Post-marketing safety evaluation and objective benefit-risk assessment of sublingual immunotherapy tablets in allergic rhinitis.

To evaluate the post-marketing safety profile and benefit-risk balance of FDA-approved sublingual immunotherapy (SLIT) tablets for allergic rhinitis. A retrospective pharmacovigilance analysis was conducted using FAERS data (2014-2025). Reports including Oralair®, Grazax®/Grastek®, Ragwitek®, and Odactra® as suspected drugs were analyzed descriptively. A total of 1509 cases were identified, mostly in adults. Most adverse events (59.17%) were non-serious. The most common reactions were throat irritation, oral pruritus, dyspnoea, pruritus, and throat tightness. Serious outcomes were uncommon, in

Invasive haemodynamic validation of an artificial intelligence echocardiographic tool for detecting heart failure with preserved ejection fraction.

INTRODUCTION: To externally validate an FDA-approved artificial intelligence (AI) tool for detecting heart failure with preserved ejection fraction (HFpEF) using echocardiographic video clips, comparing its performance to invasive haemodynamic criteria in a real-world referral cohort with unexplained dyspnoea. METHODS: We retrospectively analysed 47 patients who underwent transthoracic echocardiography (TTE) and right heart catheterization (RHC), including 28 with both rest and exercise haemodynamics. The AI model evaluated apical 4-chamber video data and classified outputs as HFpEF, no HFpEF

Chronic thromboembolism phenotypes after acute pulmonary embolism: identification with the SEARCH algorithm.

BACKGROUND: Chronic dyspnoea and exercise impairment are common after acute pulmonary embolism (PE) but poorly characterised. We performed a prospective observational study to validate an algorithm to diagnose discrete post-PE outcomes, including chronic thromboembolism phenotypes. METHODS: Six pulmonologists independently reviewed records from 150 consecutive patients ≥3 months after acute PE who had undergone a stepwise diagnostic algorithm (SEARCH), which incorporates Symptom screening, cardiopulmonary Exercise testing, pulmonary Arterial perfusion scans, Resting echocardiography, Confirma

Parasternal electromyography in participants with mild or moderate chronic obstructive pulmonary disease in primary care: cohort study to assess technical and clinical application.

BACKGROUND: Neural respiratory drive (NRD) measurement, reflecting the balance between respiratory muscle load and capacity, is quantified using surface parasternal electromyography (EMGpara). EMGpara tracks recovery from severe exacerbations of chronic obstructive pulmonary disease (COPD). Among stable COPD participants, we hypothesised the existence of a relationship between NRD, breathlessness and airway obstruction. STUDY AIMS: (1) assessing the feasibility of measuring EMGpara in COPD participants with forced expiratory volume in 1 s (FEV1) ≥50% predicted in primary care; (2) investigati

Acute Airway Crisis in Mucopolysaccharidosis VI: Management Challenges.

Background and Clinical Significance: Mucopolysaccharidosis type VI is a rare lysosomal storage disorder due to arylsulfatase B enzyme deficiency, leading to progressive multisystem disease and complex airway. Acute respiratory infections can precipitate airway embarrassment. A structured treatment guideline is currently lacking. We present a 7-year-old MPS VI male with respiratory distress, highlighting the challenges in management. Case Presentation: Case review focusing on clinical presentation, imaging findings, and multidisciplinary decision-making during acute deterioration. A child diag

Exercise-induced atrioventricular block in a patient with a pacemaker: a functional consequence of AV search algorithms-a case report.

BACKGROUND: Algorithms designed to promote intrinsic atrioventricular (AV) conduction in dual-chamber pacemakers aim to reduce unnecessary right ventricular pacing and its long-term adverse effects. The AV Search+ algorithm periodically prolongs the AV delay to facilitate native conduction and is particularly useful in non-pacemaker-dependent patients. However, under certain conditions, such algorithms may lead to unexpected disturbances in AV synchrony, especially during exercise. CASE SUMMARY: A 65-year-old woman with symptomatic second-degree AV block (Mobitz II) underwent dual-chamber pac

Dysphagia, aspiration, and respiratory problems in people with chronic respiratory illness.

PURPOSE: Dysphagia, aspiration, and respiratory dysfunction are interlinked with shared neural control and biomechanical demands. Recent respiratory-swallow coordination research and scrutiny of long-standing dysphagia interventions necessitate reappraisal of aspiration risk, airway protection, and clinical decision making. These issues are particularly relevant in chronic respiratory illness and palliative care, where dyspnoea, frailty, and quality-of-life considerations strongly influence eating and drinking decisions. RECENT FINDINGS: Breathing and swallowing are coordinated by overlapping

Exercise cardiac magnetic resonance biventricular volumetric reserve in heart failure with preserved ejection fraction.

