A Differential Diagnostic Nomogram for Mycoplasma pneumoniae and Bordetella pertussis in Pediatric Upper Respiratory Infections.

BACKGROUND: Differentiating Mycoplasma pneumoniae (MP) from Bordetella pertussis (BP) infections in children at primary care hospitals is challenging due to similar symptoms and overlapping epidemic seasons. This study evaluated the use of C-reactive protein (CRP) levels and lymphocyte counts from a single blood test to quickly distinguish between MP and BP infections. METHODS: The study included a derivation cohort (n = 301) and a validation cohort (n = 87). Logistic regression analyses identified predictors for differentiating MP and BP infections, leading to the development of a prognostic

Triple M Overlap Syndrome: A Rare Complication of Immune Checkpoint Inhibitors.

Immune checkpoint inhibitors are a major part of many cancer treatment protocols. Most side effects are generally minor, including fatigue, rashes, flu-like symptoms, headaches, or cough. However, some potential side effects may require prompt treatment to ensure optimal outcomes, so it is essential for clinicians to be knowledgeable about these rare but possible complications. We report a case of an 88-year-old woman who presented with the triple M overlap syndrome (TMOS) of myasthenia gravis, myositis, and myocarditis after receiving four rounds of atezolizumab treatment. This diagnosis was

Bordetella parapertussis Evades Neutrophil-Mediated Killing by Failing to Trigger Innate Activation Even After IFN-γ Priming.

B. parapertussis is one of the etiologic agents of whooping cough and its incidence has increased following the COVID-19 pandemic. Since current pertussis vaccines fail to induce protection against B. parapertussis, the course of infection critically relies on the innate immune response. However, in the absence of opsonic antibodies, B. parapertussis fails to activate resting neutrophils, thereby preventing bactericidal responses. We here investigated whether neutrophil priming with gamma-interferon (IFN-γ), the main proinflammatory cytokine present during infection, changes this outcome. We o

Balloon-Stopper Retrieval and Peripheral Parking After Inadvertent Ostial Right Coronary Stent Deployment.

BACKGROUND: Percutaneous treatment of right coronary artery (RCA) ostial lesions is technically challenging due to cardiorespiratory motion. CASE SUMMARY: We describe a 70-year-old man undergoing PCI for an ostial RCA lesion where a forceful cough caused complete stent expansion within the aortic root. The stent remained mechanically engaged to the delivery shaft. A 4.0 × 15 mm balloon was inflated at the guiding catheter tip as a "balloon stopper," enabling controlled retrieval and "peripheral parking" (intentional implantation) in the right brachial artery. DISCUSSION: This case illustrate

Aspergillus tubingensis pneumonia with pulmonary oxalosis in a virologically suppressed person with HIV: A case report.

Opportunistic aspergillosis has occasionally been reported in individuals with advanced HIV infection but remains rare overall and is particularly uncommon under well-controlled conditions. Here, we present a case of Aspergillus tubingensis pneumonia in a virologically suppressed person with HIV (PWH) in his late sixties. He was receiving bictegravir-based antiretroviral therapy, with a CD4 cell count of 368/μL and undetectable HIV RNA two months before symptom onset. His symptoms began with a cough, followed 20 days later by a fever and dyspnea, which required hospitalization. Computed tomogr

Pulmonary Vein Stenosis Following Radiofrequency Catheter Ablation Presenting With Massive Pleural Effusion: A Case Report.

We report a rare case of pulmonary vein stenosis (PVS) presenting with massive pleural effusion 5 months after radiofrequency catheter ablation (RFCA) for atrial fibrillation. A 58-year-old woman developed persistent fever, cough, and left lower lobe consolidation with a large pleural effusion. Initial antibiotic therapy was ineffective. Three-dimensional computed tomography (3D-CT) revealed severe stenosis of the left inferior pulmonary vein. Following balloon angioplasty, both pulmonary consolidation and pleural effusion resolved. Because PVS may present with nonspecific findings and delayed

The Ethics of Lung Cancer Education for Pulmonary Fibrosis Patients: Addressing Diagnostic Overshadowing and Cancer Risk.

Idiopathic pulmonary fibrosis (IPF) is a chronic, progressive interstitial lung disease associated with a substantially increased risk of lung cancer. Yet IPF is still primarily conceptualized and communicated as a disease of progressive fibrosis. This creates an important diagnostic challenge: worsening cough, breathlessness, fatigue, functional decline, or weight loss may be attributed to progression of fibrosis even when malignancy is also a possibility. In the absence of standardized cancer surveillance guidelines for IPF, ensuring that patients understand their elevated cancer risk become

Flower aqueous extract of Hylocereus undatus (Haw.) Britton & Rose attenuates influenza infection: involvement of the TLR4/MyD88/NF-κB signaling axis.

ETHNOPHARMACOLOGICAL RELEVANCE: The flowers of Hylocereus undatus (Haw.) Britton & Rose (HUF), commonly known as "Jianhua" or "Bawanghua", represent a well-known medicinal-edible plant in South China. In folk practice, HUF is traditionally applied to ameliorate respiratory disorders, including cough, sore throat and other influenza-like symptoms. Cumulative pharmacological evidence has demonstrated that HUF extracts possess anti-inflammatory and immunomodulatory activities. Nevertheless, its protective effects against influenza infection remain largely unclarified, with the underlying molecula

Anesthetic management for tracheal resection and reconstruction: conventional principles and emerging innovations.

PURPOSE OF REVIEW: Tracheal resection and reconstruction (TRR) surgery is one of the most challenging procedures in thoracic anesthesia because it requires continuous coordination between the anesthesiologist and surgeon while sharing the airway throughout surgery. This review aims to provide a clinically useful framework for anesthesia management in patients undergoing tracheal surgery. It integrates traditional principles of managing shared airways with novel developments in anesthesia, including robot-assisted non-intubation surgery, flow-controlled ventilation (FCV) with ultrathin tubes, a
