Neurotoxicity of apamin and MCD peptide upon central application.

Besides apamin, the structurally related MCD peptide (mast cell degranulating peptide; peptide 401) is another centrally acting peptide from bee venom. In contrast to apamin, it is hardly neurotoxic upon intravenous injection in mice. Following intraventricular injection, as little as 0.3 microgram/animal produce convulsions and respiratory arrest in mice. The clinical picture differs from that elicited by apamin, and apamin is about 10 times more potent than MCD peptide when given intraventricularly. Apamin and MCD peptide injected into the spinal cord of rats in nanogram amounts, produce cir

Early recognition and aggressive treatment of gastroesophageal reflux following repair of esophageal atresia.

Twelve patients with gastroesophageal reflux following repair of esophageal atresia are presented. Reflux produced recurrent stricture, failure to thrive, repeated pneumonitis, and in one patient, respiratory arrest and nearly death. Treatment consists of positional therapy or fundoplication operation both of which seem less successful in this combination of lesions than with GER and a normal esophagus. There was one death as a late postoperative complication of fundoplication.

[Distribution of respiratory arrests in preterm and term infants (author's transl)].

Incidence, distribution and periodicity of respiratory arrests larger than or equal to 2 seconds duration were studied in relation to age and sleep states by means of polygraphic recording in 61 tracing of 44 preterm and term infants free of pathologic conditions known to increase the risk of respiratory arrests. Respiratory arrests at any infant's age were found to be more frequent during active sleep. Incidence of respiratory arrests decreases with age in all sleep states. At any age the average duration of respiratory arrest is longer in quiet sleep. When restrictive definition is applied p

Botulism in infancy. Report of a case.

A 22-day-old infant developed infant botulism characterized by profound weakness, hypotonia, respiratory arrest, areflexia, ptosis, pupils that responded poorly to light, and absent gag reflex. Stool examination yielded Clostridium botulinum type B organisms and type B toxin. Electromyography provided rapid diagnostic assistance. With supportive care, reovery was complete. This "new" disease probably is more common than now appreciated.

[Toxicologic and pharmacologic study of tributyltin oxide (TBTO)].

Toxicological study on bis-(tributyl-tin) oxyde shows a delayed toxicity of this compound in acute experiments. The same result is obtained in sub-chronic study. Most of the animals die after a great loss of weight. In pharmacological study, T. B. T. O. depresses central nervous system, but his action on ortho and parasympathetic system is slight and non specific. In rat, rabbit and dog, the effect is a fall in blood pressure that result from a depression of the vascular smooth muscle. The death of the animal is due to respiratory arrest by a central mechanism.

The clinical picture of rabies in man.

After an incubation period of one to two months rabies presents with non-specific prodromal symptoms and often with paraesthesiae of the bitten area. As in canine rabies there are furious and dumb forms of the disease. In man, furious rabies is characterised by hydrophobia: terror and excitation with spasms of inspiratory muscles, larynx and pharynx precipitated by attempts to drink and by a variety of other stimuli. Hydrophobia may represent an exaggerated respiratory tract irritant reflex with associated arousal potentiated by the selective destruction of brain stem inhibitory systmes. Also

[Treatment of alkyl phosphate poisoning with purified serum cholinesterase (author's transl)].

A 26-years-old man injected himself intramuscularly with 3.75 gram of demeton methyl in an attempt to commit suicide. After 36 hours severe symptoms of poisoning developed with loss of consciousness and respiratory arrest. As symptomatic treatment failed to improve his condition has given injections of purified serum cholinesterase. This was followed by a rise in the cholin esterase levels and the increased activity was accompanied by a rapid and definite improvement in the clinical findings. The results clearly indicate the usefulness of this method of treatment. Transfusions of fresh blood t

[Reanimation outside the hospital by the mobile intensive care station in Zurich].

Of 502 patients handled by a mobile intensive care unit, 70 were resuscitated: 20 immediately, 9 within 3 min and 41 later than 3 min from onset of cardiac or respiratory arrest. There were 9 temporary and 10 longterm survivors among the 29 patients who underwent resuscitative measures within 3 min of collapse. First-aid training programs in resuscitation methods are a necessary supportive measure to the operation of mobile intensive care units.

Respiratory paralysis during treatment of hypertension with trimethaphan camsylate.

