The analgesic, hemodynamic and respiratory effects of buprenorphine (0.3 mg i.v.) were monitored in 15 coronary care unit-admitted patients presenting with myocardial infarction who were in functional class I according to the Killip classification. At the time of the study, 8 of them had unequivocal precordial pain (group 1); the remaining 7 were painfree (group 2). The agent showed a prompt and potent analgesic action. A 28 year-old Japanese male with Crohn's disease maintained on total parenteral nutrition for 5 consecutive years showed congestive cardiomyopathy. His symptoms (palpitation and precordial pain), arrhythmia on ECG and cardiomegaly on chest X-ray film were progressive. By intravenous selenium supplementation of 100 micrograms/day (selenite) his symptoms disappeared dramatically and plasma selenium level, erythrocyte selenium level and glutathione peroxidase activity improved to normal ranges. It is suggested that this myocardial damage was caused by selenium deficiency.

An analysis of 1,600 consecutive treadmill exercise tests is made, with emphasis on those with an ischaemic response. An ischaemic response is defined as a horizontal or downsloping depression of the ST segment of 1 mm or more. A hypertensive response is recorded where the blood pressure exceeds 200 mm Hg systolic or 100 mm Hg diastolic during exercise. From the district Haarlem-Noord, 558 patients suffering from an acute myocardial infarction (AMI) were admitted in the years 1982 through 1985. We report the vasoactive effects of ergonovine maleate in 66 patients with chest pain and abnormal angiographic studies. In 42 patients this medication was injected directly in the coronary arteries, and in 24 in the aorta. Erroneous positioning of central venous catheters in small tributaries of large central veins is a rare occurrence. We describe three such unusual incidents involving cannulation of the left internal mammary vein. Malposition was suspected when infusion of hypertonic parenteral nutrition led to persistent precordial pain. Unusual chest pain syndromes may indicate central venous catheter malposition and have not previously been described.

Chest pain due to esophageal spasm or esophagitis may mimic pain of variant angina. Differential diagnosis of the two diseases is often difficult and requires various tests, the value of which is discussed. These problems are illustrated by three cases. The esophageal investigation should preferably be preceded by coronary arteriography. Chest pain or dysphagia due to iced drinks is not specific for esophageal spasm but may be due to coronary spasm.

All visits at a primary health care centre in Sweden were studied during four weeks. The frequency of psychiatric symptoms or psychosocial problems noted by the doctors was recorded on a special form in addition to routine registration of diagnoses. Such problems were noted in 553 out of 3 205 visits, corresponding to 17.3%. Successful percutaneous transluminal coronary angioplasty (PTCA) was performed on a 37-year-old white man with an isolated 95% right coronary artery stenosis who initially presented with type II unstable angina. Anomalous origin of the coronary arteries from the main pulmonary artery is a rare congenital malformation. While this anomaly rarely produces cardiac symptoms, the more frequently encountered anomalous left coronary artery arising from the main pulmonary artery is poorly tolerated, the majority of patients dying in infancy. This report describes a symptomatic sixty-year old man with anomalous posterior descending coronary artery arising from the pulmonary trunk and details the angiographic findings.

A group of 43 consecutive patients with AMI were randomized to treatment with a novel platelet inhibitor, ticlopidine, or placebo in a double blind study. Three patients with infective endocarditis who presented with severe precordial pain and pericardial rubs are described in whom sterile pericardial and pleural effusions were found. The pericardial pain disappeared and the effusions regressed with successful antibiotic treatment. A similar mechanism to the postmyocardial infarction syndrome is suggested.
