Left ventricular myxoma diagnosed by multimodal cardiac imaging with postoperative recurrence: a case report.

BACKGROUND: Left ventricular myxoma is a rare benign cardiac tumour. Diagnosis relies on multimodal imaging, and postoperative recurrence is uncommon. CASE PRESENTATION: A middle-aged woman with a two-week history of recurrent chest tightness and dyspnoea upon exertion was admitted to our hospital. Transthoracic and transoesophageal echocardiography revealed an irregular, hypoechoic mass (approximately 61 mm × 24 mm) in the left ventricular outflow tract. The mass had a broad base attached to the inferior and posterior walls of the left ventricle, involving the posterior mitral valve leaflet,

[Pulmonary embolism after liposuction].

A previously healthy 35-year-old woman developed recurrent syncope and dyspnoea two days after extensive thigh liposuction for lipedema. CT pulmonary angiography revealed a large saddle embolus with peripheral emboli. Fat embolism was initially suspected; however, imaging findings and laboratory parameters supported a thromboembolic origin. Treatment with anticoagulation was complicated by postoperative bleeding. This case report highlights the importance of considering both fat embolism and thromboembolic pulmonary embolism after liposuction.

Nerandomilast-Associated Pneumonitis With a Positive Rechallenge: A Case Report.

Nerandomilast is an oral preferential phosphodiesterase 4B inhibitor that slows the decline in forced vital capacity in idiopathic pulmonary fibrosis and progressive pulmonary fibrosis. Nerandomilast-associated pneumonitis has not previously been reported. A 33-year-old woman with advanced unclassifiable idiopathic interstitial pneumonia developed fever, dyspnoea, hypoxaemia and new bilateral consolidations and ground-glass opacities 9 days after starting nerandomilast. The drug was withdrawn, and her clinical, inflammatory and radiological findings improved during antimicrobial and corticoste

Determining the association between gender-related factors and the perception of exertional dyspnoea in healthy males and females.

The perception exertional dyspnoea is greater in females than males at absolute, but not relative, levels of minute ventilation (VE). The mechanisms of sex differences in exertional dyspnoea are unclear, but since dyspnoea perception is influenced by psychological, social, and cultural factors, it is possible that gender-related characteristics confound the assessment of sex differences in exertional dyspnoea. We sought to determine the association between gender-related characteristics and dyspnoea during exercise in males and females. Forty-six healthy adults (n=22 males, n=24 females) compl

Neural drive and respiratory muscle effort in emphysema following lung volume reduction.

Introduction Respiratory muscle impairment is common in patients with severe emphysema, primarily driven by lung hyperinflation. Endobronchial valve (EBV) treatment reduces hyperinflation and may restore respiratory mechanics in this patient population. Our aim was to investigate changes in neural drive and respiratory muscle recruitment following EBV treatment. Methods EBV-eligible emphysema patients from our prior study on diaphragm function were prospectively enrolled (NCT04735731). Respiratory muscle activation and timing were assessed using surface electromyography at baseline and 6 weeks

Changes in Symptoms and Lung Function During Three Months of Individually Tailored Inhaled Therapy in Chronic Obstructive Pulmonary Disease: A Prospective Observational Cohort Study.

Background. Randomised controlled trials have established what individual inhaled regimens can achieve in chronic obstructive pulmonary disease (COPD) under controlled conditions, but they recruit selected populations and enforce protocol-driven follow-up. How patients fare on regimens chosen individually for them in everyday care is less well characterised. Objective. To assess the change in symptoms and lung function over three months in a prospective cohort of patients with COPD whose inhaled treatment was tailored to the individual by the attending physician. Methods. Consecutive adults wi

Efficacy and safety of novel nebulized combination of glycopyrronium, formoterol and budesonide in comparison with metered dose inhaler of glycopyrronium, formoterol and budesonide in COPD - A randomized, double-blind, double-dummy clinical trial.

BACKGROUND: Many patients with chronic obstructive pulmonary disease (COPD) are unable to use metered dose inhaler (MDI) or dry powder inhaler effectively and require treatment with nebulized medication. No triple combination is available in nebulized form. METHODS: In this randomized, double-blind study, 314 patients with COPD received either Glycopyrronium 25 μg/Formoterol Fumarate 20 μg/Budesonide 500 μg (GB/FF/BUD Neb) by nebulization twice daily or Glycopyrronium 7.2 μg, Formoterol Fumarate 5 μg, Budesonide 160 μg MDI (GB/FF/BUD MDI) 2 Puffs twice daily for 12 weeks. Comparison of HALP and DECAF in predicting 30 day in hospital mortality in a retrospective cohort study of acute exacerbation of COPD.

BACKGROUND: Acute exacerbations of chronic obstructive pulmonary disease (AECOPD) are associated with substantial short term mortality and healthcare utilization, yet the prognostic utility of the immunonutrition based hemoglobin, albumin, lymphocyte, and platelet (HALP) score in this setting remains unclear, particularly in comparison with the well validated dyspnoea, eosinopenia, consolidation, acidaemia, atrial fibrillation (DECAF) score. METHODS: We conducted a retrospective, single centre observational cohort study of adults presenting to the emergency department (ED) with AECOPD between

Needs among persons with advanced COPD and their informal caregivers: a multi-perspective qualitative study focusing on the potential role of palliative care.

