In vivo vs in vitro response time of trancutaneous PO2 electrodes. A comparison of four devices in newborn infants.

Four devices for transcutaneous PO2 (tcPO2) monitoring have been applied simultaneously in 16 infants. Both during a maximal change in PaO2 and during physiological PO2 variations, the in vivo response time of the electrodes did not show the differences observed in vitro. We compared A, a prototype of the electrode by Huck, Lübbers and Huch (25 micrometer Telfon membrane) ; B, the commercial version of A by Hellige--Draeger (25 micrometer Telfon); C, the Radiometer TCM I oxygen monitor (25 micrometer polypropylene); and D, the Roche macrocathode electrode (6 micrometer Mylar), at 44 degree C.

Variability of the respiratory phase, respiratory pause, and numbers and durations of respirations during periodic breathing.

The analysis of cycles of periodic breathing (Cheyne-Stokes rhythms) in 35 patients showed wide variability of mean values of respiratory phases. The respiratory pauses are usually shorter and the scatter of the mean values is smaller. Only in four cases were respiratory pauses longer than respiratory phases. The duration of the respiratory phase becomes extended through the increase of number of single respirations. The latter also shows a very small scatter. The respiratory rate in 32 out of 35 patients was higher than normal and is to be considered as tachypnoea.

Intrauterine breathing movements and fetal presentation.

Fetal breathing movements in 14 twin pregnancies were examined with ultrasonic technique. The twin fetuses I in cephalic presentation showed a significantly higher percentage of apnea and periodic breathing compared with twin fetuses II. No such difference was found in twin pairs with Twin ti in breech presentation. Also in the control group of 27 singleton pregnancies, the fetuses in cephalic presentation had a higher incidence of apnea and periodic breathing than those in breech presentation. The position of the fetus seems to be decisive for the character of the breathing movements before b

Reduction of sleep apnea and bradycardia in preterm infants on oscillating water beds: a controlled polygraphic study.

The sleep and respiratory patterns of eight apneic preterm infants were polygraphically recorded for 24 hours. This polygraphic study was designed to test and extend our previous finding that gently oscillating water beds reduce apnea in premature infants. The infants who ranged in gestational age from 27 to 32 weeks and in birth weight from 1,077 to 1,650 gm served as their own controls, off and on the water bed. The 24-hour recordings were divided into four time blocks with the infant being placed on the water bed during alternate six-hour periods. Apnea was significantly reduced while the i

Hypersomnia with periodic breathing (an acromegalic Pickwickian).

The syndromes of Pickwickian, Ondine's curse, and primary alveolar hypoventilation are respiratory disorders manifesting increased sleepiness and irregular respiratory rhythms. These disorders are currently grouped as hypersomnia with periodic breathing (HPB). Polygraphic techniques have lead to a reasonable hypothesis as to the pathophysiology of the multiple variants of HPB. Discernible causes of HPB have been attributed to both central and peripheral factors. Peripheral factors encompass those conditions relating to upper airway obstruction. An acromegalic person suffering the HPB syndrome

Apnea, hypoxemia, and aborted sudden infant death syndrome.

To find out whether there is any relationship between the ventilatory response to hypoxia and the sudden infant death syndrome (SIDS), we studied the effects of mild induced hypoxia (PIO2, 120 mm Hg = 17% oxygen) in 16 infants aged 2 weeks to 6 months. Eight had recurrent apneic spells (apnea group) (five had aborted SIDS and three had recurrent apnea in the intensive care nursery) and eight were "well" preterm infants about to fly in a pressurized airplane (PIO2, 120 mm Hg) (control group). Mean birth weights were 2,245 and 1,400 gm and mean gestational ages were 35 and 30 weeks. Postconceptu

Analysis of periodic changes in ventilation in new-born infants.

1. Ventilation was recorded in thirty term infants during the first week after delivery using the trunk plethysmograph, during periods of rapid eye movement sleep (r.e.m.) and quiet sleep (n.r.e.m.). 2. Continuous histograms of tidal volume (VT) and instantaneous respiration rate (f) were samples at 0.5 sec intervals, passed through a digital filter to remove long-term trends in the data, and then subjected to frequency analysis. 3. In all the infants oscillations in VT and f were detected, with mean periods varying from 6.7 to 12.5 sec. The amplitude of these oscillations was higher during r.

Polygraphic study of periodic breathing and hypersomnolence in a patient with severe hypothyroidism.

A 67-year-old woman with primary hypothyroidism, who had Pickwickian syndrome as well, was studied electrophysiologically. A polygraphic study with simultaneous recordings of electroencephalography, actograms of the abdominal walls, the mentum and the anterior neck, and electromyogram of the intercostal muscles revealed a cyclic appearance of apnea in the sleep phase and gasping preceding arousal, which, together with macroglossia and sleep in the sitting position, suggested a cyclic obstruction of the upper airway. The obstruction appeared responsible for both alveolar hypoventilation and dis

Fluctuating intracranial hypertension due to Cheyne-Stokes respiration.

