Presumed Fatal Bronchospasm From Adenosine Administered for Assessment of Fractional Flow Reserve.

BACKGROUND: Adenosine is a commonly used vasodilating agent to induce hyperemia in fractional flow reserve. Most side effects are transient and self-limiting because of its short half-life. CASE SUMMARY: A 79-year-old man with known airway disease underwent coronary angiography for unstable angina. A moderate-to-severe lesion was found in the proximal left anterior descending artery. Pressure wire assessment was undertaken and intravenous adenosine was administered. This resulted in severe bronchospasm progressing to respiratory arrest. Despite resuscitation attempts, the patient did not surv

Fatal Expanded Dengue Syndrome Mimicking Heat Stroke With Hyperpyrexia, Subarachnoid Hemorrhage, and Refractory Multiorgan Dysfunction: A Case Report.

Dengue is a major mosquito-borne viral illness affecting tropical and subtropical regions worldwide and remains an important cause of critical care admissions. Although most infections are self-limited, a subset progresses to severe dengue characterized by plasma leakage, shock, severe bleeding, and end-organ dysfunction. Neurological manifestations are uncommon but increasingly recognized under the spectrum of expanded dengue syndrome, including seizures, encephalopathy, cerebral edema, ischemic stroke, and intracranial hemorrhage. We report a fatal case of a 29-year-old previously healthy ma

PLAIN ENGLISH SUMMARY: Outcome domains are aspects of a condition that matter to patients and clinicians and can be measured to assess treatment effects. For tinnitus, examples include 'tinnitus loudness' and 'ability to concentrate'. This study focuses on the first stage of agreeing which outcome domains should be measured in all clinical trials of tinnitus. Crucially, it involves identifying outcome domains, prior to a voting process. This article describes how we effectively involved patients in that study design process, and reflects on the impact of their input. The study first compiled a

Phenotypic and genotypic characterization of Staphylococcus aureus, including MRSA and PVL-positive isolates, in Egyptian fruit bats.

BACKGROUND: Bats are widely distributed mammals capable of harboring diverse pathogens, including antimicrobial-resistant bacteria. Staphylococcus aureus (S. aureus), particularly methicillin-resistant S. aureus (MRSA), poses a significant public health concern. This study aimed to investigate the occurrence, antimicrobial resistance profile, and presence of the Panton-Valentine leukocidin (pvl) virulence gene of S. aureus in Egyptian fruit bats. METHODS: Fifty Egyptian fruit bats were captured using mist nets at both foraging and roosting sites and morphologically identified. A total of 150

Structural fragility in digital public health communication.

Inherited genetic disorders are progressive in nature and lead to organ dysfunction or death in severe cases. At present, there are no permanent treatment options for >95% of inherited disorders. Different modes of inheritance, type of gene(s) involved, and population-based variations add further complexity to finding suitable cures for approximately 400 million patients worldwide. Gene therapy is a very promising molecular technique for the treatment of rare genetic disorders. Gene therapy functions on the basis of restoration, replacement, inhibition, and, most recently, editing of gene(s) t

Structural basis for Mg²⁺-glycine synergy in the tandem glycine riboswitch revealed by cryo-EM.

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

Targeting ALDH2 with Alda-1 to reverse cisplatin resistance in lung adenocarcinoma.

BACKGROUND: Cisplatin resistance remains a major obstacle in lung adenocarcinoma (LUAD) treatment. The role of Aldehyde dehydrogenase 2 (ALDH2), a detoxifying enzyme, in LUAD prognosis and chemoresistance is poorly understood. METHODS: We analyzed ALDH2's prognostic value using clinical cohorts, TCGA, and proteomic data. Cisplatin-resistant cell lines and xenograft models were used to assess the effect of the ALDH2 agonist Alda-1. Molecular mechanisms were investigated via gain/loss-of-function studies. RESULTS: High ALDH2 expression was significantly associated with improved survival in uni

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Authorship Inequity in Global Health Research Conducted in Low- and Middle-Income Countries and Published in High-Income Country Family Medicine Journals.

PURPOSE: The current structures of global health research and academic authorship have resulted in underrepresentation of authors from low- and middle-income countries. Although authorship inequity has been shown in other specialties, the current status of authorship in family medicine has not been examined. METHODS: We conducted a bibliometric analysis of World Organization of Family Doctors (WONCA) journals based in high-income countries for articles describing research conducted in low-income countries, lower-middle-income countries, and upper-middle-income countries from 2018 to 2023. Sudden unexpected death in epilepsy: respiratory vs. cardiac contributions.

