Peculiarities of cardio-respiratory relationships in qualified athletes with different types of heart rhythm regulation according to respiratory maneuver data.

INTRODUCTION: Our goal was to determine the differences in changes in cardiovascular and cardiorespiratory interaction indicators during a respiratory maneuver with a change in breathing rate in athletes with different types of heart rate regulation. METHODS: The results of a study of 183 healthy men aged 21.2 ± 2.3 years, who were systematically involved in various sports, were analyzed. According to the results of the analysis of the HRV study during spontaneous breathing, the athletes were divided into 4 groups taking into account the type of heart rate regulation (HRR). Group 1 (with type

Vocal cord dysfunction/inducible laryngeal obstruction induced by hyperventilation in healthy individuals, people with asthma, and following coronavirus infection.

OBJECTIVE: Vocal cord dysfunction/inducible laryngeal obstruction (VCD/ILO) commonly co-exists with asthma and can start after viral infections. In this setting evidence suggests that dysfunctional breathing may induce the disorder but this possibility has not been researched. We therefore postulated that dysfunctional breathing can induce VCD/ILO, more so in people with asthma and after viral infections. METHODS: Eight healthy control subjects, 16 people with asthma and eight people who had recent COVID-19 infection (three with asthma) were recruited. Video-recorded laryngoscopy was performe

Does hyperventilation increase the diagnostic sensitivity of routine electroencephalography in adults with suspected epilepsy?

OBJECTIVE: We aimed to explore the role of hyperventilation (HV) during routine EEG recordings in the diagnosis of adults with epilepsy. METHODS: During the COVID-19 pandemic we removed HV from routine EEG recordings due to the transmission risks. Routine EEGs conducted in the year before and after the COVID-19 pandemic were re-evaluated. RESULTS: A total of 167 HV + and 199 HV - EEGs were analyzed. To ensure comparability between the two groups with respect to age and conditions that could contraindicate HV, patients ≥ 55 years and with any conditions contraindicating HV were excluded from

Retrospective Analysis of the Safety of High-Volume Dental Articaine Preparations for Japanese Patients.

We retrospectively analyzed the safety of the use of articaine, an amide-type local anesthetic, in Japanese dental patients (n=300) treated in Thailand in 2015-2017. The dosage, adverse events (AEs) caused by local anesthesia, and treatment efficacy were examined. Articaine, which is safe for patients with liver impairments due to its unique metabolism, has not been thoroughly tested in Japan for doses above 5.1 mL. Eighty of the present patients had undergone root canal treatment (RCT), 71 underwent tooth extraction, and 149 underwent implant-related surgery. More than three articaine cartrid

Evaluation of parameters extracted from tissue residue functions in dynamic susceptibility contrast MRI: Healthy volunteers examined during normal breathing and spontaneous hyperventilation.

Dynamic susceptibility contrast magnetic resonance imaging (DSC-MRI) is the most common MRI method in clinical environments for assessment of perfusion-related parameters. In this study, special emphasis was placed on the shape of the tissue residue function under different physiological conditions. DSC-MRI-based parameters assumed to reflect arterial delay and cerebral oxygen extraction were obtained by deconvolution of tissue and arterial contrast-agent concentration time curves. The established mean transit time (MTT) estimate was supplemented by biophysical modelling for extraction of the

Efficacy and safety of medroxyprogesterone acetate on noninvasive ventilation -treated exacerbated COPD patients: a double-blind randomized clinical trial.

BACKGROUND: In acute exacerbation periods of chronic obstructive pulmonary disease (COPD), patients may experience hypoxemia or hypercapnia. Noninvasive ventilation (NIV) and respiratory stimulant drugs are used to treat this condition. Medroxyprogesterone acetate (MPA) can cross the blood-brain barrier and cause breathing stimulation and hyperventilation. This study was conducted to investigate the effectiveness of MPA in hypercapnic exacerbated COPD patients and the possibility of faster weaning of patients from NIV. MATERIALS AND METHODS: This double-blind clinical trial was conducted on c

The Cross-Cultured Validity and Reliability of the Vietnamese Nijmegen Questionnaire.

INTRODUCTION: The Nijmegen Questionnaire assesses hyperventilation syndrome (HVS), but a validated Vietnamese version is lacking. This study investigates the cross-culture validity, structural validity, and reliability of the Vietnamese Nijmegen questionnaire (VNQ) for screening HVS in primary healthcare settings. METHODS: Following Beaton's guidelines, translation and adaptation involved two independent expert panels for content validity. Exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) examined the VNQ's structure. Reliability was assessed via Cronbach's alpha, intra

Effect of controlled hyperventilation on post-laparoscopic cholecystectomy shoulder pain: a prospective randomized controlled trial.

OBJECTIVE: This study investigated whether intraoperative controlled hyperventilation could reduce the incidence and severity of post-laparoscopic shoulder pain. METHODS: In this prospective, randomized, double-blind controlled trial, 150 patients undergoing elective laparoscopic cholecystectomy were randomly assigned to either controlled hyperventilation (n = 75) or conventional ventilation (n = 75) groups. The hyperventilation group received mechanical ventilation with a tidal volume of 10 mL/kg and respiratory rate adjusted to maintain end-tidal CO2 between 30 and 35 mmHg, while the contro

Impact of mild hypo- and hyperventilation on cerebral oxygen supply during general anesthesia.

