BACKGROUND: Breathing pattern disorders (BPDs) are becoming increasingly relevant due to the rise of respiratory illnesses but are often limited to diagnoses of hyperventilation syndrome and do not consider breathing dysfunction of mechanical origin. Impaired diaphragmatic and inefficient breathing patterns often evolve into downstream musculoskeletal and psychological consequences. Respiratory research has consistently called for the standardization of methods that can consider breathing as multifactorial and also consider breathing mechanics.
AIM: This paper aims to introduce the Breathing

Vertigo and dizziness are amongst the most frequent presenting symptoms in the emergency room, accounting for up to 4% of all emergency consultations. The broadness of their differential diagnosis and the often transient nature of these symptoms pose a significant challenge to the treating physician. Combining various subtle oculomotor signs at the bedside has been very successful in distinguishing peripheral from central causes in acutely dizzy patients meeting diagnostic criteria for the acute vestibular syndrome (i.e., acute and prolonged vertigo or dizziness accompanied by nausea or vomiti

BACKGROUND: Orthostatic intolerance, with or without postural orthostatic tachycardia syndrome (POTS), is collectively referred to as orthostatic intolerance dysautonomia syndromes (OIDS). This condition often presents with daytime hyperventilation, which is considered to be secondary to sympathetic hyperactivity. This hyperventilation appears to be a key characteristic in a newly described subset of patients with OIDS who also exhibit craniocervical instability, mast cell activation syndrome (MCAS), hypermobility spectrum disorder (HSD), and the phenomenon of alternating intracranial hyperten

Hyperventilation syndrome (HVS) is a condition that is seen in almost 10% of the general adult population. It presents with difficulty breathing, often in conjunction with anxiety, dizziness and light-headedness. We report a case involving a woman in her 30s, who presented to the emergency room with a primary complaint of shortness of breath and had a prolonged expiratory phase on examination. During a follow-up office visit, she had a generalised anxiety disorder-7 (GAD-7) score of 11, consistent with moderate anxiety. The diagnosis of HVS was made, and the patient began breathing retraining

In hypoxia, the initial response in vertebrates is hyperventilation, known as the Hypoxic Ventilatory Response (HVR), which is a physiological reflex that allows fish to maintain adequate oxygen uptake to meet metabolic demands. The severity of hypoxia in aquatic ecosystems is growing due to anthropogenic impacts. This is a concern with the recent evidence that metals can affect the ability of fishes to mount the HVR. As Rare Earth Elements (REEs), such as neodymium (Nd) increase in demand with the shift to a low-carbon, green economy, there is a critical need to understand their environmental

Cold temperatures (<-15°C) increase exercise-induced bronchoconstriction (EIB), while hypoxic-induced hyperventilation exacerbates respiratory muscle fatigue for a given exercising task. This study aimed to determine the individual and combined effects of cold and normobaric hypoxia on the respiratory system responses to high-intensity exercise. Fourteen trained male runners ( V ̇ O 2 max ${{\dot{V}}_{{{{\mathrm{O}}}_2}{\mathrm{max}}}}$ : 64 ± 5 mL/kg/min) randomly performed an incremental cardiopulmonary exercise test (CPET) to volitional exhaustion under four environmental conditions: no

Patients with diabetic ketoacidosis (DKA) are at risk for adverse peri-intubation events. In severe metabolic acidosis, respiratory compensation with hyperventilation may already be maximized. During intubation, aggressive apneic ventilation is required to mitigate the worsening acidemia and prevent cardiovascular collapse. Ventilator-assisted preoxygenation (VAPOX) is a method of preoxygenation and apneic ventilation in which the ventilator delivers pressure-supported minute ventilation during preoxygenation and after the onset of paralysis, until laryngoscopy is attempted. In patients with D

Background/Objectives: In a patient suspected of having epilepsy, routine EEG primarily contributes to the recording of interictal epileptiform discharges (IEDs). Similarly, magnetic resonance imaging (MRI) has become the gold standard imaging technique for identifying epileptogenic structural brain abnormalities. Various EEG and MRI tools to improve epilepsy diagnosis will be presented. Methods: When the initial EEG fails to record IEDs, various EEG measures that can improve EEG performance are presented; a comprehensive epilepsy-targeted MRI protocol to identify, localize, and characterize a

BACKGROUND: Veterans experience a high prevalence of traumatic brain injuries (TBIs) due to combat-related incidents such as explosive blasts, penetrating injuries, and blunt force trauma. These military TBIs are often more complex than civilian TBIs, leading to increased long-term neurological consequences. Over 414,000 service members have been diagnosed with TBI since 2000, highlighting the need to understand the implications of these injuries and the neurosurgical challenges in their treatment.
METHODS: This narrative review examines the literature on TBIs in veterans, focusing on the cha

Pitt-Hopkins Syndrome (PTHS) is a rare neurodevelopmental disorder caused by mutations in the TCF4 gene (18q21.2), encoding the transcription factor 4 (TCF4). This protein is critical for central nervous system development and neuronal maturation. Mutations in TCF4, which range from point mutations to large deletions, result in varying clinical severity, including intellectual disability (ID), motor impairments, and autistic features. Despite its rarity, PTHS has gained increasing attention due to advances in understanding the genetic and molecular mechanisms underlying TCF4 function. Recent r

Cardiopulmonary exercise testing (CPET) is emerging as a useful tool in the identification of dysfunctional breathing (DB). We aimed to evaluate the prevalence and functional impact of different patterns of DB in 628 adult patients referred for CPET due to unexplained dyspnoea (August 2019-December 2023). Patients were assigned to four groups following CPET interpretation: normal, breathing pattern disorder (BPD), hyperventilation (HV), and combined BPD with HV (BPDHV). Demographic and CPET performance data were analyzed using non-parametric tests as appropriate. 94 (15.0%) patients had normal

Ammonia can easily cross the blood-brain barrier in its non-ionized form, leading to significant accumulation in the brain during severe hyperammonemia. Excess ammonia is highly toxic to the brain and can cause neurological dysfunctions, such as tremors, ataxia, seizures, coma, and even death. Hyperammonemia is often the primary manifestation of urea cycle disorders (UCDs), a group of inherited metabolic disorders characterized by impaired nitrogen detoxification due to dysfunctions in urea cycle-related enzymes and transporters. Patients with complete deficiencies of urea cycle enzymes typica

INTRODUCTION: US warfighters operating aircraft are often exposed to cockpit temperatures exceeding 80 °F, introducing the potential risk of aircrew reaching dangerously high body temperatures. Hyperthermia induces significant decrements in physical and cognitive functions subsequently threatening the survivability of warfighters. However, the changes in body temperature and ensuing negative physiological effects are understudied, especially during combat training. Therefore, the purpose of the current study was to evaluate the temporal changes in body temperature and autonomic nervous system
