Non-Pharmacological Breathlessness Action Plans for People with COPD and Their Support People: A Scoping Review and Survey of Current Practice.

INTRODUCTION: Breathlessness action plans guide people to self-manage acute-on-chronic breathlessness episodes using non-pharmacological strategies to help breathing and overcome panic. We aimed to identify plans available for people with chronic obstructive pulmonary disease (COPD) and describe their development, content, quality, use, and evidence for benefit. METHODS: Two descriptive methodologies were used, overseen by a team of people with COPD and carers/support persons, clinicians and researchers: 1) A scoping review (academic databases, internet) identified English-language plans and

Right-dominant arrhythmogenic cardiomyopathy complicated by platypnea-orthodeoxia syndrome: a novel mechanism of patent foramen Ovale-mediated hypoxaemia: a case report.

BACKGROUND: Platypnea-orthodeoxia syndrome (POS) is a rare but treatable cause of positional hypoxaemia, typically associated with intracardiac shunting. We present the first documented case of POS secondary to patent foramen ovale (PFO) in a patient with right-dominant arrhythmogenic cardiomyopathy (ACM). CASE SUMMARY: A 48-year-old male with right-dominant ACM developed progressive dyspnoea and upright hypoxaemia. Workup examinations revealed atrial flutter and PFO with right-to-left shunting. PFO occlusion resolved hypoxaemia, with sustained improvement at 3-year follow-up despite progress

How does pulmonary function impact QoL in patients with locally advanced NSCLC treated with chemoradiotherapy and durvalumab?

BACKGROUND: Impaired pulmonary function is common among patients with lung cancer and may negatively affect health-related quality of life (HRQoL). The primary objective of the present sub-study of the DART-trial was to assess the overall quality of life changes during treatment and stratified by the presence of Chronic Obstructive Pulmonary Disease (COPD). METHODS: The investigator-initiated DART trial (NCT04392505) included patients with unresectable stage III non-small cell lung cancer (NSCLC) treated with chemoradiotherapy followed by durvalumab. Baseline pulmonary function was measured b

Management of malignant pleural effusions using an indwelling tunnelled pleural catheter and non-vacuum collection system.

BACKGROUND: Indwelling tunnelled pleural catheters (TPCs) are a minimally invasive way to palliate symptoms for patients with recurrent symptomatic malignant pleural effusions. Current data on TPCs have used vacuum-assisted drainage systems. Gravity-based drainage systems may represent an alternative approach with improvement in patient symptoms, but have a smaller overall footprint and a decreased cost. We therefore aimed to understand the efficacy, complication rate and spontaneous pleurodesis rate of a TPC that uses a non-vacuum-assisted drainage system. METHODS: This is a prospective, obs

Cardiopulmonary exercise testing in contemporary cardiology: physiological insights, novel applications and evolving algorithms.

Cardiopulmonary exercise testing (CPET) allows for the study of the pathophysiology of exercise intolerance through assessment of exercise integrative physiology of the pulmonary, cardiovascular, muscular and cellular oxidative systems. Over the years, key gas exchange variables have shown a role in the interpretative translation of physiology to clinical decision making. CPET is a standard when approaching all forms of exercise intolerance, with a predominant evidence for heart failure and hypertrophic cardiomyopathy. As impaired cardiac output and peripheral oxygen diffusion are the main det

Severe cardiomyopathy due to permanent junctional reciprocating tachycardia: recovery after catheter ablation?-a case report.

BACKGROUND: Permanent junctional reciprocating tachycardia (PJRT) is a rare form of orthodromic atrioventricular reciprocating tachycardia involving a slowly conducting, decremental concealed accessory pathway. Its incessant nature may result in tachycardia-induced cardiomyopathy, which is reversible with timely rhythm control. CASE SUMMARY: A 41-year-old patient presented with progressive exertional dyspnoea and reduced exercise capacity. Continuous electrocardiogram monitoring revealed an incessant long RP narrow QRS tachycardia (heart rate 135 b.p.m.). Echocardiography demonstrated severe

Transpulmonary eicosanoid metabolite gradients: insights into metabolic flux of pro- and anti-inflammatory metabolites and lung function.

INTRODUCTION: Eicosanoids are upstream lipid mediators of inflammatory pathways. Transpulmonary gradient (ΔEIC) analysis using multisite sampling enables physiological assessment of metabolic flux across the lungs. While specific eicosanoids have been implicated in lung disease, the relationship between their metabolic flux and pulmonary physiology remains poorly understood. METHODS: We performed a cross-sectional study of patients with dyspnoea who underwent spirometry and right heart catheterisation with multisite (pulmonary artery, radial artery) sampling. Eicosanoids and eicosanoid-relate

Point-of-care ultrasound for evaluating acute dyspnoea in emergency departments: Systematic review and meta-analysis.

