Survival impact of lung transplantation for COPD.

Chronic obstructive pulmonary disease (COPD) is the primary indication for lung transplantation (LTx), but survival benefit is still under debate. We analysed the survival impact of LTx in COPD with a new approach, using the BODE (body mass index, airway obstruction, dyspnoea, exercise capacity) index. We retrospectively reviewed 54 consecutive lung transplants performed for COPD. The pre-transplant BODE score was calculated for each patient and a predicted survival was derived from the survival functions of the original BODE index validation cohort. Predicted and observed post-transplant surv

Interval versus continuous training in individuals with chronic obstructive pulmonary disease--a systematic review.

BACKGROUND: In patients with chronic obstructive pulmonary disease (COPD), interval exercise has gained recent attention as a possible means of achieving greater physiological training effects compared with continuous exercise. The primary aim of this systematic review was to compare the effects of interval versus continuous training on peak oxygen uptake, peak power, 6 minute walk test (6MWT) distance and health-related quality of life in individuals with COPD. METHODS: Randomised controlled trials comparing the effects of interval versus continuous training in patients with COPD were identi

Quantification of dyspnoea using descriptors: development and initial testing of the Dyspnoea-12.

RATIONALE: Dyspnoea is a debilitating and distressing symptom that is reflected in different verbal descriptors. Evidence suggests that dyspnoea, like pain perception, consists of sensory quality and affective components. The objective of this study was to develop an instrument that measures overall dyspnoea severity using descriptors that reflect its different aspects. METHODS: 81 dyspnoea descriptors were administered to 123 patients with chronic obstructive pulmonary disease (COPD), 129 with interstitial lung disease and 106 with chronic heart failure. These were reduced to 34 items using

Late presentation of aorto-right ventricular fistula and associated aortic regurgitation following penetrating chest trauma.

We present a case in which a 39-year-old man was assaulted in 2002 and sustained penetrating chest trauma. He was treated conservatively, and presented six years later with exertional dyspnoea and a continuous murmur. Echocardiography detected an aorta-right ventricular fistula and an important aortic regurgitation. The aorta-right ventricular fistula was repaired and the aortic valve was replaced with a mechanical prosthesis. Traumatic aorto-right ventricular fistulas and aortic regurgitation are often masked by the primary injury and may take months or years to become clinically evident.

Early recognition of heart failure in patients with diabetes type 2 in primary care. A prospective diagnostic efficiency study. (UHFO-DM2).

BACKGROUND: We hypothesize that the prevalence of unknown heart failure in diabetic patients aged 60 years and over is relatively high (15% or more) and that a cost-effective strategy can be developed to detect heart failure in these patients. The strategy is expected to include some signs and symptoms (such as dyspnoea, orthopnoea, pulmonary crepitations and laterally displaced apical beat), natriuretic peptide measurements (Amino-terminal B-type natriuretic peptide) and possibly electrocardiography. In a subset of patients straightforward echocardiography may show to be cost-effective. With

Pulmonary manifestations of ankylosing spondylitis treated as pulmonary tuberculosis: a case report and review of literature.

A 45 year old trader presented with history of persistent productive cough, progressively increasing dyspnoea, malaise and fever of 6 months prior to presentations. He also complained of severe lower backache and stiffness that radiated to both legs of 5 years duration. Chest radiograph revealed left apical fibrosis, coarse, linear shadows with cavities. There was also super infection with aspergilloma in the left apical region. The sputum AAFB was negative. Despite the fact that the patient complained of lower backache and stiffness, the plain radiograph of the affected spine was not requeste

[Interstitial pneumonitis as an adverse effect of thalidomide].

A 67-year-old man was admitted to the hospital with symptoms of progressive dyspnoea. For 2 months he had received second-line treatment with dexamethasone and thalidomide for a multiple myeloma. Physical examination revealed a tachypnoeic patient and arterial blood gas analysis revealed a respiratory alkalosis and severe hypoxaemia. A high-resolution CT scan showed diffuse ground glass opacities in both lungs. Pulmonary function testing indicated severe diffusion capacity impairment. Bronchoalveolar lavage and cultures excluded the possibility of an infectious agent. The thalidomide treatment

Cardiac surgery in nonagenarians: single-centre series and review.

BACKGROUND: Cardiac surgery is widely believed to be an excessively high-risk intervention for very elderly patients with coronary artery or valvular disease. However, as life expectancy and the prospect of sustained quality of life into older age increase, this assumption should be challenged so that surgery is not denied to patients who may derive significant symptomatic benefit with acceptable levels of operative risk. OBJECTIVE: To evaluate outcomes from cardiac surgery in nonagenarian patients. DESIGN: Analysis of prospectively collected single-centre data and review of outcomes reporte

Masked diastolic dysfunction caused by exercise testing in hypertensive heart failure patients with normal ejection fraction and normal or mildly increased LV mass.

