[Enteral contrast media-when, what, and how?].

BACKGROUND: Enteral contrast media (CM) administration in abdominal computed tomography (CT) is handled inconsistently across Germany. OBJECTIVE: To present the current recommendations on enteral CM administration based on the key questions of when, what, and how, and to translate them into radiological practice. MATERIALS AND METHODS: Synthesis of indication-based recommendations, CM classes and their technical properties, and the practical execution of both routes of administration, based on the German Radiological Society (DRG) recommendations, current guidelines, published evidence, and

Disseminated cryptococcosis with extensive cutaneous involvement in a previously healthy adult: a rare case report.

Cryptococcosis is an opportunistic fungal infection that predominantly affects immunocompromised individuals but may also cause disseminated disease in previously healthy individuals. We report a previously healthy 31-year-old man who developed extensive infiltrated erythematous papules and plaques after granulomatous mesenteric lymphadenitis presenting as acute abdomen. Skin biopsy revealed encapsulated yeast forms consistent with cryptococcosis, although fungal culture was negative. Imaging demonstrated pulmonary, lymphatic, and hepatobiliary involvement. HIV testing was negative, and no kno

Abdominal allergic reaction syndrome: a tentative definition.

BACKGROUND: Some patients with acute abdominal pain present with imaging findings suggestive of severe intestinal pathology, yet they remain hemodynamically stable and respond to corticosteroids rather than surgery. Recognizing this pattern early can prevent unnecessary laparotomy. CASES: Three female patients (aged 91, 78, and 57 years) admitted to the emergency department presented with acute abdominal pain and diffuse bowel wall edema on a CT scan. Despite alarming imaging results, all three patients remained hemodynamically stable. The symptoms improved rapidly with intravenous methylpred

Spontaneous Urinary Bladder Rupture Secondary to OHVIRA Syndrome.

As a rare mesonephric duct-induced Mullerian anomaly, OHVIRA syndrome is characterised by the triad of uterus didelphys, obstructed hemivagina and ipsilateral renal anomaly. Equally rare is the spontaneous rupture of the urinary bladder (SRUB), a condition that constitutes a life-threatening surgical emergency. This case represents an unprecedented presentation of OHVIRA syndrome involving SRUB. A 19-year-old female presented with acute abdomen superimposed on a long-standing progressive dysmenorrhea and recent dysuria. Diagnosis of OHVIRA syndrome was confirmed by imaging; however, SRUB was o

Evaluating Explicit Anatomical Priors Beyond a Mature CT Detector for Six Acute Abdominal Emergencies.

To test whether anatomical priors from a general-purpose organ segmentation model (TotalSegmentator, TS), applied at inference or during training, improve an externally validated YOLOv11 detector for six acute abdominal emergencies. The frozen detector was applied to 827 external Stanford Merlin CT studies with language-model report labels; a nested 280-study radiologist-adjudicated subset was the reference standard. TS-derived slice selection (Arm A), organ-state integration (Arm B), and their combination (Arm C) were compared with the detector alone under frozen, recalibrated, and fine-tuned

A Critical Methodological Review of Clinical Scores, Machine Learning, and Large Language Models for the Diagnosis and Management of Pediatric Appendicitis.

Background/Objectives: Acute appendicitis is the most common surgical emergency of childhood, yet its diagnosis remains difficult because presentations are atypical, inflammatory markers are nonspecific, and the consequences of error run in both directions, from negative appendectomy to missed perforation. Over the past two decades, a large body of work has tried to support this decision with clinical scores, and more recently with machine learning, deep learning, and large language models. Methods: This critical methodological review synthesises that literature across the whole care pathway r

Jujube pit ingestion leading to ileal perforation and secondary acute appendicitis in a child: a case report.

BACKGROUND: Acute appendicitis is a common pediatric surgical emergency. While most cases are primary, secondary appendicitis resulting from intra-abdominal pathologies like intestinal perforation is rare and diagnostically challenging. Sharp foreign body ingestion, such as jujube pits, can lead to intestinal perforation, peritonitis, and subsequent inflammation of adjacent organs, including the appendix. CASE DESCRIPTION: We present the case of an 8-year-old boy who presented with a one-day history of migratory right lower quadrant abdominal pain, nausea, and high fever (39.2 ℃). Laboratory

Heterotopic pancreas as a rare lead point of ileal intussusception mimicking acute appendicitis in a child: a case report and review of the literature.

INTRODUCTION AND IMPORTANCE: Heterotopic pancreas is an uncommon developmental anomaly characterized by pancreatic tissue located outside the normal pancreas without anatomical or vascular continuity. Although it is most frequently identified in the stomach and duodenum, involvement of the ileum is rare, particularly in children. When present in the small intestine, it may act as a pathological lead point for intussusception, posing a considerable diagnostic challenge because the underlying lesion is often not identified by preoperative imaging. PRESENTATION OF CASE: We report the case of a 6

Retroperitoneal necrotizing fasciitis masquerading as perforated viscus: a rare case report.

