The Modern Environment and Childhood Asthma: The Role of Air Pollution and Heavy Metal Exposure.

PURPOSE OF REVIEW: Childhood asthma is the most common chronic non-communicable disease in children and a major global public health challenge. Although genetic predisposition contributes to asthma susceptibility, the high burden of disease-particularly in high-sociodemographic-index regions-points to a central role of environmental exposures characteristic of the modern environment. This review synthesizes current evidence linking ambient air pollution and heavy metal exposure to childhood asthma, with a focus on early-life vulnerability, underlying biological mechanisms, and implications for

Wheezing in Children.

Wheezing is a whistling sound that occurs with expiration due to narrowing or obstruction of the small airways. Up to 50% of children experience recurrent episodes of wheezing by age 5 years. Diagnosis of the cause can be difficult because of the many possible etiologies. The history should include the family history, age of onset, and pattern of wheezing (transient vs persistent). The differential diagnosis varies depending on age of onset, with younger children more commonly experiencing wheezing caused by congenital abnormalities or viral etiologies. Gastrointestinal reflux disease is anoth

Indoor air pollution in kindergartens is a stronger predictor of preschool wheezing than household pets and passive smoking.

BACKGROUND: The quality of air in kindergartens, where children spend a significant part of their time, is an important determinant of health. OBJECTIVE: This study aimed to explore the relationship between indoor air pollution in kindergartens and wheezing syndrome in preschool children. METHODS: Air pollution, specifically the concentration of particulate matter (PM), was measured in 24 kindergartens. A total of 1,794 parents of preschool children were surveyed about respiratory symptoms and environmental factors affecting their children. RESULTS: The mean age of the children was 4.07 (±

Assessing the Reduction of Recurrent Admissions Using OM-85 for the Treatment of Preschool Wheeze (ARROW): Protocol for a Multi-Centre, Randomised, Double-Blind, Placebo-Controlled Trial.

AIMS: Acute wheezing illnesses are among the most common reasons preschool-aged children are admitted to hospital. Readmission rates are high, and novel preventive strategies are required. OM-85 is an orally administered bacterial lysate that stimulates anti-viral immune responses and reduces excessive airway inflammation associated with wheezing episodes. The aim of the Assessing the Reduction of Recurrent admissions using OM-85 for the treatment of preschool Wheeze (ARROW) trial is to determine the efficacy of OM-85 in preventing hospital readmissions in children admitted to hospital with pr

Moving the goalposts from symptom control to preventing airway remodeling in preschool asthma.

Recurrent wheezing in preschool aged children is a common reason for consultation to the pediatric respirologist. However, the definition of preschool asthma, the various phenotypes and associated investigations required to define these phenotypes, the goals of therapy, and considerations for therapeutics, including biologics, are all areas needing further development. Let's begin with an illustrative case.

Long-term occupational exposure to volatile organic compounds and progressive pulmonary function decline: a 5-year cohort study in the printing industry.

The printing industry involves continuous exposure to volatile organic compounds (VOCs), potentially leading to pulmonary impairments. This study aimed to evaluate long-term pulmonary impairments among printing workers over a five-year period. A five-year retrospective cohort study (2020-2024) was conducted involving 87 operational printing workers (exposed group) and 86 administrative staff members (unexposed group). Occupational exposure to VOCs (benzene, toluene, ethylbenzene, xylene, isopropyl alcohol, ethyl acetate, ethanol) was assessed using NIOSH-based air sampling and GC-FID analysis.

Beta2-agonist and anti-inflammatory combination therapy for asthma rescue: a paradigm shift in asthma care.

The Global Initiative for Asthma (GINA) report and national and international guidelines have shifted away from short-acting beta2-agonist (SABA)-only bronchodilators as first-line asthma rescue therapy, recognizing that both bronchoconstriction and inflammation underlie the increase in symptoms preceding an exacerbation. Instead, the GINA Committee supports strategies to reduce inflammation, such as anti-inflammatory reliever (AIR) therapy (as-needed, low-dose inhaled corticosteroid [ICS]-formoterol or SABA-ICS for symptom relief) and single maintenance and reliever therapy (SMART) or mainten

The impact of neonatal respiratory distress syndrome on subsequent preschool wheezing in very preterm infants: a multicenter cohort study.

BACKGROUND: This multicenter cohort study investigated the association between neonatal Respiratory Distress Syndrome (RDS) and wheezing in preschool-aged preterm infants. METHODS: A total of 525 children born at < 32-weeks' gestation were enrolled from three tertiary hospitals in Southwest China and classified into RDS (n = 275) and non-RDS (n = 250) groups. Current wheezing (≥ 1 parental-reported episode in the past year) outcomes were assessed using the International Study on Asthma and Allergy in Childhood (ISAAC) questionnaire at 3-6 years of age. RESULTS: The RDS group showed a signifi

Effect of an electronic health record-integrated machine learning asthma risk marker on pediatrician prognostic accuracy during preschool age: a pilot randomized clinical trial.

Early identification of children at risk for persistent asthma is challenging because preschool respiratory symptoms are heterogeneous and often overlap with transient wheezing illnesses. Machine learning-based prediction models may improve risk stratification, but few have been integrated into electronic health record (EHR) systems or evaluated for their effect on clinician decision-making. We evaluated whether access to an EHR-integrated machine learning-based Passive Digital Marker (PDM) improved pediatrician prognostic accuracy for school-age asthma and assessed the tool's usability, accep

Symptomatology of SARS-CoV-2 versus seasonal coronavirus infection in healthy young children.

BACKGROUND: Since SARS‑CoV‑2 became established in humans, children encounter it from early life, similar to seasonal human coronaviruses (hCoVs). To understand how SARS‑CoV‑2 compares to other hCoVs in early childhood, we measured infections and symptoms in an unvaccinated community cohort. METHODS: The VIOOL study follows healthy children under 4 years for 16 weeks during three winter seasons (2021-2024). A total of 228 participants provided weekly nasal swabs, regardless of symptoms, tested for betacoronaviruses SARS‑CoV‑2 and OC43, and alphacoronaviruses NL63 and 229E. Daily symptoms were

A nomogram for risk prediction in patients with heart failure and diabetes: Development and validation.

Heart failure (HF) commonly coexists with type 2 diabetes mellitus (T2DM), and this combination is linked to a heavier symptom burden and less favorable clinical outcomes. In this retrospective single-center study, a total of 958 consecutive patients were included, among whom 453 had T2DM, with a mean age of 68.23 ± 5.76 years. The diagnosis of HF was confirmed by a multidisciplinary team in accordance with the European Society of Cardiology criteria, and 232 patients were found to have both T2DM and HF. Relative to diabetic patients without HF, those with HF more often presented with dyspnea

Association between early-life submicron particulate matter exposure, inducible nitric oxide synthase methylation, and children's asthma/wheezing.

The impact of PM1 (particulate matter with aerodynamic diameter ≤ 1 μm) exposure on asthma risk during specific fetal lung development windows, and its potential mediation by DNA methylation, remains understudied. Thus, we conducted a nested case-control study from the Prenatal Environments and Offspring Health cohort with children having asthma/wheezing. The results revealed a positive correlation between average prenatal PM1 exposure and childhood asthma/wheezing risk (HR = 1.39, 95 % CI: 1.02, 1.90), especially during the pseudoglandular period. PM1 exposure was negatively correlated with n
