Acute airway foreign bodies are an uncommon but life-threatening pathology encountered in the emergency department. We present the case of a 41 year old female who presented for acute obstruction of the posterior pharynx following attempted ingestion of a glass methamphetamine pipe in efforts to conceal evidence from law enforcement officers. She exhibited mild trismus and muffled voice. No respiratory distress, stridor or hypoxia was appreciated. Workup revealed a vertically oriented cylindrical glass foreign body, approximately 10 cm spanning the distance between the proximal esophagus and p

INTRODUCTION: Nodular fasciitis (NF) is a benign, rapidly growing, non-neoplastic myofibroblastic lesion of the subcutaneous tissue affecting various anatomical sites throughout the body. NF of the trachea is rare, and its occurrence in the pediatric age group is highly uncommon in clinical practice.
CASE REPORT: 2-year-old female child who presented to the emergency department with severe stridor causing critical airway obstruction. The mass was successfully excised via an emergent surgical procedure, and the definitive diagnosis of nodular fasciitis was established through histopathology ex

Cervical osteophytosis, a common manifestation of degenerative spondylosis, can rarely exert mass effect to compromise adjacent airway structures. Acquired tracheomalacia secondary to anterior cervical osteophytes is exceptionally uncommon and results from dynamic airway collapse due to weakened tracheal cartilaginous support. Tracheomalacia may present with nonspecific respiratory symptoms such as dyspnea, cough, and orthopnea, and diagnosis is best established with direct visualization on bronchoscopy. We report a rare case of localized upper tracheal tracheomalacia caused by cervical diffus

We report the case of a 70-year-old man with unresectable esophageal squamous cell carcinoma who achieved durable marked response with combined immune checkpoint inhibitor (ICI) therapy following radiotherapy (RT) alone. The patient presented with dysphagia and stridor due to a bulky esophageal tumor and cervical lymph node metastasis compressing the trachea. He was diagnosed with cT3N1M0 (Stage IIIB; 8th edition of the TNM classification) or cT4(101R-Trachea)N1M0 (Stage IVA; 12th edition of the Japanese classification). He underwent RT alone (total dose, 50 Gy), resulting in substantial tumor

Stridor is a common presenting complaint in infancy. Its aetiology ranges from benign to severe, life-threatening conditions. We report here a 2-month-old child who presented with respiratory distress and stridor since 20 days of life, with intermittent worsening. On examination, there was a haemangioma on the right ear lobe. A similar lesion in the airway was suspected, and flexible bronchoscopy revealed an airway haemangioma in the larynx extending up to the carina. Further workup was performed given haemangioma in the facial region that led to a diagnosis of the Posterior fossa malformation

BACKGROUND: Laryngomalacia is the most common cause of inspiratory stridor in infancy. While most mild cases resolve spontaneously, severe cases may require surgical intervention. We report a case of severe laryngomalacia successfully treated with correction of glosso-larynx (CGL), a surgical procedure originally developed for ankyloglossia with deviation of the epiglottis and larynx (ADEL).
METHODS: A 2-month-old infant with severe symptoms was evaluated using objective anatomical and functional metrics. The patient underwent CGL under local anesthesia to release restrictive tension on the h

Impaction of a foreign body in the larynx is commonly seen in children but remains a rare occurrence in adults. Laryngeal impaction, particularly of a coin, is exceedingly uncommon and carries a potential risk of airway compromise. Prompt history taking, clinical examination, and radiographic evaluation are essential for diagnosis. We report a rare case of an adult male with a laryngeal coin impacted sagittally between the true vocal cords, presenting solely with isolated dysphonia and without dyspnea or stridor. This case is atypical due to adult presentation, minimal symptoms despite glottic

This report highlights the importance of differential diagnosis of laryngeal obstructive airway pathologies, as well as the growing value of endoscopic techniques, such as cryobiopsy, in achieving a definitive, accurate diagnosis. This is an 8-month-old infant who presented with signs of airway obstruction and a history consistent with foreign body aspiration. During endoscopy under general anesthesia, a supraglottic tumor was observed, which was resected using a cryobiopsy probe, a method that is not widely used in pediatrics and in our setting. Postoperative pathological anatomy confirmed th

