Short and long-term prognosis of hospitalization for dyspnoea based on aetiology and hospitalization ward: insights from the PARADISE cohort.

AIMS: Dyspnoea accounts for nearly 5% of emergency department (ED) visits. Our aim was to describe the in-hospital and long-term outcomes of patients admitted to the ED for dyspnoea, based on their underlying aetiology, and to determine if prognosis varies according to the hospitalization setting. METHODS: We analyzed 18 903 consecutive patients (48% male, average age 73 years) hospitalized after an ED visit for dyspnoea from January 2010 to December 2019, as part of the PARADISE cohort (PAthwAy of Dyspneic patIent in Emergency-NCT02800122). Dyspnoea causes were classified as acute heart fail

Effects of rice bran arabinoxylan compound on quality of life of cancer patients during active treatment: a randomised placebo-controlled pilot trial.

The effects of a plant-based immunomodulator, rice bran arabinoxylan compound (RBAC), on the quality of life (QoL) of cancer patients during active treatment are unclear. The RBAC-QoL study was a randomised, placebo-controlled, double-blind feasibility study to address the role of RBAC in cancer patients receiving systemic therapies. The primary outcome measure was patient-reported functional, symptom, and global QoL scores. Secondary and exploratory outcome measures included nutritional indices and cytokine changes. Adult patients (n = 29) with solid organ tumours (≥ stage II) undergoing syst

Comparative effectiveness and safety of digital health delivery models for pulmonary rehabilitation in patients with chronic obstructive pulmonary disease: a protocol for systematic review and network meta-analysis of randomised controlled trials.

INTRODUCTION: Chronic obstructive pulmonary disease (COPD) affects approximately 480 million individuals globally and is projected to reach 600 million by 2050, representing a substantial burden on healthcare systems and patient quality of life. Pulmonary rehabilitation is a cornerstone intervention for COPD management, delivering clinically meaningful improvements in exercise capacity, health-related quality of life and dyspnoea. Despite strong guideline recommendations and established efficacy, only 2%-4% of eligible patients with COPD access traditional centre-based pulmonary rehabilitation

Patient-reported dyspnoea predicts six-fold increased hospital mortality.

BACKGROUND: Patients can report respiratory discomfort (dyspnoea) on a 0-10 scale. Patients reporting dyspnoea at admission have a four-fold risk of in-hospital death. We asked whether assessing dyspnoea throughout hospitalisation identifies additional patients at risk of death or other poor outcomes. METHODS: We conducted a retrospective cohort study of non-intensive care unit patients at a tertiary care hospital. On each shift, bedside nurses documented patients' pain and dyspnoea ratings. We tested associations with inpatient mortality, 2-year mortality and other outcomes. RESULTS: We eva

Stenting for external outflow graft obstruction of a left ventricular assist device via a surgical axillary approach.

External outflow graft obstruction is an uncommon late complication in patients supported with a fully magnetically levitated left ventricular assist device. Progressive extrinsic compression of the outflow graft can impair pump performance and lead to symptomatic deterioration, requiring intervention to restore graft patency. This video tutorial presents a hybrid procedure combining surgical transaxillary access with endovascular stent implantation for treatment of external outflow graft obstruction when transfemoral access is not feasible. A 72-year-old woman with ventricular assist device s

Developing and validating the CE-MACE model to predict 1-year major adverse cardiovascular events post-COPD exacerbation using routine healthcare data.

BACKGROUND: Patients with COPD are at an elevated risk of cardiovascular events, particularly following acute exacerbations. Existing prediction models underestimate their risk. We developed and validated the CE-MACE model to predict 1-year major adverse cardiovascular event (MACE) risk following a COPD exacerbation. METHODS: Using electronic health records from the UK Clinical Practice Research Datalink Aurum database, we included patients ≥40 years old with moderate or severe COPD exacerbations. The outcome was fatal or non-fatal MACE (acute coronary syndrome, arrhythmia, heart failure or i

Opioid-associated modulation of respiratory-related cortical activity in dyspnoeic mechanically ventilated patients: An electroencephalographic study.

BACKGROUND: Dyspnoea is frequent in mechanically ventilated patients and contributes to substantial distress and a heightened risk of post-traumatic stress disorder. It attests to respiratory-related brain suffering that must be actively managed. This is particularly challenging in noncommunicative patients, hence the interest of electroencephalographic surrogates. This study aims at characterizing the effects of opioids, proposed to relieve dyspnoea, on respiratory-related cortical activity in mechanically ventilated patients. METHODS: In a 16-bed intensive care unit (ICU) over a 4-month per

Method validation of cardiopulmonary exercise testing: intra- and inter-device comparisons across four metabolic carts.

AIMS: Cardiopulmonary exercise testing (CPET) is the gold standard for assessing exertional dyspnoea, providing peak oxygen uptake (VO2), and the VE/VCO2 slope. Different metabolic carts, may introduce systematic variability, confounding longitudinal assessments and multicentre trials. We evaluated intra-device reliability and inter-device comparability of CPET parameters. METHODS AND RESULTS: In this prospective, single-centre, within-subject, repeated-measures, multi-device comparison study 34 healthy adults (35 ± 11 years; 59% males) completed eight CPETs: two per cart, on four systems (MGC

People with chronic obstructive pulmonary disease and exercise behaviour: a scoping review using the CFIR framework.

