BACKGROUND: Particulate matter (PM) with distinct physicochemical properties may drive differential respiratory health risks. We investigated associations of long-term exposure to size- and constituent-specific PM with chronic respiratory diseases (CRDs) and evaluated respiratory benefits linked to PM composition changes under China's air control policies.
METHODS: A prospective cohort of 2100 adults (baseline mean age, 57.2 years) in Guangzhou, China, was followed between 2008 and 2021. Residential exposure to size-fractionated PM (PM1, PM2.5, PM10) and six PM2.5 chemical constituents were e

In this study, we compared symptoms, comorbidities, and healthcare burden between childhood-onset asthma (< 18 years) and early adult-onset (18-39 years) and late adult-onset asthma (≥ 40 years). Among 3546 participants with data on physician-diagnosed asthma and onset age, 46.4% were defined as adult-onset [864 (24.4%) had early adult-onset asthma (18-39 years) and 782 (22.1%) had late adult-onset asthma (≥ 40 years)], which, compared to childhood-onset asthma, presented with more complex, multi-symptom profiles, including productive cough, sputum production, but less wheezing. OBJECTIVE: Atopic dermatitis (AD) is frequently associated with respiratory comorbidities, yet data on pulmonary function in infancy remain scarce. This study aimed to evaluate pulmonary function in infants with AD using tidal breath analysis (TBA) and to compare these findings with those of wheezy infants and healthy controls (HC).
METHODS: A total of 115 infants under three years of age were categorized into three groups: infants with AD without any respiratory disease or symptoms (n = 50), infants with wheezing (n = 35), and HCs (n = 30), and their data were retrospectively analyzed. Pulmo

Although the incidence of childhood asthma and wheezing (AW) is increasing worldwide, the impact of ambient temperature on this is unclear. An infant birth cohort was established, and the children were followed up until two years of age. During this period, the exposure levels to atmospheric factors were individually evaluated on a daily basis as well as covariates information was collected. Both a nested case-control study and a case-crossover design were embedded to control for confounding bias and cross-validate the association between ambient temperature and childhood AW. A total of 5819 c

Recently, increasing epidemiological studies have suggested a potential association between swimming pool attendance and allergic outcomes with inconsistent findings. This systematic review and meta-analysis aims to elucidate the association between swimming pool attendance and the risk of allergic diseases. We systematically searched PubMed, Web of Science, Scopus, and Embase to identify relevant studies published up to August 8, 2025, and performed a meta-analysis of the extracted data. Furthermore, subgroup analyses, sensitivity analyses, and assessments of publication bias were also conduc

INTRODUCTION: Asthma represents a significant global public health concern, especially in children, due to its high prevalence and burden on the healthcare system. Early-life exposures, particularly to antibiotics and bronchiolitis, have been independently linked to an increased risk of wheezing and subsequent asthma. Despite extensive research on these risk factors, their interplay and combined impact throughout early childhood remains unclear. This study aims to clarify these relationships by investigating the mediating role of recurrent wheezing in the inception of childhood asthma.

BACKGROUND: Adolescence is a critical developmental stage during which environmental, metabolic, and psychosocial factors co-occur and may interact, influencing health and creating a syndemic context that remains insufficiently explored.
OBJECTIVE: We analyzed a syndemic model, considering socioeconomic determinants and metabolic risk associated with asthma and depression in adolescents.
METHODS: This population-based study (n = 2515) utilized data from the RPS Brazilian Consortium Cohorts at follow-up of 18-19 years in São Luís, Brazil. A theoretical model analyzed the association among soc

History A 69-year-old woman presented to her primary care physician with new-onset dysphonia and increasing shortness of breath, without chest pain. She described her voice as "going in and out" intermittently and reported an occasional cough. She also reported globus sensation without dysphagia. Her medical history included long-standing allergic rhinitis, arthritis, anemia, and gastroesophageal reflux disease. She had a documented history of asthma and had been using the inhaled steroid fluticasone (100 µg/d) since at least 2013, along with an albuterol inhaler as needed for the past 3 years

BACKGROUND: Short-term exposure to air pollution is a known trigger for pediatric wheeze-associated disorders (WAD), such as acute asthma and virus-induced wheezing, yet few air pollution studies account for concurrent viral circulation, which may confound or modify the observed associations. This study aims to assess whether viral circulation confounds or modifies the association between daily levels of air pollution and pediatric WAD emergency department (ED) visits.
METHODS: We conducted a time-series analysis of 12,603 WAD ED visits among children 2-18 years old across eight hospitals in