OBJECTIVE: Cerebral blood flow autoregulation is affected by several physiologic and medical factors. Especially arterial carbon dioxide pressures (PaCO2) impact cerebral blood flow. Only extensive changes in end-tidal CO2 have been studied so far. The aim of this study was to evaluate the impact of mild hypo- and hyperventilation on cerebral blood flow as assessed by regional cerebral red blood cell oxygen saturation (rSO2) in two age groups. METHODS: Two groups of patients were compared under general anesthesia before the surgical procedure was started: A younger patient group (age < 40 yea

The effects of volitional hyperpnea on biomarkers of respiratory muscle damage in healthy young men.

High-intensity exercise hyperpnea places substantial demands upon the respiratory muscles, but whether this causes respiratory muscle damage is unknown. We investigated respiratory muscle damage following volitional hyperpnea (equivalent to 85% of participants maximum minute ventilation produced during a maximal incremental cycling test) using a skeletal muscle damage biomarker panel. Eight healthy men (33 + 2 years) underwent 10-min trials of volitional hyperpnea and rest (control) two weeks apart. Serum was collected before and at 1, 24, and 48 h after both volitional hyperpnea and control t

The great transatlantic race: physiologic and otoneurologic considerations.

BACKGROUND: In the era of the competition for the first transatlantic flight at the beginning of the nineteenth century, numerous accidents occurred. Whereas engine failure, bad weather and navigational problems are usually held responsible, the human factor has hardly been addressed. METHODS: In view of current insights, an inventory was made of possible physiologic and otoneurologic factors that could have contributed to failures and accidents. RESULTS: Extreme fatigue, hyperventilation with vestibular hyperreactivity and airsickness may have played roles. When flying in the dark and in cl

Effects of inspiratory muscle metaboreflex on cerebral circulation at rest and during light-intensity exercise in healthy males.

PURPOSE: This study aimed to clarify the effects of inspiratory muscle metaboreflex on cerebral circulation at rest and during exercise. METHODS: Twelve young males randomly completed two trials (rest and exercise [leg cycling at 40 % peak oxygen uptake] trials) on separate days. In each trial, the internal carotid artery (ICA), an index of cerebral circulation, was measured using Doppler ultrasound 2 min after inspiratory loading breathing (IL condition) or non-loading breathing (control condition). During ICA assessments, participants engaged in 3 min of spontaneous breathing (SB), followed

A novel human organoid model system reveals requirement of TCF4 for oligodendroglial differentiation.

Heterozygous mutations of TCF4 in humans cause Pitt-Hopkins syndrome, a neurodevelopmental disease associated with intellectual disability and brain malformations. Although most studies focus on the role of TCF4 in neural stem cells and neurons, we here set out to assess the implication of TCF4 for oligodendroglial differentiation. We discovered that both monoallelic and biallelic mutations in TCF4 result in a diminished capacity to differentiate human neural progenitor cells toward myelinating oligodendrocytes through the forced expression of the transcription factors SOX10, OLIG2, and NKX6.2

Pathophysiological mechanisms of exertional dyspnea in people with cardiopulmonary disease: Recent advances.

Physical activity is a leading trigger of dyspnea in chronic cardiopulmonary diseases. Recently, there has been a renewed interest in uncovering the mechanisms underlying this distressing symptom. We start by articulating a conceptual framework linking cardiorespiratory abnormalities with the central perception of undesirable respiratory sensations during exercise. We specifically emphasize that exertional dyspnea ultimately reflects an imbalance between (high) demand and (low) capacity. As such, the symptom arises in the presence of a heightened inspiratory neural drive - the will to breathe

The impact of hyperventilation on brain alpha activity: An EEG study.

Hyperventilation (HV) is a major physiological risk factor in environments like high altitudes or hypoxic conditions, causing a range of physiological changes that can potentially impair cognitive functions. As an important bridge connecting brain physiological states and cognitive functions, the variation of alpha activity under the effect of HV has not been fully explored. To this end, this work aims to reveal the changes in EEG alpha activity induced by HV in terms of power spectrum and functional connectivity (FC). EEG data were recorded from 305 healthy young male subjects when they were

Dysfunctional Breathing, in COPD: A Validation Study.

Background/Objectives: The Nijmegen Questionnaire (NQ) has been a prevalent screening tool for dysfunctional breathing for the past 40 years. Until recently, the validity of the NQ has been established for the general population with hyperventilation syndrome (HVS) and for individuals with asthma, but not for people with COPD. The aim of the study was to examine the validity and reliability of the NQ in individuals with COPD. Methods: Construct, convergent, divergent and discriminant validity as well as internal consistency reliability were examined in a sample of 84 people with stable COPD. R

Decreased CO2 saturation during circular breathwork supports emergence of altered states of consciousness.

Altered states of consciousness (ASCs), induced e.g. during psychedelic-assisted therapy, show potential to treat prevalent mental health disorders like depression and posttraumatic stress disorder. However, access to such treatments is restricted by legal, medical, and financial barriers. Circular breathwork may present a non-pharmacological and hence more accessible alternative to engage similar therapeutic processes. Scientific studies of breathwork are only just emerging and its physiological and psychological mechanisms are largely unknown. Here, we track physiological and experiential dy
