Acute-Onset Iron Overload Cardiomyopathy in Diamond-Blackfan Anaemia: A Case Report of Rapid Ejection Fraction Decline.

Diamond-Blackfan anaemia (DBA) is a rare inherited bone marrow failure syndrome causing pure red cell aplasia. Patients frequently require long-term red cell transfusions, predisposing them to iron overload. This case describes a patient with DBA with rapid ejection fraction decline secondary to acute-onset iron overload cardiomyopathy. A 28-year-old male patient with known transfusion-dependent DBA and established iron overload and on deferasirox, presented with a one-week history of orthopnoea, dyspnoea and peripheral oedema. Echocardiography demonstrated severe biventricular impairment with

A machine learning approach to respiratory biomechanics: Identifying a PCA-derived membrane-volume coupling pattern in post-COVID pulmonary function.

BACKGROUND: Persistent dyspnoea after COVID-19 may not be fully explained by conventional pulmonary function tests. This retrospective cross-sectional study used secondary data to examine relationships between lung-volume and diffusion-related variables within an exploratory respiratory-biomechanical framework. METHODS: Participant-level data from 572 post-COVID participants and 72 healthy controls were obtained from an open-access multicenter dataset. Four derived relational descriptors and conventional pulmonary variables were entered into principal component analysis (PCA). Covariate-adjus

Isolated granulomatous pulmonary artery vasculitis associated with Hughes-Stovin syndrome: a case report.

BACKGROUND: Hughes-Stovin syndrome (HSS) is an exceptionally rare vasculitis, characterized by pulmonary artery aneurysms with in situ thrombosis and is increasingly regarded as part of the vascular Behcet spectrum. Diagnosis remains challenging due to its rarity, lack of validated diagnostic criteria, and overlap with other vasculitis. CASE SUMMARY: We describe a 71-year-old male who presented with progressive dyspnoea and chest pain. Serial imaging demonstrated rapid enlargement of a main pulmonary artery aneurysm from 5 mm to 68 × 55 mm over 18 months, prompting surgical repair. Histopatho

Non-Contrast MR Pulmonary Angiogram in the diagnosis of Acute Pulmonary Thromboembolism in Chronic Kidney Disease - Preventing Insult to Injury.

Computed tomography pulmonary angiography (CTPA) remains the imaging modality of choice for diagnosing acute pulmonary thrombo-embolism (PTE). However, the evaluation of kidney function should be a mandatory step prior to the administration of any contrast media, as iodinated contrast requires careful risk stratification in patients with pre-existing kidney impairment. We present a 72-year-old male with stage G4 chronic kidney disease (CKD), obesity, diabetes and hypertension with history of acute worsening of dyspnoea. Clinical findings, which included tachycardia, hypoxia and a new diagnosis

Primary thyroid diffuse large B-cell lymphoma: a case report and focused review of the literature.

Primary thyroid diffuse large B-cell lymphoma (DLBCL) is a rare subset of primary extranodal DLBCL. Thus, there are limited evidence-based specific recommendations regarding its diagnosis and treatment. We present a case report of a patient with relapsed primary thyroid DLBCL, followed by a focused literature review covering key epidemiological, diagnostic, and therapeutic aspects. In 2017, a 74-year-old woman was diagnosed with primary thyroid non-GCB-type DLBCL (Ann Arbor IE, IPI 2) after presenting with dyspnoea, hoarseness, and cough. Treatment with R-CHOP and CNS prophylaxis achieved comp

Pulmonary artery aneurysm associated with a bicuspid pulmonary valve: a case report of an unusual cause of atypical chest pain.

BACKGROUND: Pulmonary artery aneurysm (PAA) is a rare condition, most commonly associated with pulmonary arterial hypertension. Its association with a bicuspid pulmonary valve is exceptionally uncommon. CASE SUMMARY: A 62-year-old woman presented with recurrent chest pain and dyspnoea, with persistently elevated D-dimer levels leading to repeated investigations for pulmonary embolism. Serial imaging demonstrated progressive pulmonary artery dilatation. Multimodality imaging identified a bicuspid pulmonary valve and a 54 mm PAA. Given aneurysm size and persistent symptoms, the patient underwen

Monitoring symptoms and performance in rehabilitation: Can we detect COPD exacerbation?