Four cases are reported in which respiratory arrest occured coincident with the intravenous administration of large doses of trimethaphan camsylate (Arfonad) to control hypertension. The mechanism of the respiratory depression is unknown, but it may have been related to a direct effect of trimethaphan on the respiratory center or to a curare-like effect of the drug. Close monitoring of ventilatory capacity should be maintained in all patients treated with trimethaphan.

An animal model of fulminant hepatic failure: a feasibility study.

An attempt was made to produce animal model of fulminant hepatic failure in the pig by intragastric administration of acetaminophen (N-acetyl-p-aminophenol). The effect of varying doses was observed in phenobarbital-pretreated and untreated animals. Assessment of optimal dose was made in terms of duration of survival and coma in animals exhibiting biochemical and histological evidence of liver necrosis. Significant liver necrosis was observed in enzyme-induced pigs only, whereas in the noninduced pigs respiratory arrest was considered a likely cause of death. Although liver necrosis occurred c

Cardiac slowing and respiratory arrest in preterm infants.

Changes in respiration and heart rate during sleep states have been recorded by a polygraphic device in healthy preterm infants. Cardiac slowing/bradycardia often coincide with respiratory arrest/apnea. Bradycardia starts early during apneic spells. The incidence of respiratory arrest and cardiac slowing and their simultaneous occurrence is significantly increased by the active or REM sleep state. The physiologic, inhibitory mechanisms of active sleep suggest a neurogenic etiology of episodes of cardiac slowing/bradycardia and/or respiratory arrest/apnea in prematures.

'Juvenile' myasthenia gravis in early infancy.

A previously well infant developed severe muscle weakness and hypotonia at 6 months of age. This was reversed by anticholinesterase medication. However, she had subsequent further weakness and died at 10 months after an acute respiratory arrest. The clinical pattern was that of the 'juvenile' form of myasthenia gravis rather than the 'congenital' forms which have previously been described in early infancy.

Primary respiratory arrest due to basilar aneurysm.

A healthy woman had two episodes of transient, complete respiratory arrest without preceding neurologic abnormalities. Following the second episode, she lapsed into coma and autopsy disclosed a thrombosed basilar artery aneurysm. Primary respiratory arrest is a rare condition but should prompt extensive and immediate neurologic evaluation.

The influence of upper respiratory tract receptors stimulation on the respiration and heart action in rats.

1. Stimulation of the upper respiratory tract with water releases reflex respiratory arrest and very marked bradycardia due to conduction and impulse generation disturbances in the heart accompanied by ventricular hyperexcitability. 2. Asphyxia causes bradycardia as a result of an inhibition of sinus node and the development of vicarious rhythms with excessive ectopic ventricular excitability. The intensity and duration of bradycardia during asphyxia are less than during water stimulation of airways. 3. Urethane anaesthesia did not prevent the development of the above described changes appeari

Two cases of spontaneous cerebral ventriculostium are presented. The first case is that of a 3 year-old girl with a thumb-sized soft scalp tumor of the occipital region (dural hypertrophy) and hydroencephalodysplasia (Picaza). PVG revealed noncommunicating hydrocephalus with asymmetrical deformity of the lateral ventricle and agenesis of corpus callosum (Fig. 1). Ventriculoatrial shunt was performed. Three years passed under the useful life when she readmitted to our clinic complaining headache, nausea and vomiting. On the first hospital day she fell into respiratory arrest accompanied with co

Fracture of the odontoid process.

A review of 26 cases of odontoid fractures has been carried out. Late diagnosis and consequent delay in treatment were found to be important causes of non-union. Myelopathy in odontoid fractures is of two types. One type is that present immediately after injury, while the other is delayed. In treatment of long-standing cases, only fusion without posterior decompression should be done. If the latter procedure is carried out, it can produce medullospinal haematomyelia with respiratory arrest and tetraplegia. In the performance of atlanto-axial fusion, the technique of bone grafting and the use o

[Respiratory effects of electric stimulation of pancreatic afferents in lightly anesthetized cats].

Electrical stimulation of the pancreatic anterior part produces a respiratory arrest followed by a short polypnea. In some cases (20%) the same effects are obtained by electrical stimulation of the afferent nerves. The afferent fibres involved in this phenomena are situated in the great splanchnic nerves.