Cyclically fluctuating intracranial pressure (ICP) with periodic breathing was first described by Nils Lundberg in 1960. While Cheyne-Stokes respiration (CSR) frequently accompanies severe cerebrovascular accidents, it is not commonly appreciated that cycles of severe intracranial hypertension can complicate this abnormal ventilation pattern. We recently treated a patient with a hemorrhagic stroke in whom episodes of elevated ICP were synchronously associated with CSR.

[Periodic oscillating respiration outside of comatous stages].

A periodic oscillating breathing was observed in 11 subjects during study of their pulmonary function. All these patients were male and more than 50 years old. A cardiovascular disease was clinically evident in eight of them. Arterial hypoxemia was found in five subjects and a light hypocapnia in three. Analysis of oscillating rhythms first, revealed unexistence of ventilatory pauses between periods of deep breaths in some subjects and secondly, showed existence of permanent or discontinuous periodic rhythms. Breath by breath changes in ventilation were essentially induced by oscillations in t

Cardiovascular effects of apnoea in preterm infants.

Cardiovascular reflex responses have been studied in 9 newborn preterm infants with apnoeic episodes and in 2 preterm infants with periodic breathing. Respiration, blood pressure, heart rate, and peripheral blood flow were simultaneously recorded. Peripheral blood flow was measured in the leg by venous occlusion plethysmography. During apnoea, bradycardia and peripheral vasoconstriction occur. There is little change in blood pressure though pulse pressure increases. No cardiovascular changes were seen before the onset of apnoea. Periodic breathing had little effect on peripheral blood flow. Pr

[Distribution of respiratory arrests in preterm and term infants (author's transl)].

Incidence, distribution and periodicity of respiratory arrests larger than or equal to 2 seconds duration were studied in relation to age and sleep states by means of polygraphic recording in 61 tracing of 44 preterm and term infants free of pathologic conditions known to increase the risk of respiratory arrests. Respiratory arrests at any infant's age were found to be more frequent during active sleep. Incidence of respiratory arrests decreases with age in all sleep states. At any age the average duration of respiratory arrest is longer in quiet sleep. When restrictive definition is applied p

Hypersomnia and periodic breathing. Report of a case and review of the literature.

A patient with hypersomnia and periodic breathing has been studied. Both airflow obstruction and an abnormally of the respiratory control mechanism were implicated in the pathogenesis of the ventilatory arrhythmia. It is suggested that the older terms "Pickwick" syndrome and primary alveolar hypoventilation are abandoned for more descriptive terms, e.g. "hypersomnia with periodic breathing".

Polygraphic studies of normal infants during the first six months of life: III. Incidence of apnea and periodic breathing.

The incidence of apnea and periodic breathing was studied in full-term infants between birth and 6 months of age. Apnea was defined as a pause equal to or exceeding six seconds, periodic breathing as two cessations of breathing within a 20-second period, each equal to or longer than three seconds but less than six seconds. Sleep and cardiopulmonary variables were monitored. Apnea was common in the normal full-term infant. The incidence of apnea was highest in the newborn period and apneas exceeding 15 seconds were limited to this age. A reduction in apnea incidence occurred between birth and 3

Respiratory muscle EMG in newborns: a non-intrusive method.

A non-intrusive method of recording the EMG of respiratory muscles with electronic suppression of the ECG artifact and averaging with a running window was employed in newborn infants at term to study respiratory patterns in different behavioural states. There are clear state-related differences in the diphragmatic and intercostal activity patterns. During most of the time in state 1 (quiet sleep) sustained tonic activity is found in the diaphragm as well as in intercostal muscles, but is virtually absent during state 2 (active sleep). During state 1 intercostal activity slightly precedes diaph

[Pickwickian syndrome and hypersomnia with periodic respiration].

A classical definition of Pickwickian syndrome associates alveolar hypoventilation, obesity and hypersomnia with periodic breathing. Obesity in itself is enough to explain the alveolar hypoventilation and some of the sleep disorders, but in fact all three elements of this syndrome are intricated. Obesity, whether associated with a Pickwickian syndrome or not, affects ventilatory mechanics similarly. With the cause of sleep disorders are associated central ventilatory pauses, appearing periodically followed by phases of apnoea secondary to buccopharyngial hypotony. The alveolar hypoventilation

Response of ventilation and of intracranial pressure during rebreathing of carbon dioxide in patients with acute brain damage.

A study of the ventilatory response to rising CO2 in 43 patients with acute brain damage, using a rebreathing method, has revealed several instances of abnormally low responsiveness. The increased responsiveness previously reported in chronic brain damage was not observed in these patients by this method. The incidence of a very low response to CO2 tended to be associated with hyperventilation and brain-stem damage, but the numbers were insufficient to define a statistical relationship. There was no association between responsiveness to CO2 and either bilateral forebrain damage or periodic bre