BACKGROUND: In patients with COPD, exacerbations mark disease progression, with significant reduction in quality of life and impact on mortality. Early detection is crucial to mitigate their consequences. The aim of this study was to investigate whether monitoring symptoms and exercise endurance at each pulmonary rehabilitation maintenance session could predict severe exacerbations of COPD. STUDY DESIGN AND METHODS: Prospective multicentre study including 128 stable COPD patients in maintenance pulmonary rehabilitation programmes, in 21 French rehabilitation centres (NCT05492149). At each ses

Sudden cardiac death due to aortic stenosis.

BACKGROUND: Characteristics of subjects with sudden cardiac death (SCD) due to aortic stenosis (AS) are not well defined. Aim of this study was to present clinical and autopsy findings of SCD subjects in whom AS was determined to be the cause of death. METHODS: Study population was based on the Finnish Genetic Study of Arrhythmic Events (Fingesture), a large registry which included 5869 consecutive subjects of autopsy-verified SCD between 1998 and 2017. The cause of death was determined based on medicolegal investigation combining data from medical records, police reports and autopsy. Clinica

Acupuncture-Induced Pneumothorax Following Thoracic Acupuncture in a Patient With Ehlers-Danlos Syndrome: A Case Report.

Acupuncture is widely used as an adjunctive therapy for chronic pain and headache disorders and is generally considered a safe procedure when performed by trained practitioners. However, pneumothorax remains one of its most serious, albeit uncommon, complications. Patients with connective tissue disorders, such as Ehlers-Danlos syndrome (EDS), may theoretically be more susceptible to procedure-related complications because of underlying tissue fragility, although reports describing acupuncture-induced pneumothorax in this population are exceptionally rare. We present the case of a young woman

Giant cardiac hemangioma: case report and literature review.

INTRODUCTION: Primary cardiac tumours are rare, with haemangiomas constituting only 2% of benign cardiac tumours. Although benign, these lesions can compress adjacent cardiac structures, leading to symptoms such as arrhythmias, heart failure, or even sudden death. This study presents a rare case of a giant cardiac haemangioma and provides a review of the current literature on its diagnosis and management. CASE REPORT: A 58-year-old female presented with progressive abdominal pain and dyspnoea. Imaging revealed a large, heterogeneous mass in the pericardial sac measuring 11.9 × 11.7 × 6.7 cm.

Ventilatory drive modulates the temporal evolution of multidimensional dyspnoea during constant-load exercise in healthy adults.

How increased ventilatory drive shapes the temporal evolution and multidimensional experience of exertional dyspnoea remains incompletely understood. This study examined whether experimentally increasing ventilatory drive via dead-space loading (DSL) alters dyspnoea during constant-load exercise. Nine healthy adults (7 male; V̇O2peak 100 ± 16% predicted) performed constant-load cycle ergometry to task failure at 50%Δ between the gas-exchange threshold and V̇O2peak under control (CTRL) and DSL (15% vital capacity) conditions in a randomised crossover design. Dyspnoea intensity, dyspnoea unpleas

Presumed transdermal superwarfarin toxicity in a pest control worker.

A man in his early 60s presented with dyspnoea superimposed on a 2-month to 3-month history of spontaneous mucocutaneous bleeding. Laboratory testing revealed unmeasurably elevated international normalised ratio (INR) and activated partial thromboplastin time. CT of the neck demonstrated supraglottic soft tissue swelling with severe airway narrowing, requiring intubation for airway protection. Further history revealed occupational exposure to rodenticides during pest control work without consistent use of personal protective equipment. Markedly reduced activity of vitamin K-dependent clotting

Two Cases of Right-Sided Pleural Effusion as the Initial Manifestation of Severe Ovarian Hyperstimulation Syndrome.

