Explainable Machine Learning to Identify Clinical, Environmental, and Social Factors Associated with Diagnosed Pediatric Asthma.

BACKGROUND: Pediatric asthma reflects clinical, physiologic, environmental, and social determinants, yet most classification models use clinical variables alone and are difficult to interpret. OBJECTIVE: To use explainable machine learning on nationally representative data to identify and rank factors associated with a reported physician diagnosis of asthma in U.S. children, and to test whether fewer higher-ranked features retain full performance. METHODS: We analyzed 6,567 children aged 6 to 17 years in NHANES 2007-2012 (weighted asthma prevalence 18.8%). Of 477 harmonized variables, 22 ent

Case Report: Multisystem response to dupilumab in a child with atopic dermatitis, asthma, eosinophilic esophagitis, and other gastrointestinal manifestations.

BACKGROUND: Atopic dermatitis (AD), asthma, and eosinophilic esophagitis (EoE) may coexist because they share type 2 inflammatory pathways. Evidence for dupilumab in children with EoE and other gastrointestinal manifestations remains limited. CASE PRESENTATION: A 7-year-old girl had poorly controlled AD, recurrent asthma, EoE (>100 eosinophils/high-power field), and lower-density eosinophilic infiltration in gastric, ileal, and colonic biopsies. These extraesophageal findings did not uniformly satisfy proposed thresholds for definite non-EoE eosinophilic gastrointestinal disease. Severe AD de

Infant respiratory syncytial virus and childhood asthma: a nationwide registry-based, sibling-controlled, and genome-wide association study.

BACKGROUND: Early-life respiratory syncytial virus (RSV) infection is associated with childhood recurrent wheeze or asthma (RW/A), but causality and shared genetic liability remain unclear. METHODS: We combined Finnish nationwide registries and Nordic genetic cohorts. First, in 965,312 Finnish children born between 1998 and 2014, we defined severe RSV as RSV hospitalisation before age 1 year, and recurrent wheezing or asthma (RW/A) as inhaled medication reimbursement between ages 1 and 7 years, and compared medication and eosinophil trajectories by RSV history. Second, we assessed familial co

Transient hypogammaglobulinemia of infancy: revisiting an old entity.

INTRODUCTION: Transient hypogammaglobulinemia of infancy (THI) is one of the most common primary antibody deficiencies presenting in early childhood and is characterized by delayed maturation of humoral immunity with spontaneous normalization of immunoglobulin G (IgG) levels in most patients. Although THI has traditionally been considered a benign, self-limited disorder, increasing evidence suggests that it represents a heterogeneous immunologic condition with variable clinical manifestations, immune abnormalities, and long-term outcomes. AREAS COVERED: This narrative review summarizes eviden

Interobserver Agreement in Pediatric Asthma Examination Findings Between Emergency Medical Services and Emergency Department Clinicians.

OBJECTIVES: Asthma exacerbations constitute 10-15% of all pediatric emergency medical services (EMS) calls. There are several validated scoring tools to assess acute asthma severity, which have good inter-rater reliability and correlate with clinical outcomes in the emergency department (ED), but have not been validated with EMS clinicians. This study sought to assess the level of agreement between EMS and ED clinicians on components of asthma scoring tools to inform a prehospital asthma severity scoring tool. METHODS: We conducted a prospective observational study of children 2-18 years old

Identification of clinical phenotypes and development of a predictive model for rapid lung function decline in COPD patients.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) exhibits significant clinical heterogeneity. This study integrates phenotype identification with longitudinal lung function analysis to develop a prediction tool for rapid decline in a large Chinese cohort. METHODS: This study included 38,863 COPD patients from a national multicenter screening program. Latent class analysis identified clinical phenotypes. Among 1,215 patients with baseline and 24-month follow-up spirometry, rapid decline was defined as annual FEV1 decline ≥40 mL or FEV1% predicted decline >1.5%. LASSO regression and the

Intrathoracic Adipose Tissue and Airway Structure, Lung Function, and Respiratory Symptoms: The Framingham Heart Study.