INTRODUCTION AND IMPORTANCE: Necrotizing fasciitis is a rapidly progressive, life-threatening soft-tissue infection associated with high mortality, particularly when diagnosis is delayed. While most cases involve the extremities or perineum, retroperitoneal necrotizing fasciitis is exceedingly rare. Its deep anatomical location often results in vague clinical presentations, allowing extensive infection before overt signs develop, which poses significant diagnostic and therapeutic challenges. Early recognition and timely intervention are critical to improving outcomes. PRESENTATION OF CASE: We

A diagnostic cascade from cholecystitis to Meckel's diverticulum rupture: a case report.

BACKGROUND: Meckel's diverticulum (MD) is the most common congenital anomaly of the gastrointestinal tract, yet it rarely becomes symptomatic in adults. Perforation is an uncommon and often overlooked complication that may mimic more frequent abdominal conditions, particularly in the postoperative setting. MD presenting shortly after unrelated laparoscopic surgery is exceptionally rare and can pose a significant diagnostic challenge. CASE PRESENTATION: A 36-year-old woman presented with acute right upper quadrant pain and was diagnosed with calculous cholecystitis. She underwent laparoscopic

Stable disease after multimodal treatment of SMARCA4-deficient undifferentiated tumor concurrently involving the stomach and mediastinum: a case report.

We report a clinically rare SMARCA4-deficient undifferentiated tumor concurrently involving the stomach and mediastinum. A 54-year-old man presented with a 1-month history of left upper abdominal pain radiating to the anterior chest and back. Contrast-enhanced computed tomography of the chest and abdomen showed thickening and abnormal enhancement of the gastric wall along the greater curvature, multiple enlarged lymph nodes along the lesser curvature, and a posterior inferior mediastinal mass. Biopsies of both the gastric body lesion and the mediastinal mass revealed a poorly differentiated/un

Neonatal appendicitis with rectal bleeding: an unusual presentation.

BACKGROUND: Neonatal appendicitis is an exceptionally rare condition with a high risk of delayed diagnosis and perforation. Its nonspecific clinical presentation may mimic other causes of acute abdomen and gastrointestinal bleeding in newborns, particularly necrotizing enterocolitis. CASE PRESENTATION: A 36-week, 2500-g male neonate developed abdominal distension, irritability, decreased oral intake, and bright red rectal bleeding on the second day of life. Abdominal radiography demonstrated multiple air-fluid levels, while ultrasonography revealed dilated bowel loops and free intraperitoneal

Giant ectopic splenic volvulus: a rare surgical emergency.

BACKGROUND: Splenic volvulus is a rare condition most often associated with a wandering spleen (WS) due to congenital or acquired ligamentous laxity. A giant ectopic splenic volvulus in adults without identifiable risk factors is exceptionally rare. CASE PRESENTATION: We report the case of a 46-year-old woman with no significant past medical history who presented with acute abdominal pain. Computed tomography (CT) revealed a giant ectopic spleen measuring approximately 20 cm, with torsion of the splenic pedicle, consistent with splenic volvulus. Emergency surgery was performed. Giant Jejunal Diverticulosis Causing Acute Mesenteric Volvulus: a Case Report.

BACKGROUND: Large jejunal diverticula are usually asymptomatic but can cause rare, life-threatening complications including obstruction, perforation, and bleeding. Secondary mesenteric volvulus caused by jejunal diverticulosis is exceptionally rare. It should be strongly considered when computed tomography (CT) reveals a mesenteric "whirl sign" in an adult presenting with acute obstructive symptoms. CASE PRESENTATION: A 76-year-old female presented to the emergency department with acute, diffuse abdominal pain and clinical evidence of mechanical intestinal obstruction. A CT scan demonstrated

Laparoscopic and Robotic Appendectomy: Efficiency and Cost Comparison.

Acute appendicitis is a common cause of emergency abdominal surgery, most frequently affecting adolescents and young adults. Diagnosis is often complicated by variable clinical presentations, and timely recognition is critical to prevent complications such as perforation, peritonitis, and intra-abdominal abscess formation. Minimally invasive surgery (MIS) has transformed the operative management of appendicitis with laparoscopic appendectomy (LA) established as the standard operative approach due to reduced surgical trauma, faster recovery, lower complications rates, and shorter hospital stay

Duodenal perforation masquerading as an intra-abdominal abscess: successful management with pedicled falciformopexy.

Duodenal perforation typically presents as an acute abdomen. In contrast, presentation as an intra-abdominal abscess is rare and may delay diagnosis. We report a 25-year-old male who presented with 8 days of upper abdominal pain and 4 days of progressive abdominal distension. Despite marked abdominal distension, he had no signs of peritonitis. Laboratory investigations showed marked leukocytosis. Chest radiography showed no classical subdiaphragmatic free air, whereas computed tomography confirmed pneumoperitoneum. Emergency exploratory laparotomy revealed an intra-abdominal abscess with dense
