When Postextubation Stridor Is Not Edema: Bilateral Lateral Subglottic Mucosal Tears.

Iatrogenic tracheal injury following endotracheal intubation is a rare but potentially life-threatening complication. Most reported injuries involve the posterior membranous tracheal wall. We describe a case in which a patient required rapid sequence emergency intubation in the ED and, three days later, following extubation, developed acute stridor and increasing oxygen requirements, found to be secondary to bilateral, inferiorly based lateral mucosal tears of the subglottic larynx and superior trachea with an associated grade 1 subglottic stenosis. This case highlights the importance of maint

Relapsing Polychondritis With Severe Airway Involvement Mimicking Refractory Asthma: A Diagnostic Challenge.

Relapsing polychondritis (RP) is a rare immune-mediated disorder that can cause severe tracheobronchial involvement and life-threatening airway compromise. We present a diagnostically challenging case of a 30-year-old non-binary patient whose progressive respiratory symptoms were initially attributed to severe refractory asthma. Despite repeated courses of systemic corticosteroids and escalation of asthma-directed therapy, they developed recurrent breathlessness, wheeze, and inspiratory stridor. Pulmonary function testing demonstrated severe fixed airflow obstruction with minimal bronchodilato

Postextubation Stridor: An Analysis of Incidence, Risk Factors, and Clinical Outcomes in Mechanically Ventilated Patients.

Background Postextubation stridor (PES) is a potentially life-threatening complication following endotracheal intubation. However, detailed reports on its incidence and outcomes remain limited under standardized extubation protocols. In this study, we aimed to investigate the incidence, clinical characteristics, risk factors, and outcomes of PES, and evaluate the cuff leak test (CLT) for predicting PES. Methods This retrospective study included all patients who received mechanical ventilation for 48 hours or more at a single tertiary care center between January 2009 and December 2023. A compre

Bedside flexible bronchoscopy in the neonatal intensive care unit: Current evidence, clinical applications and future directions.

BACKGROUND: Lung ultrasound and targeted echocardiography let the neonatal team examine lungs and heart at the cot side, but neither shows the airway. Flexible bronchoscopy does, and it is increasingly performed inside the neonatal intensive care unit (NICU). METHODS: Narrative review of published series of flexible bronchoscopy in infants cared for in neonatal units, supported by pediatric intensive care studies and society standards. Certainty of evidence was not graded. RESULTS: Twelve neonatal series (652 infants) were identified; all but one were single-center and most were retrospectiv

Early Distraction in Neonates With a Bioresorbable Plate for Bilateral Vocal Fold Immobility.

We describe a novel surgical technique in two neonates with idiopathic bilateral vocal fold immobility (BVFI) using early posterior cricoid distraction with placement of a poly-l-lactic acid (PLLA) bioresorbable plate to improve the glottic aperture. Both patients experienced complete resolution of stridor and achieved full oral feeding within 2 weeks, avoiding the need for tracheostomy or gastrostomy tube placement.

CNPY3 variants associated with a recessive developmental and epileptic encephalopathy affect inflammasome and TLR function.

BACKGROUND: Canopy FGF signalling regulator 3 (CNPY3) is an endoplasmic reticulum (ER)-resident chaperone essential for toll-like receptor (TLR) folding, trafficking and inflammasome activation. In rare paediatric cases, biallelic CNPY3 variants have been associated with developmental and epileptic encephalopathy (DEE), but available data remain sparse. METHODS: 50 individuals from 38 unrelated families with biallelic CNPY3 variants and DEE were evaluated for exome and genome findings, standardised clinical phenotype descriptions, EEG results, and neuroimaging data. A selection of variants un

Obstructive Fibrinous Tracheal Pseudomembrane Following Short-Term Intubation: A Case Report and Management Considerations.

INTRODUCTION: Obstructive fibrinous tracheal pseudomembrane (OFTP) is a rare but serious complication of endotracheal intubation that can cause airway obstruction, postextubation stridor, and respiratory distress. Due to its nonspecific presentation, it is often misdiagnosed and early recognition and intervention are critical to prevent respiratory compromise. CASE: We present the case of a 17-year-old female who required 36 h of intubation for status epilepticus. She developed stridor within 24 h of extubation and presented with subglottic narrowing that did not respond to medical treatment.

High-Voltage Electrical Injury Causing a Full-Thickness Laryngotracheal Wall Defect: Reconstruction With an Anterolateral Thigh Free Flap and an Inverted Split-Thickness Skin Graft.

Full-thickness loss of the laryngotracheal wall is a rare and severe consequence of high-voltage electrical injury. It is anatomically and clinically distinct from the epithelialised tracheocutaneous fistula that follows tracheostomy decannulation, and it poses a far greater reconstructive problem because of progressive tissue necrosis, wound contamination, and exposure of the airway and major cervical vessels. We report the case of a 30-year-old man with approximately 30% total body surface area deep electrical burns involving the neck, abdomen, and all four limbs, who presented with an expos

Proximal Tracheal Mucoepidermoid Carcinoma Complicated by Early Anastomotic Stenosis-A Case Report.

Proximal tracheal mucoepidermoid carcinoma may remain occult despite prolonged upper-airway symptoms and normal flexible nasoendoscopy. Stridor following tracheal resection should prompt assessment of the anastomosis, as early postoperative stenosis caused by A-frame deformity and granulation tissue may be successfully managed with combined laser therapy, balloon dilatation, and intralesional corticosteroid injection.

