Psychosocial correlates of seizure frequency in Functional/Dissociative Seizures (FDS): A systematic review.

OBJECTIVES: Seizure frequency is commonly used as a primary outcome and proxy for severity in functional/dissociative seizures (FDS), despite uncertainties about its validity and reliability. This systematic review aimed to describe how FDS frequency has been measured and to identify its correlates. METHODS: Following pre-registration, PsycInfo, MEDLINE, CINAHL and Cochrane Reviews were searched using terms relating to FDS, seizure frequency and/or severity, and correlates. Eligible studies reported correlational data between FDS frequency and psychosocial variables in adults. Correlates were

Prolonging breath-hold duration in radiotherapy using cognitive engagement.

PURPOSE/OBJECTIVE: Deep inspiration breath-hold (DIBH) is commonly used in breast cancer radiotherapy to decrease cardiac radiation exposure. Using high-flow nasal oxygen and hyperventilation we achieved voluntary long deep inspiration breath-holds (L-DIBH) exceeding 3 min. Substantial variability in L-DIBH duration among subjects remains, not explained by physical factors. We investigated how cognitive engagement could impact the L-DIBH. MATERIALS/METHODS: Thirty-six subjects (20 volunteers and 16 patients) each performed 12 L-DIBHs over 4 sessions. During half of the sessions, they were ins

Intermittent hypoxia interventions for inducing ventilatory acclimatization to high altitude?

Accumulation of ∼200 h of hypoxia exposure (pre-acclimatization) has been suggested to prevent the development of acute mountain sickness when a person is subsequently exposed to high altitude. These pre-exposures are typically undertaken intermittently at or near the place of residence, primarily during sleep in hypoxic tents, and are likely to induce effective ventilatory acclimatization. However, the physiological consequences of different modes of intermittent hypoxia (IH) exposure on ventilatory acclimatization remain largely obscure and are the subject of much controversy. The aim of thi

Cardiorespiratory and Metabolic Adaptations to a 6-Week High-Intensity Interval Training Program at Different Intensities in Young Soccer Players.

This study compared the effects of high-intensity interval training (HIIT) prescribed at the respiratory compensation point (RCP) versus maximal aerobic speed (MAS) on cardiorespiratory and metabolic outcomes in young soccer players. Eighteen male national-level soccer players (15.7 ± .5 years) were randomly assigned to either the HIIT-RCP or HIIT-MAS group and completed a 6-week training program. The assessments included oxygen uptake (VO2) and running speed at the ventilatory anaerobic threshold and RCP, as well as relative buffering capacity, hypocapnic hyperventilation, blood lactate accum

Exogenous ketosis in breath-hold diving: Promise, paradox and physiological risk.

Exogenous ketosis induced by ketone ester (KE) ingestion has attracted growing interest as a potential strategy to enhance physiological tolerance to reduced O2 availability. Reported benefits arise largely from KE-induced hyperventilation, which can improve systemic and tissue oxygenation during altitude exposure and hypoxic exercise, potentially complemented by favourable effects on cellular bioenergetics. Whether these responses translate to breath-hold diving, however, remains unknown. Breath-hold performance is constrained by the integrated effects of progressive hypoxaemia, CO2 accumulat

Fenfluramine in Rett syndrome: A multidimensional clinical study.

OBJECTIVE: Rett syndrome (RTT) is a severe neurodevelopmental disorder frequently associated with drug-resistant epilepsy, autonomic dysfunction, respiratory abnormalities, sleep disturbances, and behavioral impairment. Fenfluramine has shown efficacy in developmental and epileptic encephalopathies and may provide broader therapeutic benefits through modulation of serotonergic and sigma-1 receptor pathways. METHODS: We conducted a prospective, open-label, single-center study to evaluate the efficacy, safety, and tolerability of adjunctive fenfluramine in patients with classic RTT carrying pat

Description of three dysfunctional breathing patterns in post-COVID dyspnea.

INTRODUCTION: Dysfunctional breathing (DB) can be defined as a change in breathing pattern associated with respiratory and/or systemic symptoms, after ruling out underlying respiratory or cardiac disease. Recent evidence suggests that DB contributes to dyspnea in post-COVID-19 syndrome (PCS), as demonstrated by ventilation analysis during cardiopulmonary exercise testing (CPET). Nevertheless, the lack of a standardized classification for the different subtypes of DB poses challenges for accurate diagnosis and effective management. We hypothesized that analyzing the evolution of breathing param

Dysfunctional respiration as a risk factor for Alzheimer disease: a hypothesis.

Cerebral hypoxia and ischemia are critical pathological factors in the etiology of Alzheimer disease. Common types of dysfunctional respiration, such as hyperventilation, oral breathing, thoracic breathing and hypoventilation have been shown to induce cerebral hypoxia and ischemia. Therefore, it is proposed that these respiratory abnormalities are risk factors in the pathogenesis of Alzheimer disease. Consequently, correcting these respiratory abnormalities could counteract the pathogenesis of Alzheimer disease in vulnerable populations. These therapeutic strategies might also be applicable to

MRI-based analysis of organ at risk stability during prolonged voluntary deep inspiration breath-hold in prone and supine breast cancer radiotherapy.

