Cirrhosis and hyponatremia: A review of pathogenesis, clinical relevance, and management.

This narrative review examines the prognostic significance of sodium levels in cirrhosis. Clinical findings such as ascites typically develop between 5 and 10 years post-diagnosis, often with dysregulated sodium balance. About 50 % of cirrhosis patients develop hyponatremia, linked to poor prognosis, necessitating effective assessment and management. Hyponatremia arises from splanchnic vasodilation, causing fluid accumulation and reduced effective arterial volume, which triggers adaptive responses that worsen water retention and sodium imbalance. Ascitic fluid contains significant sodium store

Severe thrombocytopenia induced by niraparib in ovarian cancer patients: a case report and literature review.

Ovarian cancer (OC) is a common gynecological malignancy, and poly (ADP-ribose) polymerase inhibitor (PARPi) maintenance therapy has significantly prolonged the survival of newly diagnosed or platinum-sensitive recurrent ovarian cancer patients, which has become a new treatment paradigm for ovarian cancer. Niraparib is a widely used PARP inhibitor in the clinical setting, and its adverse effects are also a major concern. The common adverse reactions of Niraparib mainly include various hematological adverse reactions (such as thrombocytopenia and anemia), gastrointestinal adverse reactions (suc

Diagnostic Evaluation and Management of Idiopathic Chylous Ascites: A Report of a Surgical Case.

Chylous ascites (CA) is the pathological accumulation of lymphatic fluid in the peritoneal cavity. The ascitic fluid has a high concentration of triglycerides and chylomicrons, giving it a milky appearance. Owing to its infrequent diagnosis, there is limited literature describing the workup and treatment of idiopathic chylous ascites (ICA). This report describes the case of a 39-year-old woman who presented with undifferentiated abdominal pain, with the investigations leading to a diagnosis of ICA and interventions which led to complete resolution of symptoms over a follow-up period of eight m

Ultrasound-guided modified thoracoabdominal nerve block for postoperative analgesia in laparoscopic renal cyst decompression: a randomized double-blind controlled trial.

BACKGROUND: Laparoscopic renal cyst decompression (LRCD) is a common procedure in urology, but postoperative pain remains a significant challenge. While regional nerve blocks provide more targeted pain relief, there is no universally accepted pain management strategy for LRCD. The ultrasound-guided modified thoracoabdominal nerve block (M-TAPA) may offer effective analgesia by blocking the anterior and lateral branches of the intercostal nerves (T5-T12). However, its efficacy in LRCD has not been thoroughly evaluated. OBJECTIVE: This study aimed to assess the efficacy and safety of unilateral

Freeze the Disease: Advances the Therapy for Barrett's Esophagus and Esophageal Adenocarcinoma.

Cryotherapy involves flash freezing of tissue and removing unwanted tissue. Mechanism of injury is causing cell membrane rupture by rapid multiple freeze-thaw cycles, while reserving tissue architecture and the collagen matrix. This promotes favorable wound healing. In recent years, it has gained increasing attention as a treatment option for upper gastrointestinal diseases (Barrett's Esophagus and early cancer). Currently, two FDA-approved delivery methods are available in the GI tract: Cryoballoon and spray cryotherapy, which will be discussed. In this review, we also propose to examine the

Multi-Center Validation of Artificial Intelligence-Based Video Analysis Platform for Automatic Evaluation of Swallowing Disorders.

Background: Videofluoroscopic swallow study (VFSS) is a key examination for assessing swallowing function. Although several artificial intelligence (AI) models for VFSS interpretation have shown high predictive accuracy through internal validations, AI models that have undergone external validation are rare. This study aims to develop an AI model that automatically diagnoses aspiration and penetration from VFSS videos and to evaluate the model's performance through multicenter external validation. Methods: Among the 2343 VFSS videos collected, 309 cases of Q1-grade videos, which were free of a

Impact of early postoperative oral hydration in paediatric patients undergoing elective surgery after general anaesthesia. A randomised controlled trial.

BACKGROUND AND AIMS: Early feeding or oral hydration post-surgery in paediatric patients has conventionally not been encouraged, with fasting periods ranging from 3 to 6 h. The primary objective of this study was to compare the Face, Legs, Activity, Cry, Consolability (FLACC) score as a surrogate for patient satisfaction in younger children between those who received early versus delayed postoperative feeding/hydration. Secondary objectives were to compare the incidence of complications, perioperative clinical parameters and parental satisfaction. METHODS: A single-blind, randomised trial was

Comparison of remimazolam-based and propofol-based general anaesthesia on postoperative quality of recovery in patients undergoing laparoscopic sleeve gastrectomy: protocol for a prospective, randomised, parallel-group, non-inferiority trial.

INTRODUCTION: Remimazolam is a novel short-acting benzodiazepine that exhibits sedative and hypnotic properties without compromising respiratory function and while maintaining haemodynamic stability. Its safety and efficacy have been demonstrated to be non-inferior to those of propofol in the context of general anaesthesia. Nevertheless, the non-inferiority in terms of postoperative recovery quality in obese patients has not been established. Thus, we conducted a prospective, randomised, parallel-group, non-inferiority study to compare remimazolam-based general anaesthesia with propofol-based

Management of Recurrent Transudative Effusions in Congestive Heart Failure and Hepatic Hydrothorax.

Pleural effusions are commonly present in the setting of decompensated heart failure (up to 87%) and liver failure (5%-16%). Notably, patients with decompensated congestive heart failure-induced pleural effusions have a significant 1 year mortality rate from 46% to 50%, while decompensated liver failure accompanied by hepatic hydrothorax demonstrates even higher mortality, particularly when complicated by spontaneous bacterial empyema (51% compared to 19% with refractory ascites). Diagnosis typically involves comprehensive clinical assessment alongside pleural fluid analysis. However, the mana
