A new semisynthetic 1-oxa-beta-lactam derivative, 6059-S, was evaluated for its safety and efficacy in children. Twenty-five patients were treated with 10 to 274 mg/kg per day of 6059-S by intravenous administrations. Rupture of the pericardium due to closed thoracic trauma is a very rare event which is hard to diagnose. It occurs in the form of two main clinical pictures: 1) severe chest trauma with massive haematoma requiring surgery and which permits diagnosis, and 2) precordial pain and clinical signs of pericardiac origin. Surgery is essential in view of the risk of lethal complications from cardia luxation. Operation consists of closure of the pericardiac lesion.

Radioimmunological determination of serum myoglobin in 115 patients is reported. A study is made of 525 consecutive treadmill exercise tests. Horizontal or downsloping ST segment depression of 1 mm or more was considered or ischaemic response. A systolic blood pressure exceeding 200 mm Hg or a diastolic blood pressure exceeding 100 mm Hg during or after exercise was defined as a hypertensive response. Sixteen patients with 31 aorto-coronary grafts have been studied by CT and by coronary arteriography to compare the results of the two methods in the evaluation of graft patency or occlusion. A study was performed to elucidate whether endogenous cortisol, as previously suggested, could be responsible for the decreased T3 levels seen in euthyroid patients with acute myocardial infarction. Over a period of four years, 23 patients had the diagnosis of chronic pulmonary hypertension made on the basis of elevated resting pulmonary arterial pressures above 30 mmHg mean. Two cases of 5-fluorouracil cardiotoxicity, resulting in one patient in myocardial infarction, are described. A review of the literature confirms that cardiotoxicity is a rare but genuine complication of 5-fluorouracil treatment; the cardiotoxic effect seems to range from mild angina without persistent electrocardiographic changes to severe myocardial infarction. A common syndrome of brief, sharp, severe precordial pain in 45 healthy young persons is described. The pain occurs at variable intervals, out of the blue. It typically occurs during rest or very mild activity and never on exertion. The onset is usually in adolescence. Most patients were of light or medium build, were nonsmokers and essentially healthy. It affects males and females. Recognition of the syndrome of precordial catch and reassurance of its benign nature will allay anxiety.