OBJECTIVES: To explore needs among persons with advanced COPD and informal caregivers, and to explore how palliative care can address those needs. BACKGROUND: COPD is a common, progressive disease affecting lung function and represents a leading cause of death. Advanced COPD is associated with a high symptom burden and poor quality of life. Persons with advanced COPD may benefit from palliative care, but its use among persons with COPD remains limited, also in Germany. METHODS: A qualitative, multi-perspective interview study was conducted with persons suffering from advanced COPD, informal

Rheumatic lesions of the pulmonary valve: A systematic review of the literature.

BACKGROUND: Rheumatic pulmonary valve disease accounts for<1% of cases of rheumatic heart disease. Because of its rarity, this valvular disease is often overlooked and underdiagnosed. AIM: To conduct a systematic review of reported cases of rheumatic pulmonary valve disease worldwide. METHODS: Studies reporting confirmed cases of rheumatic pulmonary valve disease, and providing sufficient clinical, diagnostic, therapeutic or follow-up data were included. Duplicate checking, selection, data extraction and quality assessment of the included studies were carried out by two reviewers, independen

Intimal sarcoma of the pulmonary vein with left atrial extension: a case report and diagnostic relevance of MDM2.

Intimal sarcoma is an uncommon, aggressive neoplasm originating in the tunica intima of large vessels. Although most cases arise in the pulmonary artery, primary involvement of the pulmonary veins is exceptional. We present the case of a 52-year-old woman who presented to the emergency department with acute heart failure and dyspnoea. Imaging studies initially suggested an atrial myxoma. Surgical resection of the lesion revealed a proliferation characterised by a relatively monotonous spindle-cell proliferation within the pulmonary vein and a separate zone showing greater pleomorphism, necrosi

Constrictive pericarditis in cystic fibrosis: a case report of an emerging cardiovascular complication successfully managed with off-pump pericardiectomy.

BACKGROUND: Constrictive pericarditis (CP) is rarely reported in people with cystic fibrosis (CF) and may be overlooked because dyspnoea, fatigue, oedema, and reduced exercise tolerance overlap with pulmonary exacerbation. As survival improves with highly effective CF transmembrane conductance regulator modulator therapy, cardiovascular causes of cardiorespiratory symptoms may become increasingly relevant in adults with CF. This case highlights the importance of recognizing CP in this population and individualizing surgical decision-making. CASE SUMMARY: A 33-year-old woman with homozygous F5

Cardiovascular Phenotypes in Women with Sjögren Disease: Hormonal, Immune, and Clinical Perspectives.

BACKGROUND: Sjögren disease (SjD) is a systemic autoimmune disorder with a marked female predominance and an increasingly recognised, although incompletely characterised, cardiovascular (CV) burden. This review examines the biological, clinical, and gender-related determinants of CV involvement in women with SjD. METHODS: We conducted a structured narrative review of PubMed/MEDLINE covering the period from January 2010 to July 2026, supplemented by backward citation searching to identify relevant earlier studies. Original SjD-specific studies were prioritised, while systematic reviews, meta-a

The Bird's Nest Sign as an Initial Presentation of Primary Pulmonary B-Cell Non-Hodgkin Lymphoma With Plasmacytic Differentiation.

The bird's nest sign (BNS) has been described as a sign on chest computed tomography (CT) in various pulmonary diseases. We report a rare case of primary pulmonary B-cell non-Hodgkin lymphoma with plasmacytic differentiation presenting with BNS, mimicking an invasive fungal disease at first. A 61-year-old Vietnamese man with a history of idiopathic thrombocytopenia and splenectomy 3 years prior to admission presented with prolonged dry cough, mild dyspnoea on exertion and chest discomfort for 3 weeks. His laboratory tests were unremarkable, but his chest CT showed bilateral consolidation with

A machine learning approach to respiratory biomechanics: Identifying a PCA-derived membrane-volume coupling pattern in post-COVID pulmonary function.

BACKGROUND: Persistent dyspnoea after COVID-19 may not be fully explained by conventional pulmonary function tests. This retrospective cross-sectional study used secondary data to examine relationships between lung-volume and diffusion-related variables within an exploratory respiratory-biomechanical framework. METHODS: Participant-level data from 572 post-COVID participants and 72 healthy controls were obtained from an open-access multicenter dataset. Four derived relational descriptors and conventional pulmonary variables were entered into principal component analysis (PCA). Covariate-adjus

Isolated granulomatous pulmonary artery vasculitis associated with Hughes-Stovin syndrome: a case report.

BACKGROUND: Hughes-Stovin syndrome (HSS) is an exceptionally rare vasculitis, characterized by pulmonary artery aneurysms with in situ thrombosis and is increasingly regarded as part of the vascular Behcet spectrum. Diagnosis remains challenging due to its rarity, lack of validated diagnostic criteria, and overlap with other vasculitis. CASE SUMMARY: We describe a 71-year-old male who presented with progressive dyspnoea and chest pain. Serial imaging demonstrated rapid enlargement of a main pulmonary artery aneurysm from 5 mm to 68 × 55 mm over 18 months, prompting surgical repair. Histopatho
