Effect of dyspnoea-oriented hypnosis as an adjunct to pulmonary rehabilitation on the affective dimension of dyspnoea and physical activity level: a randomised controlled trial.

BACKGROUND: Pulmonary rehabilitation has limited long-term effects on physical activity levels in patients with COPD. Dyspnoea-related anxiety, part of the affective dimension of dyspnoea, is a major barrier to physical activity. This study assessed whether hypnosis as an adjunct to standard pulmonary rehabilitation could improve the affective dimension of dyspnoea, as well as physical activity levels, 6 months after rehabilitation. METHODS: In a randomised, open-label trial (ClinicalTrials.gov: NCT04010825), 106 COPD patients were enrolled to receive either conventional 4-week inpatient pulm

Primary ovarian diffuse large B-cell lymphoma.

Primary ovarian lymphoma is a rare malignancy, representing <1% of ovarian neoplasms and a small subset of extranodal lymphomas, with diffuse large B-cell lymphoma (DLBCL) being the most frequent subtype. Diagnosis is often delayed because symptoms overlap with more common gynaecological or infectious conditions. We report a woman in her 20s presenting with hoarseness, exertional dyspnoea and low-grade fever. Imaging showed mediastinal lymphadenopathy, and repeated biopsies revealed necrotising granulomatous inflammation without malignancy. Persistent symptoms prompted fluorodeoxyglucose (FDG)

Body Composition and Muscle Function in Bronchiectasis: A Comparative Longitudinal Study.

BACKGROUND AND OBJECTIVE: Adults with bronchiectasis often present with altered body composition and muscle strength, yet prognostic value of peripheral muscle strength are not well understood. This study compared body composition and muscle function between adults with bronchiectasis and healthy controls and examined whether peripheral muscle strength estimates one-year clinical outcomes. METHODS: Adults with HRCT-confirmed bronchiectasis and controls underwent assessments including DXA (body composition), dynamometry (leg and shoulder strength), and core endurance tests. Participants with b

The effect of AI-assisted bedside echocardiography on inpatient care: a prospective trial.

AIMS: Focused cardiac ultrasound (FoCUS) can yield valuable information for decision-making. However, it is limited by the skills required to acquire and interpret high-quality images. Machine learning algorithms can help mitigate this gap by providing guidance for optimal image acquisition and interpretation. We aimed to evaluate the impact of an artificial intelligence (AI) assisted, FDA-cleared FoCUS platform on clinical decision-making. METHODS AND RESULTS: This was a prospective trial with pre and post sequential allocation, conducted in two internal medicine departments. During the firs

Iatrogenic left circumflex artery aneurysm with fistulization to the coronary sinus following mitral valve replacement: a rare surgical complication case report.

BACKGROUND: Coronary artery aneurysm with arteriovenous fistula is extremely rare. Here we present a novel iatrogenic case of a left circumflex coronary artery (LCX) to coronary sinus fistula after mitral valve replacement (MVR). CASE SUMMARY: A 47-year-old woman with a history of MVR was diagnosed with an iatrogenic aneurysmal fistula from the LCX to the coronary sinus, causing dyspnoea. Surgical intervention included degenerated mitral and aortic valve replacement, tricuspid valve repair, and fistula ligation. Post-operative recovery was uneventful, with improved symptoms and stable cardiac

Evaluation of multimodal large language models for pneumothorax assessment in real-world clinical scenarios.

INTRODUCTION: Pneumothorax is a potentially life-threatening clinical condition requiring rapid diagnosis and intervention. It may present with symptoms such as dyspnoea, cough, chest pain, tachycardia, and tachypnoea. Heavy workload and technical limitations of chest radiographs may necessitate support for clinicians during the evaluation process. Recently, large language models (LLMs) have increased interest in using their ability to process both text and medical images as supportive tools in diagnostic processes. This study analyzed the performance of two widely used multimodal LLMs in diag

Clinical significance of diastolic wall strain in heart failure with preserved ejection fraction: A cross-sectional observational analysis.

BACKGROUND: Diastolic wall strain (DWS) is based on linear elastic theory, which shows that impaired diastolic wall thinning reflects resistance to deformation in diastole and thus, increased diastolic myocardial stiffness. We aim to explore the role of DWS in patients with heart failure with preserved ejection fraction (HFpEF) in terms of correlation with indices of HFpEF. METHODS: Study enrolled 53 patients with exertional dyspnoea and normal left ventricular ejection fraction. Forty patients fulfilled the criteria for HFpEF according to ESC 2023 criteria. RESULTS: Two groups were analysed

Progressive Fibrotic Pneumonitis Following Durvalumab Therapy: A Challenging Case.