INTRODUCTION: Severe ovarian hyperstimulation syndrome (OHSS) occurs less frequently due to increased knowledge of potential risk factors and implementation of preventive strategies. Right-sided pleural effusion as a presentation of severe OHSS is rare, and the symptoms can be nonspecific, which may delay diagnosis. However, early recognition is essential, as the condition can deteriorate rapidly and patients often need hospitalisation and supportive treatment. CASE PRESENTATION: We describe two patients with right-sided pleural effusion as the initial manifestation of severe OHSS following c

Intervention with a handmade closed-loop balloon stent for a pulmonary artery fistula not closed by coil.

BACKGROUND: Coronary-pulmonary artery fistulas (CPAF) are rare anomalies that may cause coronary steal and clinical symptoms. Coil embolisation is often effective, but residual flow can persist in large fistulas. CASE: A 70-year-old male with coronary artery disease underwent coil embolisation for a right coronary-pulmonary artery fistula. Residual flow persisted, leading to exertional dyspnoea. Coronary angiography confirmed incomplete closure. Since a vascular plug was unavailable, a closed-loop balloon-stent technique was performed with guideliner support, achieving complete occlusion. Environmental Asbestos Exposure as a Possible Pathway to Early-Onset Pleural Mesothelioma: A Case From Afghanistan and Pakistan to London.

Malignant pleural mesothelioma (MPM) is strongly associated with occupational asbestos exposure and typically presents decades after exposure, in older men. The association of MPM with environmental exposure and genetic susceptibility is less recognised. We report a case of a 40-year-old man presenting in London with dyspnoea, weight loss and a persistent right-sided pleural effusion following hospitalisation for pneumonia. Imaging showed circumferential nodular thickening of the right pleura with lymph node involvement, and histopathology was in keeping with epithelioid-type MPM. With no know

Diagnostic imaging modalities in Dyspnoea and suspected acute heart failure: a systematic review with Meta-analysis.

BACKGROUND: Early identification of pulmonary congestion in patients with dyspnoea is critical for managing acute heart failure. However, the optimal imaging modality remains uncertain. We compared chest radiography, lung ultrasound, computed tomography (CT), and remote dielectric sensing (ReDS) for detecting acute heart failure. METHODS: We conducted a preregistered systematic review and meta-analysis of prospective observational studies in adults with dyspnoea. Medline, Embase, LILACS, and Web of Science were searched on April 9, 2025. Two reviewers independently screened studies, extracted

Factors associated with willingness to quit smoking in a hospital-based smoking cessation programme.

INTRODUCTION: Hospital admissions provide an important opportunity for smoking cessation interventions. However, factors associated with willingness to quit smoking among hospitalised smokers remain underexplored, particularly in the multi-ethnic Asian population. METHOD: In this cross-sectional study of hospitalised smokers referred to an inpatient smoking cessation service between 2021 and 2025, enrolment data on willingness to quit, demographics, and clinical characteristics were analysed using multivariable logistic regression. A clinical feature score was developed to assess dose-respons

Heart failure in chronic kidney disease: what the second universal definition changes.

BACKGROUND: Heart failure and chronic kidney disease (CKD) are biologically entwined and clinically difficult to disentangle. The 2026 Second Universal Definition of Heart Failure reframes heart failure as a syndrome defined by symptoms and signs, objective evidence of structural or functional cardiac abnormality, and a dynamic disease trajectory. Its implications for CKD and dialysis require focused consideration. SCOPE: This review interprets the consensus in a nephrology context. It considers the move beyond rigid left ventricular ejection fraction (LVEF) thresholds; the recognition of at-

Mitral valve regurgitation and emotional dysregulation secondary to long-standing Graves' thyrotoxicosis.

SUMMARY: Graves' disease is common, with an estimated prevalence of 0.2-2% and multiple associated complications. We present a case of severe mitral regurgitation secondary to poorly controlled Graves' disease for 11 years, highlighting the need for definitive treatment. A 30-year-old female attended hospital with palpitations, agitation and generalised pain. Her past medical history included Graves' disease diagnosed in 2014, with poor compliance and engagement with secondary care and medical treatment. Biochemical investigations and clinical examination confirmed thyrotoxicosis (TSH < 0.05 m
