Inverse Vaccine Nanomimics: In situ Antigen-Specific Immune Tolerance Induction for Durable Cardiac Repair Post-Myocardial Infarction.

INTRODUCTION: Uptake of penicillin prophylaxis to prevent recurrent rheumatic fever and its sequela rheumatic heart disease (RHD) is not optimal in the Northern Territory of Australia. The Full Moon Strategy (the Strategy) was introduced in the Central Australian region in June 2006 to improve the uptake of prophylactic penicillin: clients and healthcare workers were encouraged to use the full moon as a cue for the timing of the 4 weekly prophylactic penicillin injection. OBJECTIVE: To determine the impact and effectiveness of the Strategy on knowledge and uptake of benzathine penicillin prop

Severe sleep-related hypoventilation in antibody-positive and antibody-negative autoimmune encephalitis: an emergency.

BACKGROUND AND OBJECTIVES: Although sleep disorders of neurological origin have been documented extensively in cases of autoimmune encephalitis, the impact and clinical significance of sleep disordered breathing remain underexplored. We aimed at determining the prevalence of sleep disordered breathing in autoimmune encephalitis. METHODS: A cohort study of consecutive patients with autoimmune encephalitis referred to a tertiary centre for sleep pathology was conducted over a period of 4.5 years. Patients underwent an overnight video-polysomnography with capnography. We investigated the presenc

Malarial pneumonopathy in Malawian children with cerebral malaria.

BACKGROUND: Respiratory distress is a well-recognized clinical presentation in pediatric malaria and is associated with increased mortality. Traditionally described as "acidotic breathing," this phenomenon has been attributed to compensatory hyperventilation secondary to metabolic acidosis. However, this explanation may not adequately capture causative mechanisms. We sought to quantify the prevalence of respiratory distress in children with cerebral malaria (CM) and to evaluate underlying pathophysiologic contributors. METHODS: In this prospective, observational study, 97 Malawian children ag

Fatal European subtype tick-borne encephalitis in a fully vaccinated immunocompetent child: a case report with viral sequencing.

BACKGROUND: Tick-borne encephalitis (TBE) is considered to have a more favourable outcome in children compared to adults, and death from TBE in children is infrequent. Severe cases are primarily seen in immunocompromised or unvaccinated individuals. We report a case of TBE with a fatal outcome in a previously healthy child. The case is unique in several aspects: an atypical clinical presentation without fever or biphasic illness, and a fulminant course despite full vaccination and absence of known predisposing risk factors for severe disease. In addition, sequencing revealed the TBE virus (TBE

Silent Progression of Adult-Onset SSPE: From Ocular Onset to Evolving EEG.

BackgroundSubacute sclerosing panencephalitis (SSPE) is a rare progressive encephalitis due to persistent measles infection. While classically a childhood disorder, atypical, adult-onset, and subclinical variants are increasingly reported. Ocular findings may precede neurological involvement by years. We aimed to underline EEG's role in tracking disease evolution from isolated ocular signs to neurological progression.CaseA 31-year-old woman presented with isolated ocular complaints and bilateral optic atrophy. Cerebrospinal fluid revealed measles IgG and IgG index positivity, confirming SSPE.

Antipathogen defense; b-caryophyllene; vector-virus mutualism Plants respond to insect herbivory by producing and releasing distinctive blends of volatile compounds. These herbivore-induced plant volatiles (HIPVs) serve several roles, including direct defense through toxic or repellent effects on herbivores and indirect defense by attracting the herbivores' natural enemies (Bleeker et al. 2009; Veyrat et al. 2016). HIPVs also act as warning signals, informing undamaged tissues within the same plant and nearby plants of a pending insect attack (Heil and Bueno 2007). HIPVs are mostly composed of

Expression of V‑domain Ig suppressor of T cell activation in ovarian cancer and its regulation of follicular regulatory T‑cell function via the AKT/MAPK pathway.

BACKGROUND: Patients with eating disorders (ED) often suffer from compulsive exercise behavior, which is associated with lower short-term response to treatment and poorer long-term outcome. Evidence-based interventions specifically targeting compulsive exercise behavior have been scarce so far. We developed a manualized group therapeutic approach integrating cognitive-behavioral therapy, exercise therapy and exposure with response management to promote healthy exercise behavior. Our objective was to examine the feasibility and acceptance of this new approach as add-on to regular inpatient trea

Iturin A disrupts Cryptococcus neoformans membrane integrity, attenuates virulence, and synergizes with amphotericin B.

