Evaluating Cardiac Impairment From Abnormal Respiratory Patterns: Insights From a Wireless Radar and Deep Learning Study.

OBJECTIVES: Assessing the bidirectional impacts of heart function impairment and sleep-disordered breathing remains underexplored. Thus, this study analyzed respiratory patterns from a wireless radar framework to explore their associations with echocardiographic (2D-echo) measurements. METHODS: Background details, 2D-echo parameters, and biochemical data were collected from patients in a cardiology ward in northern Taiwan. Their radar-based respiratory patterns from the night before and the night of the 2D-echo were obtained, averaged, and used to derive indices such as the respiratory distur

Retrotrapezoid nucleus chemoreception: mechanisms of function and contributions to disordered breathing in disease.

The brain regulates breathing in response to changes in CO2/H+ by a process referred to as respiratory chemoreception. The retrotrapezoid nucleus (RTN) is essential for this function. RTN neurons are intrinsically activated by CO2/H+. Astrocytes contribute as well, by providing a CO2/H+-dependent purinergic drive to augment neural activity directly and indirectly by causing vasoconstriction. Here, we summarize preclinical studies in rodents that identify: (i) mechanisms of CO2/H+ detection by RTN neurons; (ii) how this information is integrated at the neural network level; and (iii) how RTN ne

Cardiopulmonary Exercise Testing in the Prognostic Assessment of Heart Failure: From a Standardized Approach to Tailored Therapeutic Strategies.

Cardiopulmonary Exercise Testing (CPET) is the gold standard for the functional assessment in patients with heart failure (HF), providing objective parameters that reflect the integrated response of the cardiovascular, respiratory, and muscular systems, in addition several CPET-derived variables have shown independent prognostic value in patients with both reduced (HFrEF) and preserved ejection fraction (HFpEF) HF. This review aims to critically analyze the main CPET prognostic variables in heart failure, highlighting their underlying pathophysiological mechanisms, their predictive capacity fo

Development of periodic breathing in awake healthy individuals under controlled progressive hypoxia.

The aim of this study was to better understand the ventilatory response to controlled progressive hypoxia in healthy individuals and identify factors influencing the development of periodic breathing under hypoxic conditions. The study was conducted in a hypoxia room at the Complementary Medical Centre in Genk, Belgium. Eighteen healthy participants were subjected to controlled progressive hypoxia, with oxygen levels ranging from 0.21 to 0.10. Oxygen saturation (SpO2) was measured using a Nellcor® PM10N pulse oximeter, and carbon dioxide levels, including end-tidal carbon dioxide (EtCO2), were

Automated hypoxia and apnea identification for neonates via enhanced respiratory signal modeling with deep learning.

Neonatal respiratory monitoring is crucial for assessing breathing patterns, but the lack of real-time clinical data limits the development of machine learning (ML) models. This study provides a synthetic signal generation framework to replicate infant respiratory cycles with physiological fidelity. The dataset simulates normal and pathological breathing patterns such as apnea, hypoxia, and periodic breathing, including Gaussian noise and exponential functions, to maintain biological realism. A feature extraction pipeline was created to examine time- and frequency-domain characteristics, enabl

Exogenous ketone monoester supplementation: a potential alternative strategy to mitigate altitude sickness?

Exposure to high-altitude reduces oxygen availability, leading to hypoxemia. To combat this physiological stressor, the body initiates a cascade of short- and long-term compensatory responses (i.e., high-altitude acclimatization). Some notable adaptations include rapid increases in ventilation, heightened sympathetic neural activity, and reductions in plasma volume during early acclimatization, followed by increases in hemoglobin mass and concentration. Despite these physiological responses, many of the ~ 40 million people who travel to high altitude regions (i.e., > 2500 m) each year, suffer

AltitudeOmics: Breathing variability at rest and during exercise across 16 days of acclimatization to hypobaric hypoxia.

Ventilatory acclimatization to high altitude is an important adaption to prolonged time at altitude. Changes in breathing variability over a 16 day acclimatization period to altitude, when awake, were quantified. We tested the hypothesis that the increase in ventilatory loop gain with acclimatization would exacerbate breathing variability at rest and during exercise and that these changes would be correlated with the changes in cerebrovascular CO2, chemoreceptor sensitivity and successful ventilatory acclimatization. Breath-by-breath ventilation, partial pressure of end-tidal O2 and CO2 in 21

A Review Study of Causes of Sudden Infant Death Syndrome.

PURPOSE: The Sudden Infant Death Syndrome (SIDS) is the unexpected death of an apparently healthy infant for which no reason can be found after a postmortem examination. The primary causes of SIDS have not been determined. The purpose of this study was to examine the primary causes of this syndrome by reviewing the research results and new theories and in view of recent experimental findings. METHODS: The theories on SIDS were reviewed. The results of computer simulations of this disorder using an isomorphic model that were published in the past were compared to new experimental results and h

[Overview of the most important (patho)physiological mechanisms of high-altitude acclimatization in healthy individuals].

