The arrival of a patient with chest pain syndrome (precordial) to the emergency represents a diagnostic challenge for the physician. Around 6 million persons are seen each year at the Emergency units in the USA. More than half of the patients are admitted for their cardiac evaluation. Its cardiac origin is confirmed in 10 to 15%, and about 15% of them develop myocardial infarction. However, 5 to 10% of patients are dismissed and develop myocardial infarction during the next 48 h. We report a 43 years old female who developed an intense precordial pain and arterial hypotension. The patient was admitted to the emergency room in cardiogenic shock. An emergency angiography revealed a total occlusion of the left main coronary artery. An endoluminal coronary angioplasty with the placement of two stents was performed and coronary reperfusion TIMI III was achieved. The patient had a good evolution and one month later, a surgical revascularization was done, to avoid new occlusions. She was discharged in good conditions and in functional capacity I.

Myocarditis has only rarely been described as a manifestation of acute Q fever. Among our series of 1276 patients in whom acute Q fever was diagnosed during 1985--1999, myocarditis was diagnosed in 8.
