Reimagining land system science in seascapes.

Hearing impairment is due to various causes including ototoxicity from aminoglycosides. The susceptibility to aminoglycosides increases in the presence of certain mitochondria gene mutations. There is unrestrained use of aminoglycosides in many developing nations which may worsen the burden of hearing impairment in these countries but there is lack of data to drive required policy changes. Streptomycin (an aminoglycoside) is part of the drug regimen in re-treatment of tuberculosis. Exploring the impact of streptomycin ototoxicity in tuberculosis patients provides a unique opportunity to study

A new anesthesia management for giant intracardiac and intravenous leiomyomatosis.

BACKGROUND AND CLINICAL SIGNIFICANCE: Intravenous leiomyomatosis is a very rare disease that occurs in women of childbearing age and perimenopausal women. It starts from the wall of the venous blood vessels of the uterus or pelvis, which spread upward along the blood vessels to the inferior vena cava. When the tumor extends into the cardiac chambers, patients are at high risk for perioperative anesthesia when they undergo surgery. CASE PRESENTATION: A 38-year-old female patient was admitted to the hospital due to recurrent syncope. Transthoracic echocardiography revealed a mass occupying the r

Magnesium sulphate for women at risk of preterm birth for neuroprotection of the fetus.

BACKGROUND: Magnesium sulphate is a common therapy in perinatal care. Its benefits when given to women at risk of preterm birth for fetal neuroprotection (prevention of cerebral palsy for children) were shown in a 2009 Cochrane review. Internationally, use of magnesium sulphate for preterm cerebral palsy prevention is now recommended practice. As new randomised controlled trials (RCTs) and longer-term follow-up of prior RCTs have since been conducted, this review updates the previously published version. OBJECTIVES: To assess the effectiveness and safety of magnesium sulphate as a fetal neuro

Seizure-induced apnea causes sudden death independent of cortical spike wave discharge in mouse models of Tuberous Sclerosis Complex.

Sudden unexpected death in epilepsy (SUDEP) is the leading cause of seizure-related mortality for people with epilepsy (PWE). Current evidence suggests that many cases of SUDEP are due to seizures that produce respiratory arrest, aka seizure-induced apnea. Patients with tuberous sclerosis complex (TSC) are at a significant risk of SUDEP, and mouse models of TSC have early mortality. Previous reports in TSC mouse models demonstrate that although Vigabatrin suppresses seizures, it does not improve survival. In other mouse models, our team has demonstrated that seizure-induced apnea occurs during

Case Report: Successful multimodal management of a lethal cyanide poisoning - insights into the role of early blood purification and potential bedside monitoring indicators.

This article reports a rare and lethal case of cyanide poisoning, aiming to explore its early diagnosis and successful treatment strategies. Upon admission, the patient presented with cardiac and respiratory arrest, deep coma, and severe metabolic acidosis. The core resuscitation measures included early mechanical ventilation, vasoactive drug support, bicarbonate therapy, and the early initiation of continuous blood purification as part of a multimodal strategy. We observed that a combination of elevated central venous partial pressure of carbon dioxide (PvCO2) and elevated lactate provided po

The Differential Diagnosis of Coma in the ICU: Hyperacute Postoperative Guillain-Barré Syndrome-A Case Report.

Guillain-Barré syndrome (GBS) is a rare, immune-mediated neurological disorder that can be challenging to diagnose in postoperative patients due to atypical manifestations and overlapping conditions. This case report highlights the diagnostic and therapeutic challenges of GBS following vascular surgery. We present the case of a 56-year-old man scheduled for elective femoro-popliteal bypass surgery. The initial neurological development of the patient's GBS was masked by delirium requiring sedation and culminated with cardio-respiratory arrest in the 72 h following admission in the post-operativ

Prophylactic use of extracorporeal membrane oxygenation for complete traumatic tracheal rupture: A case report.

Traumatic tracheal rupture is a rare, critical, and life-threatening injury, which could quickly result in asphyxia, even cardiac and respiratory arrest. We managed the case of a 19-year-old female student who suffered a neck injury due to an impact with a bulldozer. A CT scan revealed a complete rupture of the trachea. Tracheal intubation was performed due to respiratory failure. Fiberoptic bronchoscopy showed that the tracheal tube's tip was completely obstructed by soft tissue, allowing ventilation only through the tube's side port. After the prophylactic use of veno-venous extracorporeal m

Early aggressive treatment in WFNS grade v aneurysmal subarachnoid hemorrhage with respiratory or cardiac arrest.

BACKGROUND: With advances in emergency medical systems, an increasing number of patients with aneurysmal subarachnoid hemorrhage (SAH) complicated by respiratory or cardiac arrest (CA) are transported alive to emergency centers. However, the pathophysiology and optimal management of these ultra-severe cases remain poorly defined. We hypothesized that cardiopulmonary status at presentation may help identify patients with physiological reversibility and potential for meaningful recovery. We evaluated clinical outcomes and factors associated with favorable recovery in patients with World Federati

Successful prehospital resuscitation of a young female patient with myasthenia gravis experiencing cardiopulmonary arrest due to complete airway obstruction: a case report and review of the literature.

