomas are globular growths of Aspergillus fumigatus, a benign aspergillosis of the lungs. It usually affects patients who are immunocompromised and have anatomically defective lung structures. The majority of aspergilloma cases are asymptomatic, despite the fact that 10% of cases spontaneously resolve. Most patients do not have any symptoms from their lesions. Direct serological or microbiological evidence of an Aspergillus species along with radiologic evidence is required for the diagnosis of an aspergilloma. CASE: We describe a 35-year-old adult Oromo male patient who h

Pneumocystis jirovecii Pneumonia and HIV-Associated Nephropathy in Acute HIV Infection.

HIV is a retrovirus that affects the body's immune system, primarily dendritic cells, macrophages, and CD4+ T cells. As a result, several opportunistic infections are associated with HIV infection, including Pneumocystis jirovecii pneumonia (PJP), Toxoplasma gondii (toxoplasmosis), Cryptococcus (cryptococcosis), and Mycobacterium avium complex (MAC) infection. HIV is also associated with acute kidney injury and chronic kidney disease. The classic kidney disease related to HIV is HIV-associated nephropathy (HIVAN). HIVAN pathogenesis is linked to glomerular and renal tubular epithelial cell inf

Non-sputum-based samples and biomarkers for detection of Mycobacterium tuberculosis: the hope to improve childhood and HIV-associated tuberculosis diagnosis.

In 2014, the World Health Organisation (WHO) published target product profiles (TPP) for development of novel tuberculosis (TB) diagnostics. One of the key highlights is the need for point-of-care non-sputum-based tests capable of detecting all forms of TB through identification of characteristic biomarkers or biosignatures. Compared to the limitations associated with sputum-based TB tests, non-sputum samples are easy to collect, non-invasive, with potential to improve TB diagnosis among children and among people living with HIV/AIDS (PLHIV). This review gives an overview of the existing evide

A dataset of Solicited Cough Sound for Tuberculosis Triage Testing.

Cough is a common and commonly ignored symptom of lung disease. Cough is often perceived as difficult to quantify, frequently self-limiting, and non-specific. However, cough has a central role in the clinical detection of many lung diseases including tuberculosis (TB), which remains the leading infectious disease killer worldwide. TB screening currently relies on self-reported cough which fails to meet the World Health Organization (WHO) accuracy targets for a TB triage test. Artificial intelligence (AI) models based on cough sound have been developed for several respiratory conditions, with l

Swyer-James-MacLeod Syndrome in an East African Patient.

Swyer-James-MacLeod syndrome (SJMS) is a rare pulmonary disorder characterized by unilateral hypoplasia of pulmonary vasculature, often resulting in emphysema and sometimes bronchiectasis. Although its exact cause remains uncertain, SJMS is believed to be a complication of childhood respiratory infections, such as those caused by respiratory syncytial virus, influenza, Bordetella pertussis, and Mycobacterium tuberculosis. The condition often presents with nonspecific respiratory symptoms, making diagnosis challenging and frequently delayed. We report a case of a 23-year-old Kenyan female with

TB or Not TB, That is the Question?

A 26-year-old female presented with a 3-month history of dry cough, unintentional weight loss, night sweats and fatigue. Her background history was significant for ulcerative colitis, managed with Adalimumab for almost 2 years. Clinical examination was unremarkable, apart from some mild pallor. Abnormal chest x-ray findings prompted a computerised tomography (CT) thorax which demonstrated multifocal peri-bronchial consolidation. The differential diagnosis was multifocal organising pneumonia and tuberculosis (TB). Extensive investigations, including invasive bronchial imaging and biopsy, ultima

Clinical Presentation and Management of Pulmonary Infection Caused by Mycobacterium paraffinicum.

Non-tuberculous mycobacteria (NTM) are environmental organisms that rarely cause infections in healthy individuals but can be opportunistic pathogens in those with compromised immune systems or chronic lung conditions. Mycobacterium paraffinicum, a newly recognized species, is among these rare pathogens. We report a rare case of pulmonary infection with M. paraffinicum, detailing its clinical presentation, investigative findings, and treatment outcome. A 50-year-old male smoker with a history of chronic obstructive pulmonary disease (COPD), severe emphysema, and other comorbidities presented w