BACKGROUND AND AIMS: Heart failure with preserved ejection fraction (HFpEF) is increasingly recognized as a syndrome of reserve dysfunction. However, integrated assessment of biventricular (LV/RV) volumetric reserve under physiological stress remains underexplored. We aimed to investigate whether exercise cardiac magnetic resonance (Ex-CMR) can reveal distinct volumetric reserve profiles across the HFpEF spectrum. METHODS: In this retrospective analysis of a prospective observational, multicentre study, supine ergometer Ex-CMR was performed in HFpEF patients across early to advanced stages (s

The impact of vaping and smoking on nicotine intake and toxicant exposure among youth in England compared with youth in North America.

BACKGROUND: Youth vaping prevalence varies across countries and may be related to differing regulations/products. The emergence of cheap disposable vapes and high-concentration nicotine salts heightened concerns related to youth's ease of access, dependence and potential health risks. OBJECTIVES: We examined youth in England versus Canada and the United States and: how patterns of vaping/smoking varied, given the countries' different regulatory frameworks nicotine and potential toxicant exposure in youth who vape, smoke or do neither in youth who use salt and free-base nicotine respiratory sy

Health-related quality of life (HRQoL) of women with breast cancer undergoing treatment in a tertiary care centre in India.

INTRODUCTION: Breast cancer in India accounts for 178361 of all cancers (Globocan 2020 report). Treatment causes financial, social, and psychological stress, impacting patients’ quality of life (QoL). Multiple studies have reported QoL among survivors, however few studies focused on patients undergoing treatment in India. Hence, the present study aimed to measure the QoL and its influencing factors associated with breast cancer undergoing various treatments in the oncology department of a tertiary care hospital in India. METHODS: A hospital-based cross-sectional study (Nov 2022 - Jan 2023) use

Vegetation on the chordae and papillary muscle of the mitral valve: a diagnostic and therapeutic enigma.

Vegetations on the high-pressure side of the mitral valve, particularly the chordae tendineae, are rare. We report two cases of infective endocarditis (IE) with atypical presentations. Case 1: A 34-year-old male with pulmonary aspergilloma, who presented with fever and respiratory distress. Blood cultures grew Staphylococcus species, and echocardiography revealed a 16 × 8 mm vegetation on the mitral valve chordae. Surgery was deferred due to disseminated fungal infection, and the patient improved with antimicrobial therapy. Case 2: A previously healthy 36-year-old male presented with fever, vo

[Oxygen in medicine - basics, applications and risks].

Oxygen is a central component in the treatment of acute and chronic diseases associated with hypoxemia. Nevertheless, in everyday clinical practice it is often administered reflexively, even when documented or clearly suspected hypoxemia - the most important prerequisite for appropriate oxygen therapy - is absent. Dyspnea alone is not an indication, as patients with breathlessness but without hypoxaemia ge-nerally do not benefit from oxygen administration. Controlled titration with clearly defined target ranges is essential in order to avoid risks such as hypercapnia, coronary and cerebral vas

Carnosine augments the efficacy of pulmonary rehabilitation in COPD: insights from a preclinical model.

Pulmonary rehabilitation (PR) improves exercise tolerance and dyspnoea in chronic obstructive pulmonary disease (COPD) but does not address the elevated cardiovascular disease (CVD) risk. Vascular dysfunction, an early CVD feature, is driven by oxidative stress-a shared pathogenic mechanism in COPD and CVD. Carnosine, a bioactive dipeptide with antioxidant and pH-buffering properties, may enhance muscle performance and protect against vascular injury. The present study sought to determine whether carnosine supplementation combined with exercise training could modify early CVD features in a pre

Assessing the impact of proton versus photon therapy on health-related quality of life in lung cancer.

BACKGROUND AND PURPOSE: A more conformal dose deposition of intensity-modulated proton therapy (IMPT) may enhance health-related quality of life (HRQoL) through reduced radiation-induced toxicities, versus photon-based intensity-modulated radiotherapy (IMRT). However, most patients are selected for IMPT based on relative reduction in mean heart dose and correlated 2-year mortality. A HRQoL difference would thus not necessarily be expected. This study evaluates HRQoL outcomes in patients with lung cancer treated with IMPT versus IMRT. MATERIALS AND METHODS: Patients considered for IMPT via the

Case report: primary pericardial tumour-challenges in imaging diagnosis and management.

BACKGROUND: Primary pericardial tumours are exceedingly rare and frequently misdiagnosed due to nonspecific clinical presentations. They may initially manifest through cardiovascular complications such as pericardial effusion, arrhythmias, or embolic events, creating significant diagnostic and management challenges. CASE SUMMARY: We report a 62-year-old female with a history of dyslipidaemia who was admitted for progressive dyspnoea, mild chest pain, and palpitations. Cardiovascular evaluation revealed new-onset atrial fibrillation, large pericardial effusion, and bilateral pleural effusions.
