A Prehospital Decision Support Tool for the Diagnosis of Acute Heart Failure: The RAPID-CHF Score.

AIMS: Diagnostic uncertainty is a major barrier to the timely treatment of heart failure (HF) in the prehospital setting. We aimed to develop and validate a decision support tool using readily available clinical variables to predict the probability of HF among dyspnoeic patients transported by emergency medical services (EMS). METHODS AND RESULTS: A population-based cohort of all adults transported by EMS for dyspnoea in Victoria, Australia was chronologically split into derivation (2015-2017) and temporal validation (2018-2019) cohorts. Two models were developed: (1) a full multivariable log

Evaluating a Digital Patient Reported Outcome Measure for Long COVID in a Community Rehabilitation Service.

BACKGROUND: Long COVID presents with a wide range of persistent symptoms which can significantly affect daily functioning and increases pressure on already stretched National Health Service. A Digital Patient-Reported Outcome Measure application incorporating 4 outcome measure scales (COVID-19-Yorkshire Rehabilitation Scale, EuroQol 5 Dimension Scale, Modified Fatigue Impact Scale, and Medical Research Council Dyspnoea Scale) was introduced into a Long COVID community rehabilitation pathway in 2021. However, there was little known about its acceptability, uptake, or user experience. METHODS:

Unmasking platypnoea-orthodeoxia syndrome: a systematic review of the pathophysiology, clinical spectrum and outcomes of percutaneous intracardiac shunt closure.

BACKGROUND: Platypnoea-orthodeoxia syndrome (POS) is a rare but underdiagnosed cause of positional dyspnoea and hypoxaemia. It results from right-to-left interatrial shunting through a patent foramen ovale (PFO) or atrial septal defect (ASD), often precipitated by anatomical or functional distortions. OBJECTIVES: We systematically characterised the clinical spectrum, anatomical correlates and procedural outcomes associated with percutaneous closure in patients diagnosed with intracardiac POS. METHODS: A systematic search of PubMed, Embase and MEDLINE databases was conducted. Case reports, ca

Fragile immunity, fatal foe: A cluster of Pneumocystis jirovecii pneumonia cases in children with haematological malignancies on chemotherapy.

BACKGROUND: Pneumocystis jirovecii causes a potentially fatal interstitial pneumonia, especially in immunosuppressed hosts. It was the most common cause of death in children with leukaemia, before the introduction of prophylaxis. AIMS: In this study, we evaluated the clinical and demographic characteristics of proven PJP cases in children with haematological malignancy on chemotherapy. The study also determined the association of PJP characteristics with various phases of chemotherapy. METHODS: This was a retrospective study conducted at a tertiary care teaching hospital in Delhi. Cases incl

The burden of COVID-19 in hospitalized people with diabetes mellitus in Brazil: Insights from four years of the pandemic.

AIMS: Coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), is more severe in people with diabetes mellitus due to immune dysfunction, exacerbated inflammation and increased risk of co-morbidities and mortality. In this context, this study aims to analyse the epidemiological profile of hospitalized people with diabetes mellitus and COVID-19 in Brazil over 4 years of the pandemic. METHODS: The epidemiological analysis was conducted using data from the Open-Data-SUS (Sistema Único de Saúde, Brazilian Unified Health System) platform (Brazili

Pleuritis Induced by Brigatinib During the Treatment of Non-Small Cell Lung Cancer.

This report describes the first documented case of brigatinib-induced pleuritis in a patient with ALK-positive non-small cell lung cancer. A 77-year-old woman developed dyspnoea, fever, elevated inflammatory markers, and bilateral pleural effusion shortly after starting brigatinib as third-line therapy. Pleural fluid analysis showed lymphocyte-predominant exudative effusion without evidence of malignancy or infection, suggesting drug-induced pleuritis. Discontinuation of brigatinib led to rapid clinical and radiographic improvement. Although disease progression later occurred, pleuritis did no

Comparing the effectiveness, cost-effectiveness and implementation of low-dose oral modified release morphine in people with chronic breathlessness: a synopsis of a RCT and process evaluation.

BACKGROUND: Chronic breathlessness is common and disabling in chronic medical conditions. There is good rationale to use opioids from neuroscience and exercise laboratory studies. OBJECTIVES: We assessed the 56-day effectiveness, cost-consequences and safety of long-acting oral morphine for breathlessness in people with cardio-respiratory diseases or cancer. METHODS: We conducted a parallel-group, randomised, placebo-controlled trial in 11 UK outpatient services, comparing 10-20 mg daily oral morphine with placebo (and blinded laxative) in people with moderate to severe breathlessness, with

Addressing Unmet Needs in the Management of Small Cell Lung Cancer: Treatment of Brain Metastases, Quality of Life, and the Role of Real-World Evidence.

Small cell lung cancer (SCLC) is a highly aggressive neuroendocrine tumor. Although patient survival has been significantly improved by immune checkpoint inhibitors combined with chemotherapy, three major unmet needs remain in SCLC management. First, while prophylactic cranial irradiation (PCI) can reduce the risk of brain metastases, its neurotoxicity limits its use, particularly in elderly patients or those with neurological deficits. Whole-brain radiotherapy (WBRT) remains the primary approach for brain metastases, and hippocampal-avoidance WBRT (HA-WBRT) combined with neuroprotective agent

Short- and Long-Term Mortality in Patients Hospitalized for Dyspnoea with Acute Heart Failure, Respiratory Infection, or Both: Insights from the PARADISE Cohort.

