Sudden death secondary to compression of the upper airway by esophageal distension.

Achalasia is a motor disorder of the esophagus, with an estimated prevalence of 1 in 10,000 people worldwide. Clinically, it is expressed by progressive dysphagia for solids and liquids, regurgitation of undigested food, and chest pain. In very rare cases, achalasia could become complicated by severe esophageal distension. We report the case of a 45-year-old man who experienced a sudden loss of consciousness during a meal. Endotracheal intubation was impossible due to a tracheal obstruction of unknown origin. Autopsy revealed major distension of the esophagus (maximum diameter of 18 cm), fille

"I Knew I was Going to Die …": The Experiences of Non-Consensual Strangulation Amongst Australian Men Who Have Sex with Men.

Strangulation is often used in cases of intimate partner violence, sexual assault, and homicide. Strangulation is a particularly intimate form of asphyxia, involving manual or ligature-based external pressure applied to the front and sides of the neck. This study examines non-fatal non-consensual circumstances of strangulation experienced by 19 Australian men who have sex with men. Although their experiences were diverse, interpretative phenomenological analysis identified four prominent experiential themes. These themes included (1) Strangulation: A method of coercive control, (2) Blurry line

Protective effects of enteral acetate supplementation against brain injury in a rat model of necrotizing enterocolitis.

BACKGROUND: Necrotizing enterocolitis (NEC) is a severe gastrointestinal disease in preterm infants, often leading to brain injury. The microbiota-gut-brain axis (MGBA) plays a key role, with short-chain fatty acids (SCFAs) emerging as potential therapeutic agents. This study explores the impact of SCFAs, particularly acetate, in mitigating NEC-related brain injury. METHODS: A NEC rat model was established via overfeeding, hypoxia, and asphyxia. Intestinal injury, SCFA levels, systemic inflammation, and neuroinflammation were assessed through histology, gas chromatography-mass spectrometry, E

Infant Breathing and Bedding Characteristics: Objective Tests for Evaluating Their Relationship.

Bedding characteristics have been associated with an increased risk of respiratory impairment and sudden infant death. However, the relevant bedding properties and the mechanisms influencing newborn respiration remain poorly understood. Existing evidence is fragmented across epidemiological, clinical, forensic, and regulatory studies, with limited integration of respiratory physiology and engineering-based measurement frameworks. Consequently, current safety guidelines rely largely on qualitative criteria and simplified mechanical tests that may not adequately capture physiologically relevant

Risk factors for necrotizing enterocolitis in preterm infants: a 1:1 matched case-control study.

IMPACT: This study demonstrates that neonatal sepsis, blood transfusion, combined antibiotic use, asphyxia, and maternal gestational diabetes (GDM) are independent risk factors for NEC. By utilizing a 1:1 matched case-control design, we effectively eliminated the confounding effects of gestational age and birth weight, revealing critical perinatal drivers of the disease. The resulting prediction model (AUC = 0.937) provides a robust tool for early screening. These findings underscore the urgent need for enhanced antibiotic stewardship and rigorous maternal GDM management to reduce NEC incidenc

Atherogenic index of plasma predicts gestational diabetes mellitus and correlates with disease severity and adverse pregnancy outcomes.

BACKGROUND: Early identification of gestational diabetes mellitus (GDM) is essential for timely intervention. The atherogenic index of plasma (AIP) may reflect early lipid dysregulation preceding GDM onset. METHODS: This retrospective cohort included 660 pregnant women who underwent first-trimester (11-13⁺⁶ weeks) lipid testing and GDM screening at 24-28 weeks based on IADPSG criteria. Demographic and biochemical data were extracted from medical records. Logistic regression identified independent predictors of GDM, and receiver operating characteristic (ROC) analysis evaluated predictive perf

MicroRNAs and genes regulating responses to hypoxia and inflammation expression levels in blood leukocytes as potential biomarkers of initial oxygen deficiency tolerance.

INTRODUCTION: Hypoxia-inducible factors (HIFs) play central roles in evoking responses to hypoxia, and their activities are regulated by numerous molecules, including microRNAs. Organisms typically show differing tolerances to oxygen deficiency; the most common method of determining such tolerance involves examining the effects of sublethal hypoxic exposure (SHE) in a decompression chamber, which can lead to pathological changes in the internal organs. The aim of the present study was to investigate the microRNAs and genes regulating cellular responses to hypoxia and inflammation expression le

Vasoactive support in asphyxiated neonates: Evidence, uncertainty, and clinical practice.

Perinatal asphyxia is a critical condition frequently associated with hemodynamic instability, often necessitating vasoactive support. Despite the established benefits of therapeutic hypothermia, significant mortality and long-term morbidity persist, particularly in severe cases. The objective of this narrative review is to critically evaluate the available evidence on vasoactive therapies in asphyxiated neonates, compare the hemodynamic effects of commonly used agents, and propose a physiology-based, phenotype-driven approach to cardiovascular management during therapeutic hypothermia. Benefits of neonatal resuscitation training in high- and low-resource settings.

Globally, around 1.9 million stillbirths occur each year, and around 2.3 million newborns die in the first 28 days after birth each year. Many of these deaths are preventable. The burden of stillbirths is likely even higher, as many are unreported. A disproportionate number of fresh stillbirths and neonatal deaths occur in low-resource settings. About 1 million neonatal deaths occur in the first 24 h after birth, with preterm birth and birth asphyxia/intrapartum-related death being the leading causes. For newborns who have difficulty establishing effective breathing at birth, prompt and effect