BACKGROUND: Wheezing in the pre-school aged group (under 5 years) is a common presentation and significant healthcare burden. It is a heterogenous presentation representing a spectrum of phenotypes, and although the causes may be multifactorial, viral infection is the most common trigger, with rhinovirus and Respiratory Syncytial Virus (RSV) being the most commonly detected. Rigorous evidence-based guidance for the acute management of preschool wheeze (PW) with respect to which children are likely to benefit from oral corticosteroid therapy (OCS) is lacking. RCTs of OCS use in PW have not adeq

Background: Impedance pneumography (IP) is a non-invasive technique for assessing tidal breathing in young children and enables home-based recordings without active patient cooperation. By deriving tidal breathing flow-volume (TBFV) curves and indices such as the expiratory variability index (EVI), IP has been proposed as a tool for identifying obstructive breathing patterns and monitoring airway function in early childhood. However, its clinical role in asthma and wheezing disorders has not been systematically evaluated. This review aimed to assess the evidence of IP in differentiating health

Background: Mucous stools in infancy are commonly attributed to non-IgE-mediated gastrointestinal food allergies and are generally considered transient and benign. However, whether mucous stools may indicate an atopy-prone clinical phenotype and relate to later respiratory atopy remains insufficiently explored. Objective: To evaluate the long-term risk of respiratory atopy (asthma and/or allergic rhinitis) in infants presenting with mucous stools during the first year of life and to identify early clinical predictors of this risk. Methods: This retrospective cohort study included infants who p

BACKGROUND: Rhinovirus C (RV-C) is one of three species of rhinoviruses (RVs), which cause the common cold, preschool wheezing illnesses and exacerbations of asthma. RV-C types are more virulent, especially in children, but progress in developing treatments is limited by difficulties in generating high-titer virus preparations. The goals of this study were to optimize methods for large-scale production and purification of RV-C to facilitate structure and immune response studies.
METHODS: We optimized protocols for the propagation and purification of RV-C15a, a clinical isolate adapted to HeLa

Growing evidence has suggested that probiotic consumption can decrease the incidence of Atopic dermatitis (AD) in infants and children. However, even meta-analyses have reported uncertain findings. The current umbrella meta-analysis aimed to assess the findings of multiple meta-analyses on the efficacy of probiotic supplementation on AD and other atopic manifestations in infants and children. A systematic search of the Literature was carried out in PubMed, ISI Web of Knowledge, Scopus, Cochrane Central Library, EMBASE, and Google Scholar from inception up to September 2024. BACKGROUND: Mycoplasma pneumoniae pneumonia (MPP) is a prevalent respiratory infection. Refractory MPP (RMPP) presents more severe symptoms and requires more intensive treatment compared to general MPP (GMPP). This study aimed to identify distinguishing clinical, laboratory, and radiological characteristics between RMPP and GMPP and develop an early predictive model for RMPP risk stratification.
METHODS: A total of 568 patients, including 130 RMPP cases and 438 GMPP cases, were enrolled. Clinical information, laboratory tests, and radiological features were compared. Univariate and multivaria

PURPOSE OF REVIEW: Pesticides, intertwined with agriculture, public health, and personal comfort, are tremendously diverse in structure, mechanisms of action, and environmental persistence. We searched for recent data and research perspectives on the association between pesticide exposure and the genesis and severity of non-occupational asthma and allergic airway diseases. RECENT FINDINGS: Early life exposure to pesticides is associated with increased prevalence of asthma, wheezing and allergic rhinitis in preschool children. Biomarker studies provide a quantitative link between pesticide expo

Excessive dynamic airway collapse (EDAC) is characterized by weakness in the posterior membranous wall of the airway, which results in more than 50% narrowing of the central airway lumen during expiration. EDAC differs from tracheobronchomalacia, which involves the weakening of the cartilage rather than the membranous wall. EDAC poses a diagnostic challenge due to overlapping symptoms with chronic obstructive pulmonary disease and asthma, including dyspnea, cough, and wheezing. The diagnosis of EDAC relies on dynamic airway imaging techniques, including bronchoscopy, dynamic computed tomograph