INTRODUCTION: It is not clear whether in patients with the clinical suspicion of heart failure with normal ejection fraction (HFNEF) exercise assessment of diastolic function may help to unmask abnormalities not detected by resting measurements. METHODS AND RESULTS: A combined radionuclide angiographic and haemodynamic exercise study was performed to confirm definite diastolic dysfunction in hypertensive patients with exertional dyspnoea and no other detectable cause of their symptoms. Only patients with normal baseline left ventricular (LV) systolic and diastolic function parameters as evalu

Nasogastric tube syndrome: a case report.

Nasogastric tube syndrome (NTS) is an uncommon complication of an indwelling nasogastric tube. Ulceration and infection in the posterior cricoid region causes dysfunction in the abduction of the vocal cords and may seriously compromise patients' airway. This pathology should be considered in patients with prolonged nasogastric intubation who start with moderate pharyngeal pain. We report a case of a 70 year-old woman, admitted to the Neurology department of our centre due to an ischaemic cerebral accident who presented acute dyspnoea requiring emergency tracheotomy following prolonged nasogast

A rare case of suture material obstructing the closure mechanism of a prosthetic aortic valve: a case report.

Prosthetic aortic valve dysfunction presenting as aortic regurgitation is a complication of mechanical valve replacement. We describe a case of late valve dysfunction caused by an annular suture of excessive length obstructing the closure mechanism of a bileaflet prosthetic valve. We present this rare cause of valve dysfunction in an 80-year-old male patient who presented with haemolysis and dyspnoea. At the time of operation it was found that a long vertically positioned annular valve suture was interfering with the normal closure mechanism of one of the prosthetic leaflets causing eccentric r

Pulmonary rehabilitation in elderly patients with chronic obstructive pulmonary disease.

Elderly patients with chronic obstructive pulmonary disease (COPD), neglected in terms of pulmonary rehabilitation some decades ago, have nowadays evidence-based recommendations to attend it. There are no serious limitations excepting the locomotor handicap (neurological and musculoskeletal disability), severe cardiovascular and cognitive impairment. Multiple outcomes are expected to show up, as improvement of cardiovascular fitness and exercise capacity, relieving symptoms as dyspnoea, depression and anxiety. Meeting people like them on pulmonary rehabilitation premises, with same limitations

[Cryptogenic organizing pneumonia--case report].

INTRODUCTION: Cryptogenic organising pneumonia is a particular form of inflammatory and fibroproliferative lung disease. The disease onset is subacute with cough, dyspnoea, fever, weight loss, and elevation of biological inflammatory markers. Chest imaging usually shows multifocal alveolar opacities predominating in the subpleural regions. Lung biopsy reveals budding connective tissue filling the distal airspaces. CASE OUTLINE: A 57-year-old electrician complaining of cough, dyspnoea, and fatigue was diagnosed with pneumonia and treated with antibiotics and antihistaminics. After clinical and

A Dutch case of a takotsubo cardiomyopathy after pacemaker implantation.

An 83-year-old female patient with symptomatic atrial fibrillation was referred to the Department of Cardiology for a scheduled electrocardioversion. Because of a junctional escape rhythm after the electrocardioversion she received a DDD pacemaker which was complicated by dyspnoea and ST-segment elevations in the inferior and precordial leads. Because of suspicion of an acute myocardial infarction she was transferred to a PCI centre. The coronary angiogram showed no abnormalities. In the initial phase, an echocardiogram was performed. The echocardiogram showed apical akinesis and a reduced lef

[Stress cardiomyopathy, a topical cardiac disorder].

Stress cardiomyopathy (Tako-Tsubo, Broken Heart syndrome, or apical ballooning syndrome) was recently recognized as a distinct clinical entity. The aims of this review are to define this acute and reversible cardiomyopathy and to list its major clinical, biological and angiographic features. We performed a Medline scan for all relevant case series. The studies thus identified suggest that the apical ballooning syndrome accounts for 2% of ST-elevation infarcts, mainly affects women, and occurs after major emotional or physical stress. Most patients present with chest pain and dyspnoea, cardioge

Light chain deposition disease as a rare cause of restrictive cardiomyopathy.

We report an unusual case of a 47-year-old Caucasian woman who presented with severe dyspnoea as a manifestation of restrictive cardiomyopathy, found to be due to myocardial deposition of kappa light chains. Non-routine specific immunofluorescence stainings of endomyocardial biopsy specimens were key for the diagnosis of myocardial light chain deposition disease. We discuss non-amyloidotic cardiac immunoglobulin deposition disease in contrast to cardiac amyloidosis.
