BACKGROUND: Little information is available about the relation between right ventricular infarction (RVI) and the presence of atrioventricular block. Thus, the objective of this study was to analyse a possible correlation between the conditions, which are frequently associated with inferior acute myocardial infarction (AMI).

We reviewed the clinical characteristics and outcome of cases of acute myocardial infarction occurring from January 1, 1985, through December 31, 1987, in the population of a long-term care institution for the elderly. The total number of patients in the series was 43. Comparisons were made between those patients transferred to a general acute-care hospital and those who remained at the facility. A 48-year old man was admitted with suspected acute myocardial infarction because of severe precordial pain and monophasic ST-elevations in the ECG. The patient's history of an ongoing infection, the localization, extent, and course of the ECG changes as well as the development of a pericardial effusion suggested viral perimyocarditis. Since the duration of precordial pain plays a key role in the indication for thrombolytic treatment in acute myocardial infarction, a retrospective study was made of the last 200 infarctions at Monthey Hospital with particular reference to the factors influencing this duration (distance, type of transport, prior intervention by a physician, loss of time in hospital). We report 65 cases (1970-1989). They were processing in a data base program for cardiology records. A number of selected variables, were analysed by un statistical package.
