Ground State Energy Fluctuations of Pinned Elastic Manifolds.

We describe the atypical fluctuations of the ground state energy of the random elastic manifold, a disordered model defined on a lattice of linear size L with internal dimension 0 ≤ d < 4 embedded in a medium of dimension N ≫ 1 . The ground-state energy results from a competition between confinement, elasticity and disorder. We obtain an exact description of the large deviation rate function with speed N L d and its different phases, corresponding to different patterns of replica symmetry breaking (RSB). Our results show that the ground-state energy satisfies a central limit theorem and we

Severe Orolingual and Laryngeal Angioedema after Thrombolysis with Alteplase: Angiotensin II Receptor Blockers Should Not Be Underestimated.

INTRODUCTION: Alteplase is a recombinant tissue plasminogen activator (rt-PA), used for intravenous thrombolysis in ischaemic strokes. Orolingual angioedema is a rare but potentially lifethreatening complication of alteplase. Concomitant treatment with angiotensin-converting enzyme (ACE) inhibitors has been identified as a primary risk factor. Angiotensin II receptor blockers (ARBs) have been rarely related to angioedema. CASE PRESENTATION: A 72-year-old woman with a personal history of type 1 diabetes mellitus, dyslipidemia, stage 3 CKD, and arterial hypertension, in treatment with valsartan

A Case of Life-Threatening Stridor: Vallecular Cyst in a 2-Month-Old Infant.

Vallecular cysts are uncommon birth defects that substantially restrict an infant's airway. This paper emphasizes the difficulties in diagnosing and managing a transfer case with limited resources. If these cysts are not detected early, they can have major clinical consequences. A 2-month-old female infant presented with progressive inspiratory stridor, respiratory distress, vomiting, and fever over one month. Initial investigations suggested pneumonia, but direct laryngoscopy revealed a large vallecular cyst compressing the laryngeal structures. Blood cultures grew Staphylococcus epidermidis

Stridor in Bilateral Medial Medullary Infarction: A Case Report, Literature Review, and Pathophysiologic Insights Into a Rare Presentation of an Uncommon Stroke.

BACKGROUND: Upper airway obstruction secondary to bilateral vocal cord paralysis is not a known classic presentation of bilateral medial medullary infarction (BMMI). This may potentially confound the diagnostic approach, particularly when coexisting with bulbar symptoms and quadriplegia. Prompt recognition is essential for timely and appropriate airway management and subsequent treatment. CASE PRESENTATION: A 74-year-old female presented with a three-week stepwise progression of asymmetric quadriparesis, slurred speech, and a prominent biphasic stridor. Flexible fiberoptic laryngoscopy reveal

[Acute upper airway obstruction in a 2 year old child; when an inspiratory stridor is not what it seems].

A toddler with upper airway obstruction was admitted to a peripheral hospital, with a differential diagnosis of laryngitis subglottica, unresponsive to treatments and deteriorating. The child was brought to theatre for airway management. After induction, the glottic opening was not visible. Several attempts using different techniques, adjuncts and staff were not successful and a cannot intubate cannot oxygenate scenario, severe hypoxemia and cardiac arrest occurred. After minutes, a scalpel tracheotomy led to reoxygenation and return of circulation. After transfer to a tertiary center, rigid l

Vocal Cord Dysfunction / Inducible Laryngeal Obstruction (VCD/ILO) as a Mimic of Anaphylaxis: A Retrospective Cohort Study.

BACKGROUND: Vocal cord dysfunction (VCD), or inducible laryngeal obstruction (ILO), is an episodic upper airway disorder that can mimic anaphylaxis. OBJECTIVE: To determine the prevalence of VCD/ILO among adults referred to allergy clinic for suspected anaphylaxis and to identify clinical markers that distinguish VCD/ILO from anaphylaxis. METHODS: We conducted a retrospective study of adults referred for suspected anaphylaxis to a tertiary allergy clinic in Newcastle, Australia, in 2023. Patients were classified as confirmed VCD/ILO (laryngoscopy-proven), suspected VCD/ILO (spirometry findin

Right vagal nerve stimulation for epilepsy - case series and systematic review of the literature.

BACKGROUND: Vagus nerve stimulation (VNS) is an established therapy for refractory epilepsy, traditionally implanted on the left to avoid potential cardiac complications attributed to right-sided placement (R-VNS). These concerns are largely based on animal models, with limited human evidence. METHODS: To gain a deeper understanding of the field, we conducted a systematic review of all published cases of R-VNS and a retrospective review of all patients in our quaternary neurosurgical center (Vancouver General Hospital) who underwent R-VNS. RESULTS: The review identified nine studies involvin

Interrater Reliability of Point-of-Care Ultrasound for Predicting Post-Extubation Stridor in Pediatrics.

OBJECTIVES: Post-extubation stridor (PES) is a serious complication in pediatric patients following endotracheal intubation and is linked to laryngeal edema. Prior studies suggest that the laryngeal air column width difference (LACWD), measured by point-of-care ultrasound (POCUS), may predict PES. There is a critical need to evaluate whether such techniques can be reliably reproduced among typical providers in the pediatric critical care setting. We aimed to assess the predictive value of LACWD for PES, examine its correlation with endotracheal tube (ETT) cuff pressure, and determine whether L

Cries, Stridor, and Clues: Unmasking a Rare Airway Obstruction in an Infant.

Laryngomalacia is the predominant cause of airway obstruction resulting in stridor among newborns, constituting a significant percentage of all congenital stridor instances. Vallecular cysts, though rare, represent an important differential diagnosis, responsible for a notable percentage of all congenital laryngeal cysts. The frequency of vallecular cysts is considered low among newborns. We report a vallecular cyst case in a baby who had respiratory distress and inspiratory stridor. Diagnosis was made by direct laryngoscopy, and the cyst was completely excised, resulting in the resolution of

Subglottic inflammatory myofibroblastic tumor: A rare case report in a pediatric patient.

Background: Inflammatory myofibroblastic tumour (IMT) of the pediatric laryngotracheal complex is exceedingly rare, with fewer than nine subglottic IMT cases reported in the literature up to 2025. We present an additional case and highlight the importance of early recognition. Case presentation: A previously healthy 10-year-old boy presented with a 3-week history of progressive dyspnea on exertion and nocturnal biphasic stridor, initially mislabelled as refractory asthma. Contrast-enhanced CT revealed a left-sided subglottic soft-tissue mass that reduced the airway lumen to 2.7 mm, while rigid

Long-term remission in a cat with laryngeal squamous cell carcinoma treated with stereotactic radiation therapy.

CASE SUMMARY: A 14-year-old spayed female domestic shorthair cat was referred for evaluation of a laryngeal mass. The cat had a 2-month history of stridor and hoarseness, without any reported decrease in appetite. Laryngeal ultrasonography, fluoroscopy, endoscopy and contrast-enhanced CT revealed a 1.7 cm laryngeal mass without metastasis. A biopsy obtained under laryngoscopic guidance confirmed squamous cell carcinoma (SCC). The cat underwent stereotactic radiation therapy (SRT) in three 10 Gy fractions on consecutive days (total 30 Gy). Mild drooling occurred during treatment and was control