BACKGROUND: Lack of exercise constitutes an independent predictor of mortality in patients with chronic obstructive pulmonary disease (COPD). Despite a considerable number of intervention studies having been conducted to promote exercise behaviour among these patients, implementation outcomes remain poor, and little is known about the factors influencing intervention delivery. From an implementation science perspective, we analysed previous research on exercise behaviour in people with COPD, synthesising assessment indicators, evaluation tools, and the implementation-related facilitators and b

The effect of SARS-CoV-2 testing on healthcare-seeking behaviour at primary care level.

BACKGROUND: Diagnostic self-testing for SARS-CoV-2 may lead to selection bias in test-negative case-control designs (TND) for COVID-19 vaccine effectiveness (VE) at the primary care level. We investigated whether SARS-CoV-2 self-testing among those with acute respiratory infection (ARI) after the acute pandemic phase was associated with primary care healthcare seeking in the general Dutch population. METHODS: We pooled questionnaire data from three study rounds (June 2022, November 2022, and April 2023) of the nationwide PIENTER Corona cohort study. Among 3152 participants aged ≥18 years, we

Self-expandable valve-in-valve transcatheter aortic valve implantation for a failed sutureless Perceval bioprosthesis: a case report.

BACKGROUND: Early structural degeneration of sutureless Perceval bioprostheses is uncommon but may result in severe haemodynamic compromise and heart failure. Valve-in-valve transcatheter aortic valve implantation (ViV-TAVI) represents a less invasive therapeutic option in high-risk patients. CASE SUMMARY: A 78-year-old woman presented with progressive dyspnoea and lower-extremity oedema 3 years after surgical aortic valve replacement with a Perceval bioprosthesis. Transthoracic echocardiography revealed severe mixed prosthetic valve dysfunction with markedly elevated transvalvular gradients.

Sequential Systemic Thrombolysis for Clot-In-Transit after Thrombolytic Failure in Massive Pulmonary Embolism.

INTRODUCTION: A clot-in-transit (CIT) is a mobile thrombus within the right heart associated with acute pulmonary embolism (PE) and high mortality. Although systemic thrombolysis remains the standard treatment for massive or high-risk PE, management of thrombolytic failure and associated CIT remains challenging, particularly when surgical embolectomy and catheter-directed thrombectomy are unavailable. Sequential systemic thrombolysis represents a practical therapeutic option in these high-risk scenarios. CASE DESCRIPTION: A 54-year-old woman with hypertension presented with progressive dyspno

Characterising Patient Pathways Prior to the Diagnosis of Interstitial Lung Disease.

BACKGROUND AND OBJECTIVE: Interstitial lung diseases (ILDs) lead to breathlessness, cough, deteriorating quality of life and early death. Patients are typically diagnosed late in their disease course when existing lung damage cannot be reversed. Accordingly, early diagnosis is the best strategy for preserving lung function and extending lifespan. Our aim was to identify signs, symptoms and health patterns and determine when they arise in the years preceding ILD diagnosis. METHODS: We identified incident ILD cases from a large primary care electronic medical record database from the United Kin

Undiagnosed Coronary Artery Disease in Patients with COPD.

Background: Coronary artery disease (CAD) commonly coexists with chronic obstructive pulmonary disease (COPD), but may be under-recognised, since symptoms such as dyspnoea and chest discomfort are often attributed to lung disease. We hypothesised that coronary artery disease is highly prevalent in patients with COPD, even in the absence of typical angina symptoms. Methods: This study aimed to detect CAD in patients with COPD. We conducted a single-centre observational study, including 76 patients with no known previous cardiovascular events. To detect ischaemic heart disease, three methods wer

Stress Echocardiography in the Diagnosis and Evaluation of Pulmonary Hypertension: Practical Recommendations, Haemodynamic Phenotyping, and Application in Adults and Children.

Pulmonary hypertension (PH) is a complex condition in which early diagnosis remains challenging, particularly in patients with exertional symptoms and normal or borderline resting haemodynamics. Although right heart catheterisation is the diagnostic gold standard, transthoracic echocardiography is the recommended first-line non-invasive test. However, resting echocardiography provides only a static assessment and may underestimate disease severity in early or latent pulmonary vascular disease due to preserved pulmonary vascular compliance and adaptive right ventricular responses. Because pulmo

Development and psychometric evaluation of the DysPalKT for assessing nurses' knowledge of dyspnoea care in patients with cancer receiving palliative care.

BACKGROUND: Dyspnoea is one of the most prevalent and distressing symptoms among patients with advanced cancer receiving palliative care. Nurses, particularly in non-specialist settings, often report insufficient preparedness in managing dyspnoea. Despite its clinical relevance, existing tests inadequately assess nurses’ knowledge of dyspnoea care. This study aimed to develop and evaluate the Knowledge of Dyspnoea Care in Patients with Cancer Receiving Palliative Care (DysPalKT) to address this gap. METHODS: The DysPalKT was developed through a multi method approach including literature synthe

The timing of the commencement of pulmonary rehabilitation in hospitalized patients with acute exacerbation of COPD: a systematic review and network meta-analysis.

BACKGROUND: Pulmonary rehabilitation (PR) is an effective intervention for patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) following hospitalization. However, the optimal timing for initiating PR after admission remains controversial. This study conducted a systematic review and network meta-analysis to evaluate the therapeutic effects of initiating PR at different time points, with the aim of providing evidence-based recommendations to inform clinical decision-making and guideline development. METHODS: Randomized controlled trials (RCTs) on PR following AEC