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or

Physiotherapy Education and Breathing Retraining for Patients With Hyperventilation Syndrome: An Observational Study.

Purpose: The hyperventilation syndrome (HVS) is a dysfunctional breathing disorder with significant consequences on quality of life. HVS treatment involves an educational approach and breathing exercises. Although better understanding of HVS is needed, it is also relevant to provide evidence of HVS physiotherapy treatment efficiency. This study aimed to assess the effects of respiratory physiotherapy in HVS patients. Method: Patients referred by a physician to a physiotherapist for HVS-related symptoms were included in this prospective observational study. They received six respiratory physiot

Minimizing preparation time for repeated prolonged deep-inspiration breath holds during breast cancer irradiation using pre-oxygenation with high-flow nasal oxygen and voluntary hyperventilation.

OBJECTIVES: Deep inspiration breath-holds (DIBHs) reduce heart and lung toxicity during breast cancer radiotherapy. Consecutive DIBHs are stressful, time-consuming, and leads to position changes. To facilitate the introduction of pre-oxygenation using high-flow nasal oxygen (HFNO) and hyperventilation to prolong DIBHs (L-DIBHs), we examined the effect of hyperventilation time on the duration of L-DIBHs. Additionally, to minimize total treatment time, the feasibility of several successive L-DIBHs was examined. METHODS: Our previous protocol imposed 3 min of hyperventilation at 16 breaths per m

Approaches for difficult-to-induce-seizures electroconvulsive therapy cases (DEC): a Japanese expert consensus.

BACKGROUND: Seizure threshold increases with age and the frequency of electroconvulsive therapy (ECT). Therefore, therapeutic seizures can be difficult to induce, even at maximum stimulus charge with available ECT devices. Such cases are known as difficult-to-induce-seizures electroconvulsive therapy cases (DECs). However, no clinical guidelines exist for DECs; thus, clinicians often face difficulties determining treatment strategies. This study aimed to obtain a consensus among clinical experts regarding the treatment of DECs. METHODS: We asked Japanese ECT experts to rate 14 approaches unde

A scoping review protocol on brain PaCO2 levels at altitude.

BACKGROUND: Aeromedical transfer of patients with ischemic stroke to access hyperacute stroke treatment is becoming increasingly common. Little is known about how rapid changes of altitude and atmospheric pressure can impact cerebral perfusion and ischemic burden. In patients with ischemic stroke, there is a theoretical possibility that this physiologic response of hypoxia-driven hyperventilation at higher altitude can lead to a relative drop in PaCO2. This would ultimately result in cerebral vasoconstriction, and therefore worsening of the ischemic burden in patients with ischemic stroke. Cur

Toward a hyperventilation detection system in freediving: a proof of concept using force sensor technology.

BACKGROUND AND AIM: Hyperventilation before breath-hold diving (freediving) is widely accepted as a risk factor for hypoxic syncope or blackout (BO), but there is no practical way to address it before dives. This study explores the feasibility of using a force sensor to predict end-tidal carbon dioxide (P ETCO2) to assess hyperventilation in freedivers. METHODS AND RESULTS: Twenty-one freedivers volunteered to participate during two national competitions. The divers were instructed to breathe normally and perform three dry apneas of 1, 2, and 3-min duration at 2-min intervals in a sitting pos

Intraoperative Superficial Temporal Artery-middle Cerebral Artery Bypass Failure during Combined Bypass Surgery in Children with Moyamoya Disease.

Pediatric patients with moyamoya disease frequently show rapid progression with a high risk of stroke. Indirect revascularization is widely accepted as a surgical treatment for pediatric moyamoya disease, but it does not augment cerebral blood flow immediately, which leaves patients at risk for stroke peri-operatively. This delay in flow augmentation may make adding direct bypass the better option. This study documents our cases of direct bypass failure that underwent indirect bypass supplemented with superficial temporal artery-middle cerebral artery bypass to evaluate the adverse effects of