BACKGROUND: Obesity is associated with airway disease, but the contribution of specific thoracic adipose depots remains unclear. We examined whether intrathoracic adipose tissue (ITAT), defined as adipose tissue within the thoracic cavity excluding pericardial adipose tissue (PAT), was associated with airway phenotypes. METHODS: We included Framingham Heart Study participants with CT-derived thoracic adipose tissue measures. Cross-sectional associations of ITAT with CT-based airway wall and lumen measurements, spirometry, and respiratory symptoms were evaluated using multivariable regression.

An analysis of occupational exposure to particulate matter and erythrocyte morphological abnormalities among charcoal-grill restaurant workers.

BACKGROUND: Workers in charcoal- and wood-fired grill restaurants are exposed to high concentrations of combustion-derived particulate matter (PM) and carbon monoxide during long shifts in poorly ventilated kitchens. Whether this exposure is associated with measurable changes in circulating erythrocyte morphology is not established. METHODS: This study used a comparative cross-sectional design and compared charcoal-grill workers (n = 25) with a group of office personnel (n = 25) in Kocaeli University. At each occupational unit, the researchers assessed indoor air conditions, measuring particu

Endobronchial fibroepithelial polyp presenting as persistent wheezing in a 9-year-old girl: a case report.

Fibroepithelial polyps are benign tumours that usually arise from cutaneous or mucosal surfaces. In children they most commonly involve the skin, oral cavity, and genitourinary tract, whereas endobronchial localization is exceptionally rare. We report the case of a 9-year-old girl with persistent biphasic wheezing, cough and dyspnoea on exertion despite appropriate asthma-directed treatment. High-resolution computed tomography of the chest revealed an endobronchial lesion associated with distal bronchiectatic changes, and bronchoscopy confirmed the presence of a protruding endobronchial polyp.

Safety and Efficacy of [177Lu]Lu-DOTATATE in Neuroendocrine Tumors at a Tertiary Cancer Center in Jordan: A Multinational Arab Cohort.

The outbreak of coronavirus named COVID-19, initially identified in Wuhan, China in December 2019, has spread rapidly at the global scale. Most countries have rapidly stopped almost all activities including industry, services and transportation of goods and people, thus decreasing air pollution in an unprecedented way, and providing a unique opportunity to study air pollutants. While satellite data have provided visual evidence for the global reduction in air pollution such as nitrogen dioxide (NO2) worldwide, precise and quantitative information is missing at the local scale. Here we studied

Anterior Osteophytes at L3-L4 as a Primary Predictor of Adjacent Segment Degeneration After Lumbar Instrumented Fusion.

INTRODUCTION: Degenerative lumbar spine disorder (DLSD) is a significant public health concern, presenting with low back pain, radicular leg pain, and reduced quality of life. Although conservative management is the first-line treatment, some patients require surgery due to persistent symptoms or progressive neurological deficits. Lumbar instrumented fusion is a commonly performed procedure for DLSD. Adjacent segment degeneration (ASD) is a frequent complication after spinal fusion, occurring at either the cranial or caudal side of the fusion mass. While the role of spinopelvic parameters in A

Mitochondrial metabolic dysfunction in airway epithelial cells promotes goblet cell metaplasia and more severe asthma.

Type 2 (T2) inflammation is a defining feature of asthma and associated with epithelial dysfunction, including goblet cell metaplasia and ciliary loss. Here, we investigated whether T2 inflammation and associated 15 lipoxygenase-1 (15LO1) activity alters mitochondrial metabolism in airway epithelial cells (AECs) and contributes to disease pathophysiology. Freshly isolated AECs from T2-high asthmatic participants exhibited increased oxygen consumption rates (OCR), including elevated basal respiration and proton leak, alongside reduced coupling efficiency compared to T2-low participants. These f
