Littré's hernia within a recurrent inguinal hernia sac: an unusual cause of acute abdomen in the elderly-a case report.

Littré's hernia is defined as the protrusion of a Meckel's diverticulum through a hernia defect; it represents fewer than 0.1% of complicated hernias, and its presentation as a recurrent hernia in the elderly is exceptional. We report an 81-year-old man with a previous open right inguinal herniorrhaphy who presented with abdominal pain, vomiting, and a non-reducible right inguinal mass consistent with intestinal obstruction. Exploratory laparotomy revealed a strangulated recurrent right inguinal hernia Nyhus IVB/European Hernia Society (EHS) L3R (lateral inguinal hernia, size 3 [>3 cm], recurr

Ketoacidosis with SGLT2 inhibitors in routine clinical practice of type 2 diabetes: Scandinavian cohort and nested case-control study.

BACKGROUND: SGLT2 inhibitors increase the risk of ketoacidosis, but data from routine clinical practice are scarce. We used nationwide registers with the aim of assessing the incidence, risk factors, and prognosis of ketoacidosis during SGLT2 inhibitor treatment in patients with type 2 diabetes. METHODS: In this cohort and nested case-control study we used data from three Scandinavian countries. In a cohort of SGLT2 inhibitor-treatment episodes among patients with type 2 diabetes aged at least 18 years in Sweden, Denmark, and Norway, we estimated ketoacidosis incidence. Using a nested case-co

Postmenopausal Uterine Torsion with Hemorrhagic Shock Mimicking a Giant Ovarian Tumor: A Case Report.

BACKGROUND: Postmenopausal uterine torsion is an extremely rare gynecological emergency that is often challenging to diagnose preoperatively because of its atypical presentation. This case is particularly noteworthy for a 10-year history of an untreated abdominal mass that rapidly evolved into a life-threatening condition, along with a striking discrepancy between the initial imaging suspicion and the final pathological diagnosis. CASE PRESENTATION: A 75-year-old Asian woman presented with chronic abdominal distension for 10 years and acutely worsening severe generalized abdominal pain for 1

Giant parasitic leiomyoma: A rare cause of acute appendicitis and bowel injury.

Parasitic fibroids are an uncommon subtype of leiomyomas that can implant on peritoneal surfaces such as the omentum or bowel serosa and may mimic malignancy on imaging. While typically asymptomatic, they can occasionally present with acute abdominal complications. We describe the case of a 37-year-old woman who presented with abdominal pain and a palpable mass. Imaging revealed a large, heterogeneously enhancing lesion with central necrosis, raising concern for malignancy. Surgical exploration identified a 17.5 × 15 cm parasitic leiomyoma adherent to the bowel and appendix, resulting in acute

Obstructed hemivagina and ipsilateral renal anomaly syndrome: a case series of 4 patients with insights into diagnosis and treatment strategies.

Obstructed hemivagina and ipsilateral renal anomaly syndrome was first identified by Purslow in 1922 as a result of abnormal embryologic development of the Müllerian (paramesonephric) and Wolffian (mesonephric) ducts. The syndrome typically presents with cyclical lower abdominal pain and abnormal vaginal discharge in postpubertal females. Although the optimal timing and method of surgical treatment remain unclear, resection of the vaginal septum is commonly performed. This article presents 4 previously asymptomatic patients who developed an acute abdomen after menarche and were treated with hy

Comparative study of transabdominal ultrasound alone versus combined transabdominal and transvaginal ultrasound in gynecological acute abdomen.

BACKGROUND: Gynecological acute abdomen is a common emergency condition requiring rapid and accurate diagnostic imaging to guide timely clinical decision-making. This study evaluated the diagnostic value of transabdominal ultrasound (TA-US), transvaginal ultrasound (TV-US), and their combination in patients with suspected gynecological acute abdomen. METHODS: This single-center retrospective study analyzed 120 patients with suspected gynecological acute abdomen, divided into three groups (n=40 each) based on the ultrasound examination received: TA-US, TV-US, and combined ultrasound. Detection

Spontaneous Uterine Cornual Rupture in the Second Trimester of Dichorionic-Diamniotic Twin Pregnancy.