Sudden unexpected death in epilepsy (SUDEP) poses a significant risk to life expectancy for individuals with epilepsy. Mechanistic insight, while incomplete, has advanced through clinical observational studies and animal models. Yet we lack preventative therapies, which will depend on understanding SUDEP mechanisms. Recurrent convulsive seizures are the major SUDEP risk factor. Cardiorespiratory dysfunction precedes SUDEP, but whether cardiac arrhythmias are major proximate culprits for SUDEP remains to be determined. Here, we highlight recent data from mouse models and clinical studies that p

Case report: Multisystemic smooth muscle dysfunction syndrome: a rare genetic cause of infantile interstitial lung disease.

Multisystemic smooth muscle dysfunction syndrome (MSMDS) is an autosomal dominant disorder caused by mutations in the ACTA2 gene, resulting in variable clinical manifestation and multi-organ dysfunction. Interstitial lung disease (ILD) is a rare phenotype of this condition. We describe a rare infant case of an 8-month-old boy who presented with progressively worsening dyspnea, along with intermittent episodes of respiratory distress and cyanosis since birth. A chest CT scan revealed typical signs of ILD. Additionally, the patient exhibited congenital mydriasis, aortic coarctation, PDA, and pul

Electrocardiogram indicators for risk of respiratory failure in patients with tetrodotoxin poisoning.

Tetrodotoxin blocks voltage-gated sodium channels, which are responsible for the initiation and propagation of action potentials. Impairment of action potential conduction in myocardial cells affects the cardiac conduction system and can influence electrocardiogram output. In this retrospective study, we investigated whether tetrodotoxin poisoning prolong the PR, QRS and QTc intervals, which might be used as prognostic indices of respiratory failure after tetrodotoxin poisoning. Additionally, we evaluated whether these changes correlate with tetrodotoxin concentration in the body. Eighty-four

Predictive modeling of methadone poisoning outcomes in children ≤ 5 years: utilizing machine learning and the National Poison Data System for improved clinical decision-making.

UNLABELLED: The escalating therapeutic use of methadone has coincided with an increase in accidental ingestions, particularly among children ≤ 5 years. This study utilized machine learning (ML) methodologies on data from the National Poison Data System (NPDS) to predict pediatric methadone poisoning outcomes to enhance clinical decision-making. We analyzed 140 medical parameters from pediatric patient records. Pre-processing steps, including synthetic oversampling, addressed the imbalanced distribution of the outcome variable. We evaluated various ML models in multiclass classification tasks.

MK-212 precipitates seizure-induced death in amygdala-kindled mice via a non-5-HT2C receptor-mediated mechanism.

Epilepsy is a common neurological condition that affects over 65 million people worldwide. Despite an increasing number of anti-seizure medications being made available, many patients do not find seizure freedom with medication. The leading cause of death in this refractory population is sudden unexpected death in epilepsy (SUDEP). Both human and animal research has implicated serotonin (5-HT) in modulating seizure proclivity, severity, and mortality. More recently, evidence has pointed to the 5-HT2C receptor as a salient target for investigating the mechanisms of seizure facilitation and mort

Conventional Cardiopulmonary Resuscitation Versus Extracorporeal Membrane Oxygenation-Assisted CPR in Children: A Retrospective Analysis of Outcomes and Factors Associated with Conversion from the Former to the Latter.

BACKGROUND/OBJECTIVES: Conventional cardiopulmonary resuscitation (CCPR) has been the foundational resuscitation approach for decades. Where CCPR is unsuccessful, extracorporeal membrane oxygenation-assisted CPR (ECPR) may improve outcomes. Predicting failure of CCPR and immediate need for ECPR is difficult, and data are lacking. In this retrospective analysis, we analysed both factors that are associated with conversion from CCPR to ECPR and survival outcomes for each event. METHODS: Patients having a CPR event that occurred in the PICU between 2016 and 2022 were included. Pre-CPR-event clin

Dynamic left ventricular outflow tract obstruction induced by intra-aortic balloon pump in patient with angioedema.

BACKGROUND: Intra-aortic balloon pump is used for temporary mechanical support of failing left ventricle. It works by reducing the arterial afterload during ventricular systole to reduce myocardial work and increasing diastolic proximal aortic pressure to improve coronary perfusion. Rarely, intra-aortic balloon pump (IABP) can become the cause of severe haemodynamic compromise, causing dynamic left ventricular outflow tract obstruction. CASE PRESENTATION: An 88-yo man presented with angiotensin converting enzyme inhibitor (ACEI) - induced angioedema. He received steroids and adrenaline, but p
