Anaesthetic Management of a Patient With a Giant Ovarian Tumour Containing 28 Litres of Ascitic Fluid.

We report the anaesthetic management of a 47-year-old woman scheduled for tumour resection of a giant ovarian tumour containing 28 litres of ascitic fluid. Her preoperative abdominal circumference was 186 cm due to massive ascites, and metabolic equivalents (METs) were less than 3.5. She was planned for cytoreductive surgery under general anaesthesia combined with epidural analgesia. Anaesthetic challenges included ventilatory management to maintain airway pressures, haemodynamic stability during massive fluid shifts, extensive monitoring, pre-induction epidural catheter placement, prevention

Pulmonary thromboembolism and aortic thrombosis due to severe hyperhomocysteinemia with MTHFR C677T mutation: a case report.

BACKGROUND: Hyperhomocysteinemia is a well-established risk factor for arterial and venous thromboembolism. Among the various aetiologies, a homozygous C677T mutation in the methylenetetrahydrofolate reductase (MTHFR) gene impairs the remethylation of homocysteine, promoting a prothrombotic milieu. However, simultaneous manifestations of pulmonary thromboembolism and aortic thrombosis remain exceedingly rare and clinically underrecognized. CASE SUMMARY: We report a case of a 58-year-old male who presented with acute dyspnoea. Chest computed tomography angiography revealed bilateral pulmonary

Primary cardiac T-cell lymphoma presenting as ST-elevation myocardial infarction with fatal ventricular arrhythmias: a case report.

BACKGROUND: Primary cardiac T-cell lymphoma (PCTCL) is an extremely rare subtype of primary cardiac lymphoma. Due to the paucity of clinical data, it poses a challenge from diagnosis to management. CASE SUMMARY: A 65-year-old female presented with dyspnoea and epigastric pain. Her electrocardiogram revealed an ST-elevation in leads II, III, aVF, and V4-V6, for which she underwent an emergent coronary angiogram, which revealed normal coronaries. Further multimodality cardiac imaging demonstrated a large right ventricular mass infiltrating the inferior wall of the left ventricle. A percutaneous

Systematic review and meta-analysis assessing longitudinal changes in symptom prevalence, severity and health-related quality of life in post-COVID-19 syndrome: a protocol.

INTRODUCTION: Post-COVID-19 syndrome (PCS) is characterised by persistent symptoms, such as fatigue, dyspnoea, depression and sleep problems, following SARS-CoV-2 infection. The long-term course and impact on quality of life remain unclear. This review aims to synthesise evidence on longitudinal changes in symptom prevalence, severity and health-related quality of life (HRQoL) in adults with PCS. METHODS AND ANALYSIS: This systematic review will include longitudinal studies (randomised controlled trials, non-randomised trials, prospective and retrospective cohort studies) of adults (≥18 years

Barriers and facilitators to healthy lifestyle behaviours for people living with post pulmonary embolism syndrome. A qualitative study exploring participants lived experiences.

BACKGROUND: People living with post-pulmonary embolism syndrome (PPES) commonly experience persistent dyspnoea, fatigue, reduced exercise tolerance, anxiety, and fear of recurrence. When symptoms continue beyond three months after effective anticoagulation, quality of life can be substantially impaired. Although healthy lifestyle behaviours may improve PPES symptoms, little is known about the barriers and facilitators influencing behaviour change from the perspective of those affected. AIM: To explore patients' perceived barriers and facilitators to adopting healthy lifestyle behaviours follo

Functional impact of long COVID among healthcare workers with comorbidities in Quebec, Canada: a cross-sectional study.

INTRODUCTION: Long COVID is a frequent post-infectious chronic condition that impacts quality of life and work performance. Whether individuals with comorbidities experience a greater functional impact of long COVID is unknown. We evaluated the functional impact of long COVID among healthcare workers (HCWs) with chronic cardiovascular diseases, chronic respiratory diseases, obesity or a history of depression, and compared it with that of HCWs without comorbidities. METHODS: We conducted a cross-sectional study in Quebec, Canada. We compared self-reported long COVID cases to COVID-19-infected

Predicting low oxygen in patients with acute COVID-19 infection isolating at home: a clinical prediction model.

RATIONALE: Identifying patients who could be safely managed at home versus those needing hospitalisation was a particular concern during early COVID-19. Respiratory viruses remain a concern, including new COVID-19 variants, influenza and respiratory syncytial virus. We developed COVIDFree@Home, a mobile application and clinician dashboard for remote monitoring, to determine if remotely collected measures could predict low oxygen saturation in home-isolating patients with COVID-19. METHODS: We conducted a prospective cohort study of patients newly diagnosed with COVID-19 from three Toronto hos

A single bout of submaximal aerobic functional capacity test acutely promotes endothelial function in long COVID patients.

Persistent symptoms and vascular impairment are common among individuals recovering from COVID-19, yet little is known about acute interventions capable of improving endothelial function in this group. Submaximal functional tests, such as the six-minute step test (6MST), provoke marked cardiovascular activation and may provide a brief stimulus for vascular adaptation. This study examined whether performing a single 6MST session could trigger an immediate enhancement in brachial artery endothelial function in adults with long COVID. Forty-seven participants, symptomatic long COVID individuals a

Video-assisted thoracic surgery management of giant pulmonary bullae: Insights from a single-centre series of 53 patients.