OBJECTIVES: Deep inspiration breath-hold (DIBH) is commonly used in breast cancer radiotherapy to reduce radiation exposure to the heart and lungs. Recent techniques incorporating oxygen supplementation and hyperventilation have enabled DIBHs exceeding 2.5 minutes. However, data on internal organ stability during prolonged breath-holds (L-DIBH) remain limited. METHODS: In this prospective observational physiological study, 10 healthy female volunteers performed 2 L-DIBHs (2 minutes 8 seconds each) in both prone and supine positions, following a validated hyperventilation and oxygenation proto

Cold shock response in healthy children: reassessment and first comparison between cold and warm water immersion.

INTRODUCTION: Swimming and diving are popular recreational activities and essential skills to prevent death from drowning. While most drownings occur in cold water, cold shock response is discussed as a major cause of drowning. Until now, the data on the physiology of drowning and cold shock response in children are scarce, while drowning remains a significant concern in this population. This study was conducted to investigate the cold shock response in healthy children and compare cold and warm water immersion. METHODS: Participants were first immersed up to the neck in warm water (34 °C, cl

Removal of cardiogenic oscillations during pressure support ventilation using sliding window singular spectrum analysis: proof-of-concept.

BACKGROUND: Cardiogenic oscillations in airflow can cause ventilator autotriggering during pressure support ventilation, potentially leading to inappropriate hyperventilation. A method to attenuate these oscillations in real time may help reduce autotriggering. MATERIALS AND METHODS: High-resolution airflow and ECG signals were collected from intubated surgical patients receiving pressure support ventilation. Singular spectrum analysis (SSA) was applied in a sliding-window format to generate a smoothed respiratory waveform. We quantified attenuation of cardiogenic oscillations using ECG-align

[A case where phenobarbital was remarkably effective against drug-induced liver injury presenting with prolonged intrahepatic cholestasis caused by Ashwagandha].

This case concerns a 39-year-old male with a history of adjustment disorder, and he had been using Ashwagandha-self-prescribed and self-imported online-for stress relief and vitality enhancement for the past 3 months. He visited the emergency department with symptoms of hyperventilation, and blood tests showed total bilirubin of 2.37mg/dL, ALT of 384U/L, and ALP of 92U/L, indicating hepatocellular-type liver dysfunction, thereby raising suspicion for drug-induced liver injury (DILI). Although ursodeoxycholic acid was initiated after referring to our hospital, his condition showed minimal impro

Complex network behavior in epileptic patients treated with Vagus Nerve Stimulation (VNS): VNS responders exhibit a unique pattern, different from VNS non-responders and healthy controls.

INTRODUCTION: Vagus nerve stimulation (VNS) represents an alternative treatment option in drug-resistant epilepsy. VNS patients can be categorized as responders (R, ≥50% seizure reduction) or non-responders (NR, < 50% seizure reduction). We demonstrate that VNS responders and VNS non-responders differ in their electrophysiological characteristics based on pre-implantation EEG analysis, specifically evaluated using relative mean power (RPW) and various information Entropy estimators computed in both he frequency and time domains. Based on the RPW and the Entropy estimators, we define and analyz

Non-invasive Coronary Functional Tests in Patients with Angina and Non-obstructive Coronary Artery Disease.

BACKGROUND: Invasive coronary functional tests (I-CFTs) can identify the mechanism(s) of angina in patients with non-obstructed coronary arteries (ANOCA). In this study, we assessed whether non-invasive coronary functional tests (NI-CFTs) can also produce reliable results when assessing these mechanisms. METHODS: We performed NI-CFTs by recording coronary blood flow velocity (CBFV) in the left anterior descending coronary artery by transthoracic Doppler echocardiography in 18 patients with ANOCA who had undergone I-CFTs (an acetylcholine provocation test and an adenosine stress test) and 13 h

Yield of Hyperventilation and Other Activation Procedures and Study Parameters in Facilitating Epileptiform Discharges.

PURPOSE: Hyperventilation (HV) during EEG was restricted during the COVID-19 pandemic. The authors compared EEGs with and without HV to place HV in context of other factors that precipitate epileptiform abnormalities (EA). The authors hypothesized HV would not significantly affect the sensitivity of routine EEG in children and adults. METHODS: The authors reviewed routine EEGs completed between March 2018 and March 2022 in subjects older than 4 years. The primary outcome variable was EA (seizures and epileptiform discharges). In a subset of patients with serial EEGs with and without HV, the a

Systemic effects of metabolic acidosis.

Metabolic acidosis is a primary reduction in serum bicarbonate concentration and a consequent decrease in blood pH, which has profound implications for systemic physiology. This literature review synthesizes the current evidence of metabolic acidosis's effect on cardiovascular, vascular, pulmonary, gastrointestinal, endocrine, musculoskeletal, renal, and metabolic systems. Cardiovascular effects include impaired cardiac contractility, altered ion exchange currents, and decreased β-adrenergic response. Vascular response is dependent on vessel size, promoting vasodilation in large arteries and v
