A machine learning approach to respiratory biomechanics: Identifying a PCA-derived membrane-volume coupling pattern in post-COVID pulmonary function.

BACKGROUND: Persistent dyspnoea after COVID-19 may not be fully explained by conventional pulmonary function tests. This retrospective cross-sectional study used secondary data to examine relationships between lung-volume and diffusion-related variables within an exploratory respiratory-biomechanical framework. METHODS: Participant-level data from 572 post-COVID participants and 72 healthy controls were obtained from an open-access multicenter dataset. Four derived relational descriptors and conventional pulmonary variables were entered into principal component analysis (PCA). Covariate-adjus

Luteolin alleviates neurogenic inflammation in OVA-induced combined allergic rhinitis and asthma syndrome mice through TRPV1/ERK-related signaling.

OBJECTIVE: Combined Allergic Rhinitis and Asthma Syndrome (CARAS), a common chronic allergic respiratory disease, involves dysregulated neuro-immune interactions and neurogenic inflammation. Luteolin (LUT) has significant anti-inflammatory and antioxidant activities. However, its effects and underlying mechanism in CARAS remain unclear. This study aims to determine the effects of LUT administration during the airway challenge phase of CARAS and its association with TRPV1/ERK signaling. METHODS: BALB/C mice were used to establish an ovalbumin (OVA)-induced CARAS model and LUT was administered.

Isolated granulomatous pulmonary artery vasculitis associated with Hughes-Stovin syndrome: a case report.

BACKGROUND: Hughes-Stovin syndrome (HSS) is an exceptionally rare vasculitis, characterized by pulmonary artery aneurysms with in situ thrombosis and is increasingly regarded as part of the vascular Behcet spectrum. Diagnosis remains challenging due to its rarity, lack of validated diagnostic criteria, and overlap with other vasculitis. CASE SUMMARY: We describe a 71-year-old male who presented with progressive dyspnoea and chest pain. Serial imaging demonstrated rapid enlargement of a main pulmonary artery aneurysm from 5 mm to 68 × 55 mm over 18 months, prompting surgical repair. Histopatho

Non-Contrast MR Pulmonary Angiogram in the diagnosis of Acute Pulmonary Thromboembolism in Chronic Kidney Disease - Preventing Insult to Injury.

Computed tomography pulmonary angiography (CTPA) remains the imaging modality of choice for diagnosing acute pulmonary thrombo-embolism (PTE). However, the evaluation of kidney function should be a mandatory step prior to the administration of any contrast media, as iodinated contrast requires careful risk stratification in patients with pre-existing kidney impairment. We present a 72-year-old male with stage G4 chronic kidney disease (CKD), obesity, diabetes and hypertension with history of acute worsening of dyspnoea. Clinical findings, which included tachycardia, hypoxia and a new diagnosis

Proximal Tracheal Mucoepidermoid Carcinoma Complicated by Early Anastomotic Stenosis-A Case Report.

Proximal tracheal mucoepidermoid carcinoma may remain occult despite prolonged upper-airway symptoms and normal flexible nasoendoscopy. Stridor following tracheal resection should prompt assessment of the anastomosis, as early postoperative stenosis caused by A-frame deformity and granulation tissue may be successfully managed with combined laser therapy, balloon dilatation, and intralesional corticosteroid injection.

Intermittent hypoxia interventions for inducing ventilatory acclimatization to high altitude?

Accumulation of ∼200 h of hypoxia exposure (pre-acclimatization) has been suggested to prevent the development of acute mountain sickness when a person is subsequently exposed to high altitude. These pre-exposures are typically undertaken intermittently at or near the place of residence, primarily during sleep in hypoxic tents, and are likely to induce effective ventilatory acclimatization. However, the physiological consequences of different modes of intermittent hypoxia (IH) exposure on ventilatory acclimatization remain largely obscure and are the subject of much controversy. The aim of thi

Neonatal airway emergency: congenital laryngeal myxoma.

Laryngeal myxomas are exceptionally rare benign mesenchymal tumors, particularly in the pediatric population. We present a case of a neonate with a history of brief endotracheal intubation who developed inspiratory stridor and respiratory distress. Flexible laryngoscopy revealed a pedunculated mass on the right true vocal cord. The lesion was completely excised via microlaryngobronchoscopy. Definitive histopathological and immunohistochemical analysis-showing a hypocellular myxoid stroma, bland spindle cells, a low Ki-67 proliferation index (<5%), and positivity for CD34 with negative staining

Use of sole-agent ketamine sedation to facilitate laryngoscopy and foreign body removal in acute supraglottic airway obstruction.

A 78-year-old man presented with stridor and dysphonia after choking at home. He deteriorated, with respiratory distress and an oxygen saturation of 55% despite oxygen via a non-rebreather mask. Flexible nasendoscopy demonstrated a food bolus between the epiglottis and glottis. The team judged apnoea and assisted positive-pressure ventilation before extraction to be hazardous, while deterioration and limited cooperation made an awake technique impracticable. Sole-agent ketamine sedation facilitated videolaryngoscopy with clinically observed spontaneous breathing; the bolus, a slice of carrot,

Turbinoplasty Versus Medical Treatment for Allergic Rhinitis: A Systematic Review and Meta-Analysis.