BACKGROUND: Acute heart failure (AHF) and respiratory infection (RI) frequently coexist, with the latter commonly regarded as a trigger of AHF decompensation. However, the independent and combined prognostic impact of these conditions on survival is not well studied. We therefore assessed the association of AHF and RI, both separately and in combination, with subsequent mortality. METHODS: Patients discharged with diagnoses of AHF, RI, or both were identified from the PARADISE study, a large cohort of patients hospitalised for acute dyspnoea. Associations with in-hospital and post-discharge m

Rice Bran Arabinoxylan Compound and Tryptophan Metabolism on Quality of Life of Cancer Patients: A Secondary Analysis of the RBAC-QoL Study.

Tryptophan metabolism, with its breakdown along the kynurenine pathway, is linked to the diminishing quality of life (QoL) of cancer patients. Rice bran arabinoxylan compound (RBAC) is a plant-based immunomodulator shown to improve the global QoL and functioning beyond the placebo during systemic cancer treatment in a randomised-controlled trial (RBAC-QoL study). However, whether RBAC improved QoL through the tryptophan pathway, and the role of tryptophan and its kynurenine metabolite in these QoL outcomes, were not previously investigated. In this study, serum tryptophan and kynurenine were a

The construct validity of real-world digital mobility outcomes in people with COPD.

BACKGROUND: Recent advances in wearable technologies make it possible to accurately quantify real-world mobility performance through technically validated digital mobility outcomes (DMOs). The aim of the present study was to evaluate the construct validity (convergent, divergent, and known-groups validity) of 24 DMOs quantifying walking activity (amount and pattern) and gait (pace, rhythm and bout-to-bout variability) in people with COPD. METHODS: Part of the Mobilise-D observational cohort study, people with COPD, recruited from seven European sites, wore an activity monitor for 7 days durin

Prevalence and outcomes of anxiety and depression in fibrotic interstitial lung disease: a registry-based analysis.

Anxiety and/or depression are common in fibrotic interstitial lung disease (ILD) but with unclear treatment implications. A prospective multicentre registry of 4009 patients was used to evaluate the association of depression and/or anxiety with clinical features and outcomes in fibrotic ILD.Anxiety and/or depression was associated with female sex and worse dyspnoea and cough but not lung function or ILD medication intolerance. Having anxiety and/or depression was associated with greater odds of patients declining referral for lung transplant assessment.Patients with anxiety and/or depression e

Lung Ultrasound Abnormalities and LUS Score After COVID-19 Pneumonia: Determinants and Associations with Dyspnoea in a Prospective Cohort.

Background/Objectives: The clinical determinants and functional relevance of persistent lung ultrasound (LUS) abnormalities after COVID-19 pneumonia remain poorly characterized. We aimed to identify determinants of qualitative LUS abnormalities and global lung involvement assessed by the LUS score, and to evaluate their association with persistent dyspnoea. Methods: We conducted a prospective observational study that included 261 patients who were hospitalized for COVID-19 pneumonia and were assessed 1-6 months after discharge. A standardized 14-zone LUS protocol was used to assess qualitative

Beyond Breathlessness Intensity: A Prospective Psychometric Validation of the Multidimensional Dyspnea Profile in Heart Failure with Reduced and Mildly Reduced Ejection Fraction.

Background/Objectives: Dyspnoea in heart failure with reduced or mildly reduced ejection fraction (HFrEF/HFmrEF) is multidimensional, yet conventional unidimensional scales do not capture its sensory and affective components. The Multidimensional Dyspnea Profile (MDP) addresses this gap; however, its psychometric properties have not been established in a dedicated HFrEF/HFmrEF cohort. We assessed structural validity, internal consistency, test-retest reliability, and construct validity of the MDP using COSMIN methodology. Methods: In this prospective, single-centre psychometric validation stud

Chair-based versus standard home exercise programs in people with COPD: A randomized controlled trial.

BACKGROUND: Chair-based exercise may represent a practical alternative for individuals with chronic obstructive pulmonary disease (COPD) who experience balance limitations, fatigue, or difficulty performing standing exercises. OBJECTIVES: To compare the effects of chair-based and standard home-based exercise programmes in people with COPD. METHODS: In this randomized controlled trial, 64 clinically stable patients with COPD were allocated to a Standard Exercise Group (SGr,n = 32) or a Chair-Based Exercise Group (ChGr,n = 32). Both groups completed an 8-week home-based exercise programme (5 d

Peritoneal Lymphomatosis Mimicking Advanced Ovarian Malignancy: A Diagnostic Challenge in Diffuse Large B-Cell Lymphoma.

Diffuse large B-cell lymphoma (DLBCL) may rarely present as peritoneal lymphomatosis, closely mimicking peritoneal carcinomatosis on imaging and creating significant diagnostic uncertainty. We report the case of an 82-year-old woman presenting with progressive dyspnoea, fatigue, and systemic symptoms following treatment for presumed community-acquired pneumonia. Contrast-enhanced computed tomography demonstrated ascites, diffuse peritoneal thickening, omental involvement, and widespread lymphadenopathy, initially raising suspicion for advanced ovarian malignancy, supported by an elevated CA125