More and more people are spending time at high altitudes, either recreationally or permanently. As altitude increases, the partial pressure of inspired oxygen decreases, leading to hypobaric hypoxia and triggering a wide range of physiological adaptations. This article discusses the most relevant (patho)physiological acclimatization effects at high altitudes in healthy subjects. Hypobaric hypoxia primarily affects respiration, circulation, blood, as well as sleep and brain function. Respiration responds immediately with increased ventilation (hypoxic ventilatory response), reducing CO₂ levels

Infant periodic breathing and apneic threshold.

A mathematical model was proposed to predict the role played by apneic threshold in periodic breathing in preterm infants. Prior models have mainly applied linear control theory which predicted instability but could not explain sustained periodic breathing. Apneic threshold to CO2 which has been postulated to play a major role in infant periodic breathing is a nonlinear effect and cannot be described by linear theory. Another previously unexplored nonlinear factor affecting instability is brain vascular volume change with CO2 which affects time delay to chemoreceptors. The current model explor

Medication-induced central sleep apnea: a unifying concept.

Medication-induced central sleep apnea (CSA) is one of the eight categories of causes of CSA but in the absence of awareness and careful history may be misclassified as primary CSA. While opioids are a well-known cause of respiratory depression and CSA, non-opioid medications including sodium oxybate, baclofen, valproic acid, gabapentin, and ticagrelor are less well-recognized. Opioids-induced respiratory depression and CSA are mediated primarily by µ-opioid receptors, which are abundant in the pontomedullary centers involved in breathing. The non-opioid medications, sodium oxybate, baclofen,

Effects of periodic breathing on sleep at high altitude: a randomized, placebo-controlled, crossover study using inspiratory CO2.

Hypoxia at high altitude facilitates changes in ventilatory control that can lead to nocturnal periodic breathing (nPB). Here, we introduce a placebo-controlled approach to prevent nPB by increasing inspiratory CO2 and used it to assess whether nPB contributes to the adverse effects of hypoxia on sleep architecture. In a randomized, single-blinded, crossover design, 12 men underwent two sojourns (three days/nights each, separated by 4 weeks) in hypobaric hypoxia corresponding to 4000 m altitude, with polysomnography during the first and third night of each sojourn. During all nights, subjects'

Type one chiari malformation as a cause of central sleep apnea and hypoventilation in children.

OBJECTIVES: Chiari type 1 malformation (CM1) may occasionally lead to central sleep apnea (CSA). We studied, in a large clinical cohort of pediatric CM1 patients, the effect of CM1 on breathing during sleep. METHODS: This is a retrospective single pediatric pulmonology center study with a systematic evaluation of pediatric CM1 patients under age 18 with polysomnography (PSG) during 2008-2020. Children with syndromes were excluded. All patients had undergone head and spine magnetic resonance imaging. RESULTS: We included 104 children with CM1 with a median age of 7 (interquartile range (IQR)

Mechanisms underpinning sympathoexcitation in hypoxia.

Sympathoexcitation is a hallmark of hypoxic exposure, occurring acutely, as well as persisting in acclimatised lowland populations and with generational exposure in highland native populations of the Andean and Tibetan plateaus. The mechanisms mediating altitude sympathoexcitation are multifactorial, involving alterations in both peripheral autonomic reflexes and central neural pathways, and are dependent on the duration of exposure. Initially, hypoxia-induced sympathoexcitation appears to be an adaptive response, primarily mediated by regulatory reflex mechanisms concerned with preserving sys

A narrative review of periodic breathing during sleep at high altitude: From acclimatizing lowlanders to adapted highlanders.

Periodic breathing during sleep at high altitude is almost universal among sojourners. Here, in the context of acclimatization and adaptation, we provide a contemporary review on periodic breathing at high altitude, and explore whether this is an adaptive or maladaptive process. The mechanism(s), prevalence and role of periodic breathing in acclimatized lowlanders at high altitude are contrasted with the available data from adapted indigenous populations (e.g. Andean and Tibetan highlanders). It is concluded that (1) periodic breathing persists with acclimatization in lowlanders and the severi

Persistent and symptomatic periodic breathing beyond the neonatal period in full-term infants: A case series.

INTRODUCTION: Periodic breathing (PB) is considered physiological in the neonatal period and usually disappears in the first months of life. There are few data available on persistent PB after the neonatal period. The objective of this study was to characterize infants born at term with persistent PB after the age of 1 month through polysomnography (PSG) performed during symptoms. METHODS: This retrospective case series included infants born at term between 2012 and 2021, without an underlying disease, who presented with symptoms of persistent PB during a PSG. Persistent PB was defined as mor