INTRODUCTION: Patients with myasthenia gravis are at high risk of fatal airway complications due to impaired swallowing mechanics. We present a prehospital resuscitation case involving complete airway obstruction leading to cardiopulmonary arrest in a young patient with myasthenia gravis. CASE PRESENTATION: A 21-year-old Han Chinese female patient with a reported history of myasthenia gravis developed sudden respiratory arrest during oral intake. The diagnosis of myasthenia gravis was initially provided by on-scene witnesses (classmates), and later confirmed by family who reported her medicat

CMR-based aortic stiffness parameters in a symptomatic high risk obstructive sleep apnea cohort: results from the population-based Hamburg City Health Study.

BACKGROUND: Aortic stiffness (AS) parameters as pulse wave velocity (PWV), aortic distensibility of the ascending and descending aorta (AD AoAsc, AD AoDesc) and pulse pressure (PP) are marker for vascular ageing, whereas data concerning obstructive sleep apnea (OSA) in population-based cohorts are sparse. Thus, we aimed to identify possible associations of AS with OSA in the Hamburg City Health Study (HCHS) cardiovascular magnetic resonance (CMR) cohort. METHODS: HCHS is a population-based cohort-study quantifying PWV and AD by 2D-phase-contrast-CMR-measuring-methods and PP by blood pressure.

Safety and major adverse events of endobronchial ultrasound performed under conscious sedation: Insights from 3,454 procedures over 14 years and a systematic review of literature.

BACKGROUND: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is widely used for evaluating mediastinal lymphadenopathy. However, its safety profile requires comprehensive evaluation. This study aimed to determine the prevalence of complications associated with EBUS-TBNA and to conduct a systematic review and meta-analysis to estimate the pooled complication rates. METHODS: We retrospectively analysed a prospectively maintained database of patients who underwent EBUS between April 2011 and April 2025. We extracted demographic, procedural, and complication-related da

Do Chest Compressions Benefit the Brain Beyond Restoring Circulation? ROSC-Mediated and ROSC-Independent Pathways of Bystander CPR After Out-of-Hospital Cardiac Arrest.

Background/Objectives: Bystander cardiopulmonary resuscitation (CPR) improves outcome after out-of-hospital cardiac arrest (OHCA), but whether its neurological benefit operates solely through prehospital return of spontaneous circulation (ROSC) is unresolved. We quantified the mediated and unmediated components. Methods: Retrospective cohort study of the Korean nationwide OHCA registry (2009-2024) in adults with cardiac or respiratory arrest, excluding arrests witnessed by on-duty personnel. The primary outcome was favorable neurological survival (Cerebral Performance Category 1-2 at discharge

Respiratory Failure due to Glial Fibrillary Acidic Protein Antibody-Associated Disorders: Recovery Following Plasma Exchange.

Glial fibrillary acidic protein antibody-associated disorders (GFAP-ADs) generally respond to steroid treatment but may become fulminant when brainstem lesions cause central hypoventilation. A 56-year-old man developed fever, myoclonus, gait instability, urinary retention, and altered consciousness. An analysis of cerebrospinal fluid (CSF) showed pleocytosis and high protein levels, and magnetic resonance imaging (MRI) demonstrated diffuse leptomeningeal enhancement of the brainstem, cerebral cortex, and spinal cord. Despite two courses of intravenous methylprednisolone, he developed respirato

Accidental Subdural Injection of a Lidocaine With Epinephrine Test Dose Leading to Respiratory Arrest in a Parturient Patient: Case Report.

BACKGROUND: A subdural injection is a rare complication of neuraxial anesthesia where medication is accidentally injected into the potential space between the dura mater and arachnoid mater of the spinal cord. Subdural blocks are typically underdiagnosed due to their variable presentation; failed early detection can lead to significant long-term consequences. CASE REPORT: This is a unique case of a 33-year-old parturient who presented for epidural placement during which a 3 mL subdural injection of preservative-free 1.5% lidocaine with 1:200,000 epinephrine test dose led to rapid clinical det

Simultaneous Acute Multiple Ischemic Strokes of the Left Cerebral Hemisphere and Signs of Acute Anterior STEMI: A Case Report.

BACKGROUND Acute cerebral infarction and acute myocardial infarction are both common life-threatening conditions with high incidence rates. The simultaneous occurrence of these conditions is extremely rare. Most documented cases are limited to case reports, and no specific treatment guidelines have been established. Treatment strategies considerably vary among individuals; the prognosis is often poor, particularly in older patients. Studies of cardio-cerebral infarction are important to reduce mortality and disability. CASE REPORT A 62-year-old man was admitted to the hospital with right-sided
