A Creepy Case of Recurrent Stridor: Circumferential Subglottic Hemangioma Diagnosed by Empiric Propranolol.

Subglottic hemangioma (SGH) is a rare but potentially fatal cause of infantile stridor that is frequently misdiagnosed as laryngomalacia, croup, or idiopathic subglottic stenosis. Circumferential involvement is an uncommon and particularly dangerous variant whose endoscopic appearance overlaps with that of other concentric subglottic lesions, including congenital cricoid cartilage malformation and idiopathic subglottic stenosis. We present a two-week-old female with a history of preterm birth (36 5/7 weeks) and progressive biphasic stridor, cyanosis, and increasing respiratory distress. Operat

Postoperative Nasogastric Tube Syndrome Without Throat Pain Following Emergency Abdominal Surgery: A Case Report.

Vocal cord paralysis is a recognized complication of tracheal and nasogastric tube insertion. When associated with nasogastric tube insertion, it is termed nasogastric tube syndrome (NGTS). This syndrome is classically characterized by a triad of throat pain, presence of a nasogastric tube, and vocal cord paralysis, which is often bilateral. A 75-year-old man developed stridor and dyspnea without throat pain 12 days after tracheal extubation, while a nasogastric tube had remained in place for 18 days. The following day, laryngoscopy performed by an otolaryngologist revealed median vocal cord f

Subglottic stenosis in granulomatosis with polyangiitis: clinical, histopathological and therapeutic considerations with presentation of two cases.

Granulomatosis with polyangiitis (GPA), once known as Wegener's granulomatosis (WG), is an uncommon systemic vasculitic disorder characterized by inflammation of the vascular walls, particularly affecting the respiratory tract. One serious complication is subglottic stenosis (SGS), a narrowing of the airway just below the vocal cords. While uncommon, it can be life-threatening and may show up as stridor, dyspnea, or voice changes. Sometimes, it is the first or only sign that someone has GPA. Diagnosing this condition takes a combination of clinical evaluation, imaging, endoscopy, and blood tes

Botulinum Toxin Treatment for Uncommon Phenotypes of Laryngeal Adductor Breathing Dystonia.

Laryngeal adductor breathing dystonia (LABD) is a rare form of focal, task-specific respiratory dystonia affecting the laryngeal muscles of unknown aetiology. Unlike classical laryngeal dystonia (spasmodic dysphonia), LABD is not primarily characterised by impaired speech, but rather by dysfunction of respiratory laryngeal control. The hallmark pathophysiological alteration consists of involuntary, action-induced adductor spasms of the laryngeal muscles during respiration, particularly during inspiration. LABD must be distinguished from inducible laryngeal obstruction (ILO), a broader, heterog

Acute Laryngeal Dystonia Mimicking Anaphylaxis Following Antipsychotic Initiation: A Case Report.

Acute laryngeal dystonia (ALD) is an uncommon but potentially life-threatening adverse effect of dopamine D2 receptor-blocking agents. Because airway symptoms predominate, it may be misdiagnosed as anaphylaxis. A 24-year-old woman developed recurrent episodes of throat tightness, dysphonia and stridor within days of antipsychotic initiation and subsequent dose escalation. Initial episodes were treated as anaphylaxis based on airway symptoms and mild tachycardia. Serum tryptase was mildly elevated, further contributing to diagnostic uncertainty. However, the absence of cutaneous findings or hyp

The Many Faces of Dengue Infection: Dengue Infection Masquerading as Acute Airway Edema.

Acute supraglottic edema is an uncommon but potentially fatal cause of upper-airway obstruction. Although dengue infection is associated with diverse systemic manifestations, isolated upper-airway involvement is exceedingly rare and may pose a significant diagnostic challenge. We report an elderly woman who initially presented with features of acute stroke but was subsequently found to have life-threatening airway edema as a manifestation of acute dengue infection. Neuroimaging excluded acute cerebrovascular disease. Within six hours, she rapidly developed inspiratory stridor, hypoxemia, and h

Biomarker-Guided Diagnosis of Riboflavin Transporter Deficiency Presenting as Seronegative Myasthenia Gravis: A Short Report from a Consanguineous Family.

BACKGROUND: Brown-Vialetto-Van Laere syndrome (BVVLS) is a rare, treatable neurodegenerative disorder caused by riboflavin transporter gene variants (SLC52A2, SLC52A3). Early symptoms can mimic neuromuscular junction disorders, causing diagnostic delays. CASE PRESENTATION: A 3.5-year-old girl from consanguineous parents presented at 2.5 years with isolated bilateral ptosis, later developing dysphagia, dysphonia, facial diplegia, stridor, and respiratory compromise. Examination revealed tongue atrophy with fasciculations and bulbar weakness but normal limb strength. Diagnosed with seronegative

Hairy Polyp Causing Infant Respiratory Distress: A Case Report.

We present the case of a newborn who presented in respiratory distress with a hairy polyp that was larger than 3 cm arising from the oropharynx. Whilst performing emergency intubation, an oropharyngeal mass was detected causing airway obstruction in association with a cleft palate. The characteristics and extent of the lesion were confirmed through imaging. Surgical removal was undertaken using coblation. Histology confirmed a 34 x 22 x 16 mm lesion consisting of keratinized stratified squamous epithelium with pilosebaceous units and a well-formed tooth bell seen within the specimen. The child

"Scorched air": A narrative review of inhalational injuries in fire onboard ships.

