A Case of Refractory Nephrotic Syndrome Complicated With Tuberculous Pleurisy and Literature Review.

Nephrotic syndrome(NS) can be classified into steroid-sensitive nephrotic syndrome (SSNS) and steroid-resistant nephrotic syndrome (SRNS). Rifampicin is potentially leading to treatment resistance in NS. Here we report a case of SRNS secondary to rifampicin-induced drug interactions during antituberculosis therapy.

Serositis as an indicator of poor prognosis in pediatric systemic lupus erythematosus.

BACKGROUND: Systemic lupus erythematosus (SLE) is a multi-systemic autoimmune disease that causes inflammation of the serosa (serositis). This retrospective study aimed to evaluate the clinical characteristics of serositis in childhood-onset SLE (cSLE) and analyze its association with long-term outcomes. METHODS: We retrospectively reviewed the medical records of patients with cSLE diagnosed at a medical center in Taiwan, analyzing data collected from January 2002 to December 2022. We analyzed the clinical features of patients with serositis as pleuritis and/or pericarditis with at least a sm

Immunomodulatory Effects of Molecular Hydrogen Therapy in Systemic Lupus Erythematosus With Pericarditis and Pleuritis: A Case Report.

BACKGROUND/AIM: Pericarditis is a common cardiovascular complication associated with systemic lupus erythematosus (SLE). Oxidative stress plays a significant role in disease pathology. Molecular hydrogen has emerged as a promising therapeutic option with its antioxidant, anti-inflammatory, and antiapoptotic properties. While prior studies have demonstrated its cardioprotective effects, research on its immunomodulatory potential in autoimmune diseases remains limited; therefore, this article presents a case study with SLE experiencing pericarditis and pleuritis, receiving hydrogen therapy in Oc

A Randomized, Placebo-Controlled Trial of Hydroxychloroquine in Incomplete Lupus.

OBJECTIVE: Patients with features of systemic lupus erythematosus (SLE) who do not fulfill classification criteria can be designated as incomplete lupus erythematosus (ILE). This condition includes individuals with a high risk of progression to SLE. Treatment of ILE may reduce symptoms, severity, and incidence of SLE. METHODS: Hydroxychloroquine (HCQ) was chosen as an ILE intervention for a randomized, double-blind trial to determine whether the rate of accumulation of SLE features defined by the 2012 Systemic Lupus Erythematosus International Collaborating Clinics (SLICC) criteria could be r

Pulmonary thromboembolism as an atypical manifestation of mixed connective tissue disease in a young man: A case report.

Mixed connective tissue disease is an autoimmune disorder characterized by the presence of various abnormal autoantibodies and systemic involvement. Similar to systemic lupus erythematosus, pulmonary manifestations commonly include interstitial fibrosis, pulmonary hypertension, and pleurisy; however, pulmonary thromboembolism has rarely been reported in mixed connective tissue disease. We report a case of mixed connective tissue disease in which pulmonary thromboembolism was the initial manifestation, as diagnosed by computed tomography angiography. Following glucocorticoid and anticoagulant t

Pleuritic Chest Pain as an Initial Presentation of Pyogenic Spondylitis.

Pyogenic spondylitis typically presents with back and neck pain, fever, and fatigue. Sharp chest pain upon deep breathing, commonly associated with pleurisy, is not a typical manifestation of pyogenic spondylitis. We report a case of pleuritic chest pain as the initial presentation of pyogenic spondylitis. A 62-year-old male patient presented with a one-week history of right-sided pleuritic chest pain and fever. Chest computed tomography (CT) revealed a right pleural effusion. The patient was initially treated for bacterial pleurisy without improvement and subsequently developed back pain. Blo

New perspective on the clinical and laboratory characteristics of rheumatoid pleural effusion: A 29-case series.

OBJECTIVE: Rheumatoid pleural effusion (RPE) usually occurs in middle-aged men. Pleural fluid analyses have revealed high lactate dehydrogenase (LDH) levels and low pH and glucose levels in RPE. We aimed to investigate the clinical and laboratory features of patients with RPE since the beginning of the 21st century. METHODS: Medical records of patients with RPE were reviewed between May 2006 and October 2021. The patients were divided into <60 year (younger) and ≥60 year (older) groups. RESULTS: The younger group comprised 6 patients (median age 53.5 years, female 33%) and older group compri

Refractory Pleuritis in a Patient with Silicosis, Systemic Sclerosis, and Sjögren's Syndrome: Considering the Potential Role of Adjuvant-Induced Autoimmunity.

BACKGROUND Silicosis, which is caused by the inhalation of crystalline silica, is known to be associated with a variety of autoimmune diseases. Recently, a new pathogenesis called autoimmune/inflammatory syndrome induced by adjuvants (ASIA) has been reported, which occurs after exposure to substances with adjuvant activity, including silica, and shares clinical features observed in several autoimmune diseases. CASE REPORT A 65-year-old man with silicosis was admitted to our hospital due to fever and chronic fatigue. Symptoms such as sicca and Raynaud's phenomenon and pleural effusion appeared

Comparative diagnostic efficacy of ultrasound-guided, medical thoracoscopic, and blind pleural biopsy in tuberculous pleurisy.

BACKGROUND: Pleural biopsy (PB) procedures to diagnose tuberculous pleurisy may be blind, ultrasound-guided, or medical thoracoscopic (BPB, USPB, and MTPB, respectively). However, the diagnostic efficacy of the three methods has not been comprehensively and systematically elaborated locally or abroad. This retrospective study compared the diagnostic efficacy and safety of BPB, USPB, and MTPB. METHODS: A retrospective analysis included 333 patients (BPB =84, USPB =97, MTPB =152). Propensity score matching (PSM) generated 60, 77, and 78 matched pairs (USPB vs. BPB, USPB vs. MTPB, BPB vs. MTPB).

Performance of metagenomic next-generation sequencing, Xpert MTB/RIF and acid-fast staining for diagnosing tuberculous pleurisy and empyema.

BACKGROUND: The detection of Mycobacterium tuberculosis (MTB) is an important basis for the diagnosis of tuberculosis. Detecting fresh tissue, pus, and other samples is challenging. Both metagenomic next-generation sequencing (mNGS) and Xpert MTB/RIF have demonstrated excellent performance in the diagnosis of tuberculosis; however, their research base is still lacking in tissue or pus samples. We hope to explore the detection performance of mNGS and Xpert MTB/RIF in these sample types through this study. METHODS: This study enrolled 154 patients suspected of having tuberculosis. Fresh tissues

IgG4-related pleurisy with suspected asbestos involvement.

A man in his 60s with a history of asbestos exposure had been followed for progressive right-sided pleural thickening over several years. Imaging findings on high-resolution CT and 18F-fluorodeoxyglucose positron emission tomography/CT strongly suggested malignant pleural mesothelioma. However, both CT-guided pleural biopsy and endobronchial ultrasound-guided transbronchial needle aspiration of the mediastinal lymph nodes showed no evidence of malignancy, and a definitive diagnosis could not be established. After a 2-year interruption in follow-up, pleural thickening had further progressed. Im
