Two decades of prescription opioid-related cardiovascular safety signals: a FAERS-based pharmacovigilance study using machine learning.

INTRODUCTION: Prescription opioids are essential for pain management but have been increasingly linked to cardiovascular adverse events. Understanding these risks is critical for informed prescribing opioid drugs. METHODS: Approximately 18 million adverse event reports from 2004 Quarter 1 to 2024 Quarter 3 in the U.S. FDA Adverse Event Reporting System (FAERS) were downloaded and processed. RxNorm was used to normalize prescription opioid drug names. Empirical Bayes Geometric Mean analysis was then performed to detect signals of disproportionate reporting. Cardiovascular-related Preferred Ter

Blast injury simulation in pigs using an air-driven shock wave generator: effects of shock waves of lateral shock wave exposure.

PURPOSE: We have hypothesized that respiratory arrest caused by temporary brainstem dysfunction is the primary cause of death during the hyperacute phase of blast injury and have demonstrated that this is highly likely. To date, we have examined the effects of shock waves on pigs by exposing them to shock waves from both the dorsal and ventral sides. Therefore, in this study, we investigated the effects of lateral shock wave exposure in pigs and identified priority regions for protection against blast injuries. METHODS: Eight male hybrid pigs (age, 10-12 weeks; mean weight, 36.6 kg) were anes

The "contrast agent separation" sign in a patient on veno-arterial extracorporeal membrane oxygenation undergoing computed tomography scan angiography: A case report.

RATIONALE: In computed tomography angiography (CTA), the phenomenon of contrast agent separation (CAS) is one of the key imaging features for diagnosing arterial dissection. This case highlights that the "contrast agent separation" (CAS) sign in CTA after veno-arterial extracorporeal membrane oxygenation (VA-ECMO) may result from retrograde perfusion exceeding native cardiac output, causing uneven contrast mixing and potential misdiagnosis as Type A aortic dissection (AD). CTA combined with transesophageal echocardiography (TEE) is crucial for accurate diagnosis and management of such acute ao

Using Human Factors and Systems Simulation to Optimize the Usability of a Code Documentation Tool.

OBJECTIVES: Implementing a new electronic health record (EHR) system in high-pressure, high-stakes environments, such as documenting during pediatric and neonatal cardiac and respiratory arrest scenarios, can be challenging for health care staff. Poorly designed tools or technology that don't align with existing workflows may lead to incomplete or inaccurate documentation. Lower adoption rates and continued reliance on paper forms for code documentation can result in gaps in patient data within the EHR, requiring the transcription of critical information after the event. METHODS: This project

Brainstem dysfunction induced by laser-induced shock wave results in hippocampal CA3 neuronal injury in mice.

The brainstem is anatomically vulnerable to trauma, and dysfunction often results in respiratory arrest and subsequent hypoxemia. Although animal models of traumatic brainstem injury have provided insights into local pathology, little is known about secondary effects, such as hypoxic brain injury, in other brain regions. Although a wide variety of traumatic brain injury models have been developed, few have been designed to replicate systemic hypoxemia as a primary insult. In this study, we reproduced brainstem dysfunction using laser-induced shock wave (LISW) and investigated their effect on h

Serotonin Associated Mechanisms in Sudden Unexpected Death in Epilepsy: A Review.

Sudden Unexpected Death in Epilepsy (SUDEP) is the leading cause of death in patients with epilepsy (PWE), although the mechanisms are unclear. Early studies have shown that abnormal cardiopulmonary function plays a key role in SUDEP. Cardiopulmonary activity is regulated by the autonomic nervous system. Serotonin (5-hydroxytryptamine or 5-HT) neurons significantly influence respiration and are also closely related to epilepsy. Therefore, serotonin is hypothesized to be involved in SUDEP, and a substantial amount of research has focused on it. Notably, serotonin signals through at least 14 kno

Pediatric non-traumatic out-of-hospital cardiac arrest in a high-resource metropolitan area: epidemiology and outcomes.

BACKGROUND: Pediatric out-of-hospital cardiac arrest (OHCA) is rare and often triggered by respiratory arrest. Its management is more complex than in adults, with low survival rates. In addition, European epidemiologic data on pediatric OHCA are scarce. This study analyzed the respective key figures of a west-European metropolitan area over the course of several years. METHODS: This retrospective study examined pediatric non-traumatic OHCA cases treated by the Emergency Medical Service (EMS) Vienna between 01/2019 and 12/2023, reporting on incidences, rates of return of spontaneous circulation

MET amplification in diffuse pleural mesothelioma and response to savolitinib: a case report.