Durvalumab following chemoradiotherapy (CRT) is the standard of care for unresectable stage III non-small cell lung cancer (NSCLC). Immune-related pneumonitis (IrP) is a clinically relevant immune-related adverse event (irAE) that may limit treatment benefit and, although usually steroid-responsive, can rarely progress to chronic fibrotic lung disease. We report the case of a 71-year-old man with stage IIIc squamous NSCLC treated with CRT followed by consolidation durvalumab. After the second durvalumab cycle, he developed worsening dyspnoea, and computed tomography (CT) showed bilateral multi

Validity and responsiveness of patient-reported outcomes common terminology criteria for adverse events (PRO-CTCAE) among Italian lung cancer patients.

BACKGROUND: Patient-reported symptom monitoring may improve the outcomes of cancer treatment. Knowledge of psychometric properties may drive the selection of optimal tools. Here we report the analysis of PRO-CTCAE in Italian lung cancer patients. METHODS: A PRO-CTCAE list (74 items, 44 symptoms) was administered to on-treatment patients at registration (visit 1) and after 2-6 weeks (visit 2). Convergent validity with EORTC LC-13 and Hospital Anxiety and Depression Scale (HADS) was assessed by Pearson correlation. Known-group validity and responsiveness were tested with ECOG performance status

Dyspnoea patterns in patients with advanced diseases: a functional MRI feasibility study protocol.

INTRODUCTION: Dyspnoea is an existentially burdensome symptom in patients with advanced and progressive diseases such as cancer, chronic obstructive pulmonary disease (COPD) and advanced heart failure. Recent studies have highlighted that symptomatic treatment of dyspnoea is often ineffective and may depend on the underlying disease. Immersive virtual reality (IVR) has emerged as a 'digital therapeutic' for conditions such as pain, anxiety, and dyspnoea. Brain functional MRI (fMRI) offers the opportunity to identify distinct patterns of dyspnoea. Current findings are mainly limited to healthy

A diagnostic challenge: unveiling chronic thromboembolic pulmonary hypertension in the absence of visible thrombi: a case report.

BACKGROUND: Chronic thromboembolic pulmonary hypertension (CTEPH) is a severe condition characterized by persistent dyspnoea and hypoxia. Diagnosis is challenging, especially when computed tomography pulmonary angiography (CTPA) shows no visible thrombi and symptoms overlap with common conditions like heart failure. CASE SUMMARY: We present a 77-year-old female with heart failure with preserved ejection fraction who experienced progressive, refractory dyspnoea and hypoxia. Despite a CTPA showing no large, organized thrombi, a strong clinical suspicion prompted a ventilation/perfusion (V/Q) sc

Prevalence, symptoms, and associated factors of long COVID: a retrospective cohort study based on data from two major German health authorities.

BACKGROUND: Acute SARS-CoV-2 infection has caused substantial morbidity and mortality worldwide. As the pandemic subsided, persistent symptoms gained increasing attention. Time-based definitions distinguish ongoing symptomatic COVID-19 (weeks 4–12) from post-COVID syndrome ( > 12 weeks). Consistent with these time-based definitions, long COVID is commonly used as an umbrella term for symptoms persisting for ≥4 weeks post-infection (including ongoing symptomatic COVID-19 and post-COVID-19 syndrome). Reported prevalence estimates vary widely, with meta-analyses suggesting 43–80% for persistent s

[Spontaneous bacterial peritonitis in cardiac ascites: A case report and review of the literature].

Cardiac ascites accounts for approximately 5% of ascites cases and is typically characterized by high protein concentration, which is thought to confer protection against spontaneous bacterial peritonitis. Nevertheless, spontaneous bacterial peritonitis may develop in the setting of advanced heart failure due to intestinal congestion and impaired perfusion. We present the case of a 65-year-old male with advanced heart failure, a left ventricular ejection fraction of 19%, severe tricuspid regurgitation, and pulmonary hypertension, who presented with dyspnoea and progressive abdominal distension

Serum soluble ST2: An effective prognostic biomarker for mortality in patients with severe acute exacerbation of chronic obstructive pulmonary disease requiring invasive mechanical ventilation.

BACKGROUND: For severe acute exacerbation of chronic obstructive pulmonary disease (AECOPD) patients unresponsive to initial treatment and at high mortality risk, early biomarker-based risk stratification may refine therapies and boost survival. METHODS: A total of 84 patients with severe AECOPD were prospectively enrolled. Serum soluble ST2 (sST2) levels were measured at baseline (day 0, diagnosis) and on days 3 and 7 after systemic therapy was initiated. The primary goals were to evaluate the prognostic value of sST2 for predicting all-cause in-hospital mortality and to determine its additi