Neonatal airway emergency: congenital laryngeal myxoma.

Laryngeal myxomas are exceptionally rare benign mesenchymal tumors, particularly in the pediatric population. We present a case of a neonate with a history of brief endotracheal intubation who developed inspiratory stridor and respiratory distress. Flexible laryngoscopy revealed a pedunculated mass on the right true vocal cord. The lesion was completely excised via microlaryngobronchoscopy. Definitive histopathological and immunohistochemical analysis-showing a hypocellular myxoid stroma, bland spindle cells, a low Ki-67 proliferation index (<5%), and positivity for CD34 with negative staining

Use of sole-agent ketamine sedation to facilitate laryngoscopy and foreign body removal in acute supraglottic airway obstruction.

A 78-year-old man presented with stridor and dysphonia after choking at home. He deteriorated, with respiratory distress and an oxygen saturation of 55% despite oxygen via a non-rebreather mask. Flexible nasendoscopy demonstrated a food bolus between the epiglottis and glottis. The team judged apnoea and assisted positive-pressure ventilation before extraction to be hazardous, while deterioration and limited cooperation made an awake technique impracticable. Sole-agent ketamine sedation facilitated videolaryngoscopy with clinically observed spontaneous breathing; the bolus, a slice of carrot,

Initial nebulized budesonide versus a local off-label nebulized dexamethasone protocol in children with institution-defined grade III croup receiving epinephrine: a retrospective cohort study.

BACKGROUND: High-acuity croup requires prompt airway-directed treatment, but evidence comparing nebulized budesonide with a local institutional off-label nebulized-dexamethasone protocol is limited. This internationally unusual route is distinct from standard oral, intramuscular, or intravenous dexamethasone therapy. METHODS: We conducted a single-center retrospective cohort study of children younger than 12 years hospitalized from 1 January 2014 through 31 December 2023. Pretreatment severity was fixed at the first institution-defined Grade III assessment. We compared initial nebulized budes

Peri-Extubation Dexamethasone to Prevent Post-extubation Airway Obstruction in Critically Ill Children: A Systematic Review of Randomized Trials Across Heterogeneous Populations, Regimens, and Outcomes.

Peri-extubation dexamethasone is used to prevent post-extubation upper airway obstruction in critically ill children, but pediatric randomized trials differ substantially in population selection, dosing, timing, and outcome definitions. We systematically reviewed randomized controlled trials evaluating peri-extubation dexamethasone in mechanically ventilated children. PubMed/MEDLINE, Scopus, and Web of Science were searched from inception to June 22, 2026. Nine trials randomized 1,063 children, with 949 included in reported analyses. Because comparator architecture, dexamethasone exposure, ext

Vocal Fold Granuloma Causing Acute Airway Obstruction in a Pediatric Patient.

Vocal fold granuloma (VFG) is a benign laryngeal lesion caused by mucosal trauma to the arytenoid cartilage. Pediatric VFG is rare and is most often associated with prolonged or traumatic intubation. We present the case of a seven-year-old male who developed a 1-cm pedunculated VFG following two separate brief intubations for minor procedures. The lesion produced a dynamic "ball-valve" effect, resulting in biphasic stridor, breathy dysphonia, and intermittent airway obstruction. While computed tomography (CT) imaging was inconclusive, laryngoscopy identified a pedunculated lesion prolapsing in

Outcome of Unilateral Transverse Cordotomy or Kashima Operation for Bilateral Vocal Cord Immobility.

Endoscopic laser cordotomy, also known as Kashima operation, is used for the treatment of bilateral vocal cord palsy where the glottis chink is made posteriorly, sufficient enough for patient to breathe comfortably without any stridor. The objective of the study was to assess outcome of unilateral transverse cordotomy (Kashima operation) for bilateral vocal cord immobility. This prospective observational study was conducted in the Department of Otolaryngology-Head & Neck Surgery, Bangladesh Medical University, Dhaka, Bangladesh from January 2020 to June 2021 with 12 patients having bilateral v

Nocturnal behaviours, stridor and recurrent respiratory failure in anti-IgLON5 disease.

Anti-IgLON5 disease is a rare autoimmune encephalopathy with concurrent tauopathy, characterised by sleep dysfunction and brainstem involvement. A 77-year-old woman developed obstructive sleep apnoea and recurrent hypercapnic respiratory failure requiring repeated intubation, initially misdiagnosed as epilepsy. Video recordings captured elaborate nocturnal behaviours, including praying and offering alms during sleep, alongside inspiratory stridor and sleep attacks during eating. Serial polysomnography documented progressive loss of sleep spindles and undifferentiated non-rapid eye movement sle

A Diagnostic Challenge: Necrotizing Retropharyngeal Lymphadenitis With Microbiologically Supported Multifocal Pseudomonas aeruginosa Infection and a Tularemia-Like Presentation.

Necrotizing retropharyngeal lymphadenitis can cause rapidly progressive airway compromise and systemic illness in young children. We describe a two-year-old boy with no previously recognized chronic disease or immune disorder who presented with fever, stridor, hypoxemia, cytopenias, markedly elevated inflammatory markers, and a necrotic right-cheek lesion. Progressive airway compromise required endotracheal intubation, and contrast-enhanced computed tomography revealed bilateral necrotic retropharyngeal lymph nodes or collections with surrounding edema, as well as cervical lymphadenopathy.