Fires aboard ships remain a serious hazard, posing significant risks to crew members due to the confined and often poorly ventilated environments. Modern ships, especially military vessels with airtight compartments, are particularly vulnerable to rapid accumulation of smoke, even from minor fires. In such settings, smoke inhalation-not burns-is the leading cause of fire-related mortality. This article explores the pathophysiology, clinical manifestations, and management of inhalational injuries resulting from onboard ship fires. It aims to highlight the unique challenges faced in maritime set

The effect of aryepiglottic fold release as an adjuvant step in surgery for Type I laryngomalacia.

OBJECTIVE: This study aims to evaluate the efficacy of unilateral aryepiglottic fold release as an adjunctive surgical step in the management of Type I laryngomalacia. METHODS: This study was prospectively conducted on a sample of paediatric patients with laryngomalacia. Our patients were randomly divided into Group A (study group, n = 25), undergoing adjuvant aryepiglottic fold release (AEFR), and Group B (control group, n = 25), who did not undergo adjuvant AEFR. Demographic data, clinical features, and outcome measures were compared between groups. RESULTS: A highly significant difference

Trifid epiglottis in a neonate with choanal atresia and hydrometrocolpos: the third reported case and the first symptomatic neonatal presentation.

BACKGROUND: Trifid epiglottis (TE) is an extremely rare congenital laryngeal anomaly. To the best of our knowledge, only two cases have previously been reported in the literature. Both of them have been incidentally detected and associated with polydactyly. CASE PRESENTATION: We report the third known case of TE in a two-day-old female neonate who was admitted to the emergency department due to abdominal distension, oliguria, and poor feeding within the first 24 h of life. Examination revealed stridor during crying, left postaxial polydactyly, right syndactyly, and an anteriorly displaced anu

Shifting growth-climate limitations in Canadian forests under recent climate change.

INTRODUCTION: Although the etiology of osteoporosis is different between men and women, the underlying pathophysiological mechanism is similar, namely an absolute or relative increase in bone resorption, leading to progressive bone loss. Transforming growth factor (TGF)-beta1 is a growth factor in human bone, which is produced by osteoblasts, and which has various effects on osteoclasts and osteoblasts. The aim of our study was to determine serum TGF-beta1 levels in male patients with idiopathic osteoporosis. METHODS: Twenty five males with idiopathic osteoporosis and 25 age-matched controls

Acute Poisoning-Induced Rhabdomyolysis and Related Complications: A Report of Two Cases.

Rhabdomyolysis following acute poisoning is a rare complication. Acute poisoning with profenofos, an organophosphorus insecticide that irreversibly inhibits acetylcholinesterase, and Super Vasmol 33, a hair dye containing para-phenylenediamine, is associated with significant morbidity and mortality. Although rhabdomyolysis is an uncommon complication, it may precipitate acute kidney injury (AKI) and multiorgan dysfunction syndrome (MODS). Here, we report two cases of rhabdomyolysis following acute poisoning. The first case involved a 40-year-old nonalcoholic male admitted after self-ingestion

Dynamic Supraglottic Airway Collapse Diagnosed Using Airway Point-of-care Ultrasound: A Case Report.

INTRODUCTION: Acquired idiopathic laryngomalacia causing dynamic airway collapse is rare in adolescents and may be overlooked because it mimics more common causes of acute dyspnoea and stridor in the emergency department (ED). Airway point-of-care ultrasound (POCUS) provides a rapid, non-invasive means to visualize dynamic supraglottic obstruction when laryngoscopy is not immediately feasible or tolerated. CASE REPORT: A 13-year-old boy presented with sudden-onset respiratory distress and dyspnoea that worsened on lying flat and improved when sitting upright, without history of fever, trauma,

The Intersection of Autoimmunity and Sleep: Anti-IgLON5 as the Key Player in a Case Series.

Anti-IgLON5 disease is a recently identified autoimmune condition characterized by unique sleep disturbances and a broad spectrum of neurological symptoms. Alongside prominent manifestations such as bulbar dysfunction, gait abnormalities, movement disorders, and cognitive decline, affected individuals often experience severe and characteristic sleep disorders. These include parasomnias during both rapid eye movement (REM) and non-REM (NREM) sleep, stridor, and obstructive sleep apnea (OSA). The presence of antibodies targeting IgLON5, a cell adhesion protein with a still unclear role, is a def

Primary tracheal Ewing sarcoma.

Primary Ewing sarcoma of the trachea is an exceptionally uncommon malignancy that often presents with non-specific airway symptoms, leading to diagnostic delays. We report a woman in her early 30s with progressive cough, dyspnoea, hoarseness and stridor who was initially mismanaged as having common airway disease. Imaging revealed a near-obstructing tracheal mass, which was successfully excised bronchoscopically. Histopathology examination, immunohistochemistry and fluorescent in situ hybridisation confirmed the diagnosis of Ewing sarcoma with EWSR1 gene rearrangement. The patient was subseque