OBJECTIVE: Allergic rhinitis is a common chronic condition affecting up to 30% of adults worldwide, with nasal obstruction being the most bothersome symptom. Inferior turbinate hypertrophy is a major contributor to nasal obstruction in allergic rhinitis, and surgical reduction techniques have been developed for cases refractory to medical therapy. This systematic review and meta-analysis evaluated the efficacy of inferior turbinate reduction procedures compared with medical therapy alone in patients with allergic rhinitis. DATA SOURCE: PubMed, Embase, Web of Science, Scopus, and Cochrane Libr

Questionnaire-based evaluation of nasal breathing, oral habits, and myofunctional disorders in children after adenoidectomy.

OBJECTIVES: Questionnaires are widely used in medicine to collect essential patient information, identify disease etiologies, and assess treatment outcomes. In orthodontics, questionnaire-based assessments facilitate the identification of factors associated with dentoalveolar anomalies and myofunctional disorders. Adenoid hypertrophy (AH) is a significant contributor to the development of these conditions. However, inadequate postoperative follow-up may allow myofunctional disorders to persist or progress, despite surgical treatment. A comprehensive evaluation of factors affecting general and

Primary thyroid diffuse large B-cell lymphoma: a case report and focused review of the literature.

Primary thyroid diffuse large B-cell lymphoma (DLBCL) is a rare subset of primary extranodal DLBCL. Thus, there are limited evidence-based specific recommendations regarding its diagnosis and treatment. We present a case report of a patient with relapsed primary thyroid DLBCL, followed by a focused literature review covering key epidemiological, diagnostic, and therapeutic aspects. In 2017, a 74-year-old woman was diagnosed with primary thyroid non-GCB-type DLBCL (Ann Arbor IE, IPI 2) after presenting with dyspnoea, hoarseness, and cough. Treatment with R-CHOP and CNS prophylaxis achieved comp

Pulmonary artery aneurysm associated with a bicuspid pulmonary valve: a case report of an unusual cause of atypical chest pain.

BACKGROUND: Pulmonary artery aneurysm (PAA) is a rare condition, most commonly associated with pulmonary arterial hypertension. Its association with a bicuspid pulmonary valve is exceptionally uncommon. CASE SUMMARY: A 62-year-old woman presented with recurrent chest pain and dyspnoea, with persistently elevated D-dimer levels leading to repeated investigations for pulmonary embolism. Serial imaging demonstrated progressive pulmonary artery dilatation. Multimodality imaging identified a bicuspid pulmonary valve and a 54 mm PAA. Given aneurysm size and persistent symptoms, the patient underwen

Medical Unmet Needs in Uncontrolled Asthma Despite GINA Step 4/5 Treatment in Poland.

BACKGROUND: Asthma is a heterogeneous chronic respiratory disease characterized by symptoms such as wheezing, shortness of breath, cough, and bronchial obstruction confirmed in spirometry. According to the Polish National Health Fund (NHF), in 2023 there were 2.07 million patients with asthma (International Classification of Diseases, ICD-10 codes J45, J46) in Poland. Despite guideline-based treatment recommended by the Global Initiative for Asthma (GINA) at steps 4 and 5, a substantial proportion of patients remain uncontrolled, leading to significant clinical and societal burden. OBJECTIVE:

Monitoring symptoms and performance in rehabilitation: Can we detect COPD exacerbation?

BACKGROUND: In patients with COPD, exacerbations mark disease progression, with significant reduction in quality of life and impact on mortality. Early detection is crucial to mitigate their consequences. The aim of this study was to investigate whether monitoring symptoms and exercise endurance at each pulmonary rehabilitation maintenance session could predict severe exacerbations of COPD. STUDY DESIGN AND METHODS: Prospective multicentre study including 128 stable COPD patients in maintenance pulmonary rehabilitation programmes, in 21 French rehabilitation centres (NCT05492149). Cardiorespiratory and Metabolic Adaptations to a 6-Week High-Intensity Interval Training Program at Different Intensities in Young Soccer Players.

This study compared the effects of high-intensity interval training (HIIT) prescribed at the respiratory compensation point (RCP) versus maximal aerobic speed (MAS) on cardiorespiratory and metabolic outcomes in young soccer players. Eighteen male national-level soccer players (15.7 ± .5 years) were randomly assigned to either the HIIT-RCP or HIIT-MAS group and completed a 6-week training program. The assessments included oxygen uptake (VO2) and running speed at the ventilatory anaerobic threshold and RCP, as well as relative buffering capacity, hypocapnic hyperventilation, blood lactate accum

Initial nebulized budesonide versus a local off-label nebulized dexamethasone protocol in children with institution-defined grade III croup receiving epinephrine: a retrospective cohort study.

BACKGROUND: High-acuity croup requires prompt airway-directed treatment, but evidence comparing nebulized budesonide with a local institutional off-label nebulized-dexamethasone protocol is limited. This internationally unusual route is distinct from standard oral, intramuscular, or intravenous dexamethasone therapy. METHODS: We conducted a single-center retrospective cohort study of children younger than 12 years hospitalized from 1 January 2014 through 31 December 2023. Pretreatment severity was fixed at the first institution-defined Grade III assessment. We compared initial nebulized budes

Acupuncture-Induced Pneumothorax Following Thoracic Acupuncture in a Patient With Ehlers-Danlos Syndrome: A Case Report.

Acupuncture is widely used as an adjunctive therapy for chronic pain and headache disorders and is generally considered a safe procedure when performed by trained practitioners. However, pneumothorax remains one of its most serious, albeit uncommon, complications. Patients with connective tissue disorders, such as Ehlers-Danlos syndrome (EDS), may theoretically be more susceptible to procedure-related complications because of underlying tissue fragility, although reports describing acupuncture-induced pneumothorax in this population are exceptionally rare. We present the case of a young woman
