Causes of and risk factors for asphyxia neonatorum in sub-Saharan Africa: a study protocol for systematic review and meta-analysis.

INTRODUCTION: Asphyxia neonatorum (AN) is a leading cause of neonatal morbidity and mortality worldwide, accounting for nearly one quarter of the 2.3 million neonatal deaths reported in 2023. The burden is disproportionately high in sub-Saharan Africa (SSA), where neonatal mortality rates linked to AN exceed global averages. Despite numerous studies, evidence on the underlying causes and risk factors of AN in SSA remains fragmented and inconsistently reported. A clear distinction between causes and risk factors is essential to inform prevention, diagnosis and management strategies for AN in th

Factors associated with thyrohyoid injuries in hanging cases: An autopsy study in North Africa.

BACKGROUND: Hanging is the most frequent method of suicide in Tunisia. Its medico-legal assessment requires an integrated approach combining the circumstances of death, scene investigation, and complete postmortem examination to establish the cause and manner of death and assess possible third-party involvement. Among the internal findings, injuries of the hyoid-laryngeal complex are of particular medico-legal interest, although the factors associated with their occurrence remain incompletely understood. AIM: This study aimed to identify factors associated with thyrohyoid fractures in hanging

Diagnosis-Day Prothrombin Time and Transaminase as Exploratory Risk-Stratification Indicators for Severe Neonatal Outcomes in Acute Fatty Liver of Pregnancy.

OBJECTIVES: Acute fatty liver of pregnancy (AFLP) is a life-threatening condition associated with high neonatal morbidity. This study aimed to develop a minimal, clinically feasible prediction model using diagnosis-day laboratory indicators to stratify the risk of severe adverse neonatal outcomes (SANO). METHODS: In this single-center retrospective cohort (2004-2021), 18 pregnancies with AFLP confirmed by Swansea criteria were analyzed. Firth penalized logistic regression assessed associations between the first laboratory values on the diagnosis day and SANO (intrauterine death/severe asphyxi

A critical review of autopsy findings in cases of suicidal hanging and the effect of a layered neck dissection.

Suicidal hanging is a common case type referred to death investigation services throughout the world. It is associated with a large number of autopsy findings, but the presentation can vary significantly between cases, based on a multitude of factors related to the decedent, ligature and degree of suspension. There is also considerable variation in the reported incidence of autopsy findings within the published literature, some of which may be explained by different anthropometric characteristics of the included cases. However, study design and the thoroughness of the autopsy examination under

Autophagy-lysosomal related protein, TFEB, may serve as a biomarker of mechanical asphyxia: A primary study.

The postmortem diagnosis of death from mechanical asphyxia (DMA) remains challenging owing to the absence of specific morphological signs. The present study evaluated the potential of transcription factor EB (TFEB), a master regulator of the autophagy-lysosomal pathway (ALP), as a biomarker for MA. Through transcriptomic analysis, molecular docking, molecular dynamics (MD) simulations, and an SH-SY5Y cell hypoxia model, we observed that TFEB expression was upregulated and nuclear translocation was induced during the initial phase of hypoxia. Following prolonged hypoxia (24 h), however, autopha

Validation of the discriminatory performance of the iPREFACE score for clinically significant fetal acidemia: an outcome-enriched nested case-control study.

BACKGROUND: Cardiotocography is widely used for monitoring fetal well-being during labor; however, its clinical utility remains limited due to a high false-positive rate, particularly in predicting adverse neonatal outcomes such as cerebral palsy. Despite its implementation, the incidence of cerebral palsy has not decreased, whereas cesarean section rates have increased. Severe fetal acidemia has been shown to be associated with neonatal brain injury. The iPREFACE score was developed to address this limitation by evaluating the accumulation of fetal stress due to uterine contractions based on

Selective vulnerability of the spinal cord in a rat model of asphyxia-induced cardiac arrest.

Spinal cord injury after cardiac arrest (CA) can cause devastating neurological deficits, including paraplegia. Although lumbar spinal cord injury is clinically important, regional comparisons remain limited. In this study, we examined region-associated vulnerability of the thoracic and lumbar spinal cord using a rat model of asphyxial CA (ACA). Adult male Sprague-Dawley rats underwent sham surgery or ACA, and thoracic and lumbar spinal cord tissues were collected 24 h after return of spontaneous circulation (ROSC) from ACA survivors with hindlimb paralysis. Histopathological analysis demonstr

Clinical practice guidelines for neonatal therapeutic hypothermia across European Society for Paediatric and Neonatal Intensive Care (ESPNIC) member countries: a multidisciplinary blind comparison.