INTRODUCTION: Giant bullae (GB) of the lung are emphysematous air spaces occupying at least one-third of the hemithorax. GB may lead to progressive dyspnoea, compression of adjacent lung and pneumothorax, necessitating surgery as the definitive management in symptomatic patients. PATIENTS AND METHODS: We conducted a retrospective analysis of 53 cases of GB operated at a tertiary care centre in India from December 2020 to December 2024. RESULTS: The study cohort included 42 males and 11 females, with a mean age of 49.8 years (range: 12-74). GB occurred unilaterally in 38 patients (right lung

Surgical Fissure Completion Enables Endobronchial Valve Placement for Severe Emphysema with Collateral Ventilation.

BACKGROUND: Endobronchial valves (EBV) improve outcomes in selected patients with severe emphysema but are ineffective in the presence of collateral ventilation. The COVE procedure (closure of collateral ventilation) combines video-assisted thoracoscopic (VATS) fissure completion with valve placement, enabling therapy in patients otherwise excluded. METHODS: This prospective, ethically approved study planned to recruit 20 patients but closed early due to the COVID-19 pandemic. Thirteen consented, one withdrawing and two proceeding to EBV. Ten consecutive patients underwent the COVE procedure.

Integrated Pulmonary Severity Score (IPSS) for COPD: A Psycho-Respiratory Risk Index Supported by Explainable Machine Learning.

Background/Objectives: In chronic obstructive pulmonary disease (COPD), forced expiratory volume in one second (FEV1) explains only part of the variability in symptoms and prognosis, while anxiety and depression are common but rarely quantified in composite indices. We aimed to develop and internally validate an Integrated Pulmonary Severity Score (IPSS) that combines respiratory function, symptom burden and affective status. Methods: In a prospective observational study, 390 adults with spirometry-confirmed COPD were consecutively enrolled at two tertiary Romanian centres (October 2022-Septem

Exercise-Induced Desaturation in Patent Foramen Ovale: Mechanisms, Diagnostic Approach, and Resolution After Closure-A Narrative Review with an Illustrative Case.

Exercise-induced desaturation is an uncommon but clinically significant manifestation of patent foramen ovale, which is present in approximately one-quarter of the general population. Although patent foramen ovale is usually asymptomatic, exertion may provoke transient right-to-left shunting when dynamic changes in venous return and intrathoracic pressure favour intermittent right-to-left transit across the interatrial septum. This narrative review synthesises current evidence on exertion-provoked shunting and its contribution to otherwise unexplained dyspnoea and hypoxaemia. To illustrate the

Community-based prediction models of cardiovascular events, acute exacerbations and all-cause mortality in individuals with chronic obstructive pulmonary disease: a systematic review and meta-analysis on behalf of the International Cardiovascular and Respiratory Alliance.

INTRODUCTION: Preventable morbidity and mortality from chronic obstructive pulmonary disease (COPD) accrue from major adverse cardiovascular events (MACEs) and acute exacerbations of COPD (AECOPD). The study aims to summarise models for the prediction of these cardiopulmonary events in community-based settings. METHODS: We searched for studies of multivariable models derived, validated or augmented for the prediction of cardiopulmonary events in COPD and used community-based data sources using MEDLINE and Embase from inception through 10 April 2025. Discrimination measures for the model with

Landiolol for rate control in decompensated heart failure due to atrial fibrillation: the LARISA trial.

AIMS: Rate control of atrial fibrillation (AF) in acute heart failure (AHF) with reduced left ventricular ejection fraction (LVEF) is challenging. We evaluated the safety and efficacy of intensive rate control (IRC) using landiolol with adjunctive digoxin. METHODS AND RESULTS: Forty patients with AHF due to AF [heart rate (HR) > 130/min, LVEF <40%] were randomized to IRC with landiolol plus digoxin or standard rate control therapy. The primary endpoint was sustained HR reduction >20% or <115/min at 2 h. Haemodynamics, dyspnoea, and lung congestion were assessed serially. HR control was achiev

Idiopathic Pulmonary Fibrosis: An Overview.

Idiopathic pulmonary fibrosis (IPF) is the prototypical fibrosing interstitial lung disease, characterised by its unrelenting progressive course and poor prognosis. The incidence of IPF is rising and is becoming a major public health concern. Debilitating dyspnoea and respiratory failure results. Death occurs on average 3-5 years from the time of diagnosis. Clearer understanding of the pathobiology of the condition continues to advance and although we may now better understand disease mechanisms, our therapeutic approach remains limited. In the UK, only two treatments are licensed for IPF, and

Palliative Care in Heart Failure: A Practical Approach.

Chronic heart failure (HF) is a complex clinical syndrome and a major cause of morbidity and mortality, which affects more than 64 million people worldwide. HF is associated with decreased life expectancy and is characterised by increased hospitalisations, which increase in frequency and severity as the disease progresses. Patients with advanced HF experience a poorer health status and quality of life. Advanced HF increases the risk of premature death (80% of patients die within 5 years), as well as increasing symptoms, psychosocial and spiritual burden. The trajectory of the disease is unpred

Transient cavity dilatation during supine exercise bicycle stress testing: mechanistic insights.

BACKGROUND: Cavity dilatation is occasionally observed during supine bicycle exercise stress echocardiography (SBSE). The underlying mechanisms are poorly understood. AIMS: This study aimed to characterise patients with left ventricle (LV) cavity dilatation and a decrease in the left ventricular ejection fraction (LVEF) during SBSE. METHODS: A total of 653 patients who underwent SBSE were evaluated. Those with exercise-induced cavity dilatation (defined as increased cavity size and a decrease in LVEF) were evaluated (n = 29). A control group (n = 37) of patients with a hypertensive response