BACKGROUND: Pleural mesothelioma (PM) is a rare malignancy globally, most frequently associated with alterations in tumor suppressor genes such as NF2, BAP1 and CDKN2A. In contrast, mutations or amplifications of the MET gene are seldomly reported in PM. CASE DESCRIPTION: Here, we report the case of a 60-year-old East Asian male without asbestos exposure, never-smoker, diagnosed with advanced PM. The disease proved refractory to multiple lines of therapy, including platinum-based chemotherapy, immunotherapy, intrathoracic instillation and a subsequent individualized regimen comprising intrape

Epstein-Barr virus encephalitis in an immunocompetent elderly patient: A case report.

We report a case of severe Epstein-Barr virus (EBV) encephalitis in a 79-year-old immunocompetent patient. The patient initially presented without overt infectious manifestations, complaining solely of weakness in both lower limbs for one week. Physical examination revealed tremors in the upper limbs and tachypnea. Symptoms progressed rapidly after admission, with the subsequent development of headache and fever. Based on the clinical presentation, viral encephalitis was suspected. Cerebrospinal fluid analysis via next-generation sequencing (NGS) confirmed the diagnosis of EBV encephalitis. De

Fatal hyperthermia triggered by drug-induced fever in a patient with high-level spinal cord injury: a case report.

We report a rare fatal hyperthermic crisis likely precipitated by drug-induced fever in a muscular, obese patient with chronic high-level spinal cord injury (SCI). Shortly after antibiotic therapy was changed to meropenem, the patient developed severe agitation and hyperthermia exceeding 41 °C, accompanied by autonomic instability. Despite intensive management, including active cooling, intravenous fluids, and vasoactive support, the core temperature remained above 41 °C. Computed tomography revealed a low-density brainstem lesion consistent with infarction. The patient experienced respiratory

Insights into collapse in ambulances (CIA): a retrospective analysis of interfacility transfers from Emergency Medical Services (EMS) of Sindh, Pakistan.

BACKGROUND: Emergency Medical Services (EMS) is the cornerstone of healthcare service delivery, especially in lower and middle-income countries (LMICs). Given the scarcity of literature, the study determines the rate of patient collapse in ambulances (CIA), in association with history, complaints, and other contributing factors. CIA is a novel terminology coined by the study team, referring to out-of-hospital deaths in ambulances. METHODS: The study employed a retrospective approach from EMS records in Sindh to determine the patient collapse rate from July 2023 to February 2024. The dataset i

Dose-dependent Effects of Propofol on GABAergic Neurotransmission in the Pre-Bötzinger Complex and Its Consequences on Respiratory Rhythm.

BACKGROUND: Propofol acts primarily through γ-aminobutyric acid type A receptor-dependent central inhibition, while local inhibitory microcircuits in the pre-Bötzinger complex (pre-BötC) exert bidirectional control over respiratory rhythm. Although propofol is widely recognized as a simple respiratory depressant, it remains unclear whether propofol mimics endogenous inhibitory signaling to disrupt pre-BötC circuit function in a complex, dose-related manner. This study aimed to test the hypothesis that propofol modulates pre-BötC activity via dose-dependent reshaping of local inhibitory microci

Emergency open repair of war-related traumatic carotid artery injury caused by shrapnel in a resource-limited setting.

Penetrating carotid artery injuries are rare but life-threatening, particularly in resource-limited and conflict settings. We report a case of a 23-year-old male presenting with hypovolemic shock, respiratory arrest, and active neck hemorrhage secondary to a shrapnel injury. Emergency open exploration and primary repair of the carotid artery were performed in a field hospital without access to anesthesiology support, advanced imaging, or intensive care facilities. The patient survived without neurological deficit and was discharged on postoperative day 14. This case highlights the feasibility

A Case of Respiratory Arrest After Lung Cancer Surgery due to Recurarization of Rocuronium Bromide.

Sugammadex has been widely used to reverse rocuronium-induced neuromuscular blockade. Despite apparent complete postoperative recovery, recurrence of neuromuscular blockade (recurarization) has been reported. An 82-year-old man with squamous cell carcinoma of the lung underwent left upper lobectomy. However, 35 min after returning to the ward, he suddenly developed respiratory arrest. Chest radiography and head computed tomography revealed no abnormalities. He recovered promptly after re-administration of sugammadex. To our knowledge, this report has been the first to describe recurarization f

Sequential eradication of bacterial persisters: integrating phytochemical pharmacology with microenvironment-responsive delivery strategies.

Bacterial persister cells within extracellular polymeric substance (EPS) matrices drive antimicrobial tolerance and chronic infection relapse. Conventional bactericidal agents remain fundamentally inadequate against these dormant subpopulations due to their reliance on active cellular metabolism. This review proposes a mechanistically driven, multi-phase sequential strategy-comprising barrier disruption, metabolic resuscitation, and terminal eradication-executed via highly purified, plant-derived natural products and advanced delivery systems. We synthesize recent pharmacological evidence rega