BACKGROUND: Therapeutic hypothermia (TH) decreases mortality and neuro-developmental disability in neonates greater or equal than 36 weeks' gestation with moderate-to-severe hypoxic-ischaemic encephalopathy (HIE) due to perinatal asphyxia. TH is often applied in different ways, as national guidelines and target populations may significantly differ. We aimed to compare TH guidelines from various European countries as we hypothesised that guidelines are significantly different and often of insufficient quality. METHODS: Multidisciplinary, blind, critical appraisal of clinical practice guideline

Child sexual homicide and sexual sadism: Autopsy analysis of live encasement followed by survival.

We report the case of an 8-year-old girl who, having been reported missing, was found while concealed in a bed storage compartment in her neighbour's house. Despite medical intervention she died 13 hours later. Autopsy revealed sexual and physical assault with multiple incised and blunt force injuries, signs of possible restraint, torture and evidence of concealed trauma. Our report discusses the forensic interpretation of findings and the contributory roles of haemorrhagic shock and positional asphyxia. It demonstrates the pivotal function of forensic autopsy in unravelling what occurred in s

Health interventions and stakeholder roles in neonatal mortality reduction within low-income countries: A scoping review.

OBJECTIVES: Neonatal mortality continues to represent a major global health challenge, with the highest mortality rates being observed in low-income countries (LICs). Despite the adoption of Sustainable Development Goal (SDG) 3.2, there has been limited progress in reducing deaths due to preterm birth complications, birth asphyxia, and infections among newborns. In order to inform future policy and implementation strategies, it is essential to understand which neonatal health interventions have been implemented since 2015 and which stakeholders have been involved in their implementation and fu

Burking as a cause of homicidal asphyxia: A forensic case report.

Burking a rare and distinct form of homicidal asphyxia is when the perpetrator(s) obstruct all the external respiratory orifices while simultaneously applying thoracic compression. It is historically associated with the 19th-century serial killers Burke and Hare. It is a complex form of 'complicated asphyxia' which can present significant diagnostic challenges due to its potential for subtle or overlapping postmortem findings. This report describes the autopsy and forensic investigation of a 35-year-old male victim of a suspected homicide. External examination revealed periorbital ecchymosis,

Mesenchymal Stem Cell (MSC)-based therapies for neonatal lung and brain injury - one size fits all?

MSC-based therapies have emerged as a highly promising approach for two of the most devastating consequences of perinatal insults such as extreme prematurity and perinatal asphyxia: bronchopulmonary dysplasia (BPD) and neonatal brain injury. Both conditions share a common pathophysiological basis of inflammation, oxidative stress, and disrupted progenitor cell function in critical windows of organ development. Exogenous MSC act primarily through their secretome, releasing extracellular vesicles, growth factors, and immunomodulatory mediators that attenuate injury and support organ development.

Prevalence and risk factors for failed newborn hearing screening among preterm infants: a prospective cohort study in Nairobi, Kenya.

BACKGROUND: Despite the high burden of global childhood hearing loss, compliance with universal newborn hearing screening (UNHS) guidelines is low in many low- and middle-income countries, including Kenya. Preterm infants are especially prone to an increased risk of hearing loss due to the associated perinatal and neonatal morbidities. The objectives of this study were to investigate the prevalence and risk factors of failed hearing screening in preterm newborns. METHODS: We conducted a hospital-based prospective cohort study of preterm infants at the Kenyatta National Hospital (KNH) and the

Trends and disparities in infant mortality from intrauterine hypoxia and birth asphyxia in the United States, 1999-2023.

INTRODUCTION: Despite continued advances in prenatal and neonatal care, birth asphyxia and intrauterine hypoxia remain significant causes of preventable mortality in the early neonatal period. Fetal, maternal, and health-system factors influence outcomes immediately after birth. This study aims to analyze trends in infant mortality related to these conditions and examine differences across demographic groups. METHODS: Data on infant mortality related to intrauterine hypoxia or birth asphyxia were obtained from the Centers for Disease Control and Prevention Wide-ranging Online Data for Epidemi

Neonatal Health Outcomes Before and After Introduction of a Midwifery-Led Care Model in Embu County, Kenya: A Randomized Controlled Trial.

INTRODUCTION: Neonatal health is a global concern, particularly in Sub-Saharan Africa where the neonatal mortality ratio exceeds Sustainable Development Goals targets. To address this, midwifery-led care models have been proposed as safe and cost-effective solutions. However, their use and impact in Kenya remain under-researched. This study aimed to evaluate the effects of one of these models on neonatal health outcomes. METHODS: Two hospitals, Siakago and Runyenjes, were chosen for a Randomized Controlled Trial involving 164 mothers and 60 midwives. Data were collected using questionnaires,