BACKGROUND: Acute dyspnoea is a common yet diagnostically complex presentation in emergency departments (EDs), representing approximately 2.4% of all visits. Traditional diagnostic tools-clinical assessment, chest radiography, and laboratory tests-may lack the precision required for timely and accurate diagnosis. Point-of-care ultrasound (POCUS) offers real-time, bedside imaging and has emerged as a promising tool to address these limitations. AIM: To evaluate the diagnostic accuracy and clinical effectiveness of POCUS vs conventional modalities in adults with acute dyspnoea in the ED. METHO

Diagnostic Clues to Platypnoea Orthodexia Syndrome.

Platypnoea orthodeoxia syndrome is a rare cause of positional dyspnoea, characterised by breathlessness and arterial desaturation in the upright position that improves with lying flat. This phenomenon is typically due to a right to left shunt, most commonly cardiac, that allows deoxygenated blood to bypass the pulmonary circulation, therefore reducing oxygen saturation in the systemic circulation. This shunt is accentuated by postural changes, worsening hypoxaemia when the patient sits or stands, and ameliorated by recumbency, which alters intracardiac pressures and improves oxygenation. [Dyspnoea in invasively ventilated patients: an overview].

Dyspnoea is a highly relevant phenomenon in patients undergoing invasive mechanical ventilation, yet it is often underestimated in clinical practice. Against this background, the European Respiratory Society (ERS) and the European Society of Intensive Care Medicine (ESICM) have published a comprehensive statement on the various aspects of dyspnoea during mechanical ventilation. This article is based on this consensus document and highlights the various dimensions of dyspnoea, from its prevalence and clinical relevance to the individual experiences of patients and its physiological and psycholo

Unexpected systemic sclerosis in a patient with atopic dermatitis receiving dupilumab: a novel case report.

Dupilumab, an interleukin (IL)-4 receptor alpha antagonist, is widely used in the treatment of atopic dermatitis and has shown potential benefit in certain fibrosing skin conditions. While blockade of IL-4 and IL-13 is generally considered to inhibit fibrosis, emerging reports of localized sclerosing dermatoses paradoxically arising during dupilumab therapy suggest a more complex immunological effect. We report a 37-year-old woman with the first case of systemic sclerosis developing in a patient with atopic dermatitis treated with dupilumab. After 2 years of treatment, she presented with new-o

Fulminant growth of primary pericardial synovial sarcoma despite partial resection: a case report.

BACKGROUND: Primary pericardial synovial sarcomas are rare and aggressive, with limited data to guide treatment. Though advancements in cardiac imaging have improved diagnostic capabilities, these tumours are often discovered after significant disease progression. We report one of few extremely aggressive cases of pericardial sarcoma that led to in-hospital mortality. CASE SUMMARY: A 59-year-old woman presented with abdominal pain and dyspnoea and was found to have cardiac tamponade. Transthoracic echocardiogram and cardiac magnetic resonance imaging revealed an 11.1 cm pericardial mass, whic

Factors influencing quality of life over five years post-diagnosis in breast cancer survivors: Results from the E3N-Generations cohort.

INTRODUCTION: Quality of life (QoL) of cancer survivors may be affected in the long term by the severity of the cancer and its treatment. We aimed to measure QoL and to identify factors associated with a lower QoL in breast cancer survivors (BCS) more than five years after cancer diagnosis. METHODS: We conducted a cross-sectional study (October 2020-December 2021) among BCS participating in the French population-based E3N-Generations prospective cohort. QoL was measured using the European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire Core 30 (EORTC QLQ-C30) v

Impact of the risk-treatment paradox on in-hospital and long-term outcomes in patients with very high-risk non-ST-elevation acute coronary syndrome.

Background and objectives The risk-treatment paradox (RTP) has not been studied in very high-risk patients with non-ST-elevation acute coronary syndrome (NSTE-ACS). We aimed to describe the cohort of very high-risk NSTE-ACS patients, analyse the impact of RTP on outcomes, and identify subgroups of patients who derive the greatest benefit from immediate invasive coronary angiography (ICA). Methods This retrospective analysis included 340 patients diagnosed with NSTE-ACS who met at least one of the established criteria for very high risk at admission. RTP was defined as any management tactic oth

Transbronchoscopic Local Injection of Triamcinolone Acetonide for Subglottic Granulation Tissue: A Case Report.

Subglottic granulation tissue is a rare but recurrent cause of airway stenosis that develops after epithelial injury from endotracheal intubation or other insults. We herein report a 19-year-old woman who developed progressive dyspnoea 2 months after intubation for autoimmune encephalitis. Bronchoscopy revealed a pedunculated subglottic mass, which was resected with an electrocautery snare. Histology confirmed granulation tissue. Despite initial therapy, recurrence occurred within 2 weeks. A second bronchoscopic resection was performed, followed by the local injection of 8 mg of triamcinolone
