The effect of warm showers applied during the first stage of labor on labor pain, birth satisfaction, and neonatal outcomes: A randomized controlled trial.

The authors of this randomized controlled trial evaluated the effects of warm shower application during the first stage of labor on labor pain, birth satisfaction, and neonatal outcomes. Seventy-eight eligible women were randomly assigned to intervention or control groups, and 68 completed the study (34 per group). Women in the intervention group received a warm shower at 37 °C for at least 20 min at 4 cm 7 cm cervical dilation. Labor pain, measured by the Visual Analog Scale, was significantly lower in the intervention group at 10 and 20 min after both dilation points. No significant differen

Clinical Factors Associated with Postnatal Urinary Titin N-Fragment in Neonates.

BACKGROUND: Titin is a large sarcomeric protein (~ 3,800 kDa) essential for muscle function. The urinary N-terminal fragment of titin (N-titin) has emerged as a noninvasive biomarker for muscle injury in adults, but its clinical significance in neonates remains unclear. METHODS: A retrospective cohort study involving 523 neonates admitted to the NICU/GCU at Nihon University Itabashi Hospital between October 2021 and December 2023 was conducted. Urinary N-titin, collected within 24 hours of birth, was measured using enzyme-linked immunosorbent assay (ELISA) and normalized to creatinine (N-titi

Hemodynamically significant PDA impacts adverse outcomes in infants with BPD: a multicenter study.

BACKGROUND: Bronchopulmonary dysplasia (BPD) and significant hemodynamic patent ductus arteriosus (hsPDA) are both common and important clinical issues in extremely preterm infants. The potential impact on prognosis when these conditions coexist is a major focus of clinical concern. This study examined the relationship between hsPDA and adverse outcomes in BPD infants. METHODS: A retrospective analysis of 781 preterm infants (<32 weeks) from three hospitals (2018-2023). Based on echocardiographic assessment, infants were categorized as the non-PDA group, the non-hsPDA group, or the hsPDA grou

Multicystic Encephalomalacia in Early Infancy: A Neuroimaging Case Report.

Multicystic encephalomalacia (MCE) is an uncommon but severe consequence of perinatal hypoxic-ischemic injury in neonates. We report a 1.5-month-old male infant presenting with fever and focal seizures, with a prior history of birth asphyxia and prolonged neonatal intensive care unit stay. Neurosonography and magnetic resonance imaging revealed extensive bilateral cortical and subcortical cystic changes with cerebral atrophy consistent with MCE. This case highlights the importance of early neuroimaging to identify characteristic patterns of extensive cystic degeneration, enabling timely progno

Pediatric near-drownings: clinical insights and prediction of life-threatening events.

INTRODUCTION: Drowning is a leading cause of mortality in children worldwide. Near-drowning is a state of survival after asphyxia due to submersion in water. Drowning-related incidents have significantly increased in developed countries. Even survivors suffer from various long-lasting morbidities. Early prediction of mortality and morbidly life-threatening events is life-saving and guides decision-making in such critical situations. This study aimed to identify early predictors of mortality and other unfavorable outcomes (hypoxic brain injury and respiratory failure necessitating mechanical ve

Evaluation of hanging deaths in childhood: An autopsy study.

INTRODUCTION: Hanging is defined as a form of asphyxia caused by the suspension of the body with a ligature around the neck, where the constriction is generated by the body's own weight. The aim of this study is to examine autopsy findings in hanging cases among children, a rare cause of death in this age group, using data collected from a forensic autopsy center. MATERIALS AND METHODS: This retrospective study was conducted by reviewing electronic autopsy reports of pediatric cases referred to the Morgue Department between January 1, 2012, and December 31, 2021, for the determination of the

Introduction of central fetal monitoring and impact on neonatal outcome - a retrospective observational cohort study.

INTRODUCTION: Cardiotocography (CTG) is used for fetal surveillance to prevent asphyxia related neonatal outcomes during pregnancy and delivery. In Sweden, all delivery wards have transitioned from CTG registered and printed on paper inside the delivery room, to digital central fetal monitoring where CTG is visible both in the delivery room and on distance for multiple observers. We aimed to evaluate whether the transition from CTG on paper to central fetal monitoring has affected neonatal morbidity. MATERIAL AND METHODS: The study was conducted as a retrospective cohort study. Data from 8577

Unraveling Riboflavin-Mediated Mitochondrial Modulation as a Therapeutic Pathway in Neurological Disorders: An Integrative Systematic Review.

Mitochondrial dysfunction is recognized as a key pathophysiological mechanism in neurodegenerative diseases. Alterations in mitochondrial dynamics-including imbalances in fission and fusion, impaired biogenesis, and disrupted mitophagy-contribute to the onset and progression of neurological disorders. In this context, mitochondrial modulation has emerged as a promising therapeutic strategy. This systematic review examined the role of riboflavin, a water-soluble vitamin and essential mitochondrial cofactor, in neurological interventions through mitochondrial modulation, with emphasis on elucida

Emergency care for avalanche buried patients - a narrative review.

Avalanches claim the lives of around 160 winter recreationists and workers in hazardous snow-covered mountain regions worldwide. These fatalities result from asphyxia, trauma, and hypothermia. Survival for critically buried subjects relies on the speed of extrication, absence of trauma, presence of a patent airway and air pocket, immediate field treatment, and rapid transport to the most appropriate hospital by organized search and rescue (SAR) and emergency medical services (EMS). Survival may be improved with helicopter EMS, advancements in safety and rescue equipment technology, early deplo

Death by hanging: possibilities and limits of medico-legal evidence.

In forensic medicine, hanging is a frequent cause of non-natural death which may pose diagnostic problems on the investigator due to the manifold and sometimes ambiguous autopsy findings. The great variability of appearances potentially impedes the classification as to the manner of death (suicide, accident, homicide). The significance of some signs formerly considered to prove vital suspension could not always be confirmed in more recent studies. Therefore, traditional conception passed on from generation to generation must be critically questioned and, if necessary, replaced by evidence-base

Tracheal mucosal necrosis and sloughing after accidental endotracheal extubation in a child: a case report and literature review.

INTRODUCTION: Airway obstruction caused by the detachment of obstructing fibrinous tracheal pseudomembrane (OFTP) secondary to airway mucosal injury from endotracheal intubation is extremely rare in clinical practice. Due to the non-specific clinical manifestations and low incidence of this disease, most clinicians tend to overlook it, lack vigilance, and find it difficult to make a definite diagnosis immediately when symptoms appear. After the tracheal mucosa is injured and detached, it can form a valvular obstruction that blocks airway ventilation, leading to obvious dyspnea, acute respirato

Analysis of First and Second Sleep Environments in Unexplained Sudden Deaths in Infants Based on Scene Reenactment Data.

The American Academy of Pediatrics (AAP) recommends that a safe sleep environment consist of an infant lying supine on a separate, uncluttered, approved sleep surface. Infant awakenings may lead to moving the infant from the initial sleep environment to a second one. The Connecticut Office of the Chief Medical Examiner electronic database was searched from 2018 through 2022 for infant deaths with non-homicidal manner in which scene reenactments were performed; 87 such deaths were identified. The most common causes of death were unsafe sleep environments (64.4%) and asphyxia (21.8%). Of the tot