Although uncommon, laryngotracheal injuries carry a significant risk of mortality. This rare case of isolated laryngeal injury underscores the importance of recognizing the various potential presentations of such injuries and highlights the evidence-based management strategies that every emergency physician should consider when treating potential airway trauma. A 45-year-old male motorcyclist was involved in a road traffic accident during the night and presented to the emergency department the following morning. The patient complained of breathlessness, neck pain, hoarseness of voice, and nois

Congenital laryngomalacia is the most common cause of infant stridor and arises from interacting structural, neuromuscular, and inflammatory mechanisms that produce dynamic supraglottic collapse. Disease severity spans mild stridor to significant obstruction, aspiration, and failure to thrive, often influenced by comorbid medical conditions. Diagnosis relies on flexible laryngoscopy supported by instrumental swallowing studies and microdirect laryngoscopy and bronchoscopy when indicated. Most infants respond to conservative management, particularly targeted feeding modifications, while acid su

Laryngomalacia is the most common congenital anomaly of the larynx and a leading cause of inspiratory stridor in infants. Stridor and noisy breathing are key clinical manifestations that may signify varying degrees of respiratory compromise and can provide insight into the level of airway obstruction. A comprehensive evaluation of the upper airway is essential in children suspected of having laryngomalacia to ensure accurate diagnosis and appropriate management of both the primary condition and any associated comorbidities. Laryngomalacia presents along a spectrum of severity, with mild cases

The subglottic region is the narrowest part of the pediatric airway and is susceptible to stenosis both congenitally or through acquired means. Thorough evaluation and correct diagnosis of the airway pathology is critical for choosing appropriate intervention and optimizing outcomes. This review details the pathogenesis, diagnostic tools, preoperative workup, and surgical interventions for pediatric subglottic stenosis.

BACKGROUND: Laryngeal injuries, especially cricoid cartilage fractures, are rare but may present with airway emergencies, and appropriate airway management is vital.
CASE PRESENTATION: A 24-year-old man admitted to the emergency room with isolated blunt cervical trauma after a motorcycle accident. The patient presented dysphonia, stridor, hemoptysis, cervical subcutaneous emphysema, and increasing respiratory distress. Oral intubation was initially successful, however, worsening subcutaneous emphysema and loss of effective ventilation suggested airway disruption. Emergency cricothyrotomy was

Postintubation tracheal stenosis, a complication of prolonged mechanical ventilation via orotracheal intubation, often presents with stridor and dyspnea due to tracheal fibrosis and scarring, typically distal to the cricoid cartilage. Its complex nature necessitates a multidisciplinary approach and expertise in tracheal, thyroid, mediastinal, and neurovascular surgical techniques. This case report details the clinical management of postintubation tracheal stenosis, highlighting a successful tracheal resection and anastomosis procedure. A patient, S.A., presented at the Thoracic Surgery Departm

Severe central airway obstruction presents major challenges during bronchoscopic airway interventions. General anaesthesia with neuromuscular blockade using rigid bronchoscopy and positive-pressure ventilation is a commonly used technique for tracheal stent placement in many centres. However, patient-specific anatomical and physiological factors may occasionally influence anaesthetic planning. We report the case of an elderly woman with metastatic thyroid carcinoma who presented with progressive dyspnoea and stridor due to critical extrinsic tracheal compression. Following multidisciplinary di

Vocal cord paralysis (VCP) is an uncommon but clinically significant complication of prolonged or repeated intubation in critically ill patients. Bilateral cases may cause acute stridor and airway compromise, whereas unilateral cases present with hoarseness or aspiration, often delaying recognition and increasing morbidity. We report 4 intensive care unit (ICU) patients with postintubation VCP diagnosed by fiberoptic laryngoscopy. Presentations ranged from bilateral injury resulting in stridor to unilateral dysfunction, with management strategies including observation, tracheostomy, and poster

We report the case of a 30-year-old female, non-smoker, with no known allergies or occupational exposures, presenting with a two-year history of recurrent stridor and wheezing, recently exacerbating over the past two months. Physical examination and routine laboratory tests were largely unremarkable. Pulmonary function tests were within normal limits, with no significant bronchodilator response. Chest CT revealed a 15 × 13 mm perihilar right-sided lesion adjacent to the right pulmonary artery. Bronchoscopy identified a nearly obstructive, highly vascularized endobronchial lesion at the origin