Background Uterine rupture is a rare but life-threatening obstetric emergency, usually associated with labor in patients with a prior cesarean delivery. Spontaneous rupture remote from a uterine scar, particularly at the cornua in the second trimester, is exceedingly uncommon. Case We report a 30-year-old gravida 2 para 1 at 25 3/7 weeks' gestation with a dichorionic-diamniotic twin pregnancy who presented with abrupt abdominal pain and hemoperitoneum of unclear etiology. Despite reassuring imaging, progressive maternal deterioration and non-reassuring fetal status prompted emergent laparotomy

A foundation model for acute abdomen diagnosis stratification and triage on noncontrast computed tomography.

Accurate and timely diagnosis of acute abdominal emergencies remains challenging. Non-contrast computed tomography (NCCT) is often used as an initial imaging modality because of atypical presentations, contraindications to contrast, or resource constraints. Here we present AbdomenNet, a multi-task AI system built on a self-supervised foundation model that detects 11 acute abdominal conditions and performs three risk-stratification subtasks from NCCT images. AbdomenNet is pre-trained on 103,989 NCCT examinations and fine-tuned on 5816 annotated cases. We assess generalizability in 2528 patients

Dermoid Cyst With Adnexal Torsion Presenting as Acute Appendicitis in a Young Premenstrual Patient.

Adnexal masses are relatively frequent in adults but also present in prepubertal females. Mature ovarian teratomas, also called dermoid cysts, are slowly growing disorders that could result in adnexal torsion (AT), rupture, or hemorrhage. In these cases, low abdominal pain or acute abdomen are the commonest symptoms, leading the patient to the medical emergency unit. In adolescence, early diagnosis of this pathology is not uncommon since it can be disclosed in a routine gynecological examination. Unfortunately, in young premenarchal females, routine examination or a screening program for dermo

Case Report: Pediatric chylous ascites beyond congenital malformations-infectious causes and nutritional management with a literature review.

Chylous ascites (CA) is a condition characterized by the accumulation of lymphatic fluid in the peritoneal cavity. Although congenital malformations are the most common cause in newborns, infectious agents represent a clinically significant, potentially reversible etiology that benefits from specific therapy. Various pathogens, including bacteria, viruses, fungi, and parasites, can alter the lymphatic system and lead to the leakage of chyle into the peritoneal cavity, resulting in nutritional, immunological, and metabolic deficiencies. We describe the case of a 5-month-old infant presenting wi

Migrated biliary stent causing Meckel's diverticulitis and ileal perforation: a rare cause of acute abdomen.

Migration of plastic biliary stents is a recognized complication of endoscopic retrograde cholangiopancreatography (ERCP), although it is usually asymptomatic and self-limiting. Intestinal perforation due to stent migration is rare, and involvement of Meckel's diverticulum is exceedingly uncommon. We report a 70-year-old woman who presented with acute abdomen approximately one year after ERCP with placement of a 10F-10 cm plastic biliary stent for choledocholithiasis. Computed tomography revealed small bowel obstruction and a linear hyperdense intraluminal structure consistent with a foreign b

Fulminant peritoneal metastasis of appendiceal mucinous adenocarcinoma during pregnancy: a case report.

BACKGROUND: Appendiceal mucinous adenocarcinoma (AMA) is exceptionally rare, constituting less than 0.5% of all gastrointestinal malignancies and demonstrating an annual incidence of only 1 to 2 cases per million people. Furthermore, the majority of appendiceal adenocarcinomas are diagnosed at an advanced stage (Stage III and IV). A systematic literature review of the past 10 years revealed that only one case of aggressive appendiceal mucinous adenocarcinoma in pregnancy has been reported. CASE PRESENTATION: This case report presents a rare instance of fulminant peritoneal metastasis of append

Disseminated peritoneal leiomyomatosis incidentally diagnosed during surgery for torsion of a subserosal uterine fibroid: a case report.

BACKGROUND: Disseminated peritoneal leiomyomatosis is a rare condition characterized by the dissemination of histologically benign leiomyomas within the peritoneal and retroperitoneal cavities. Intra-abdominal findings can resemble the peritoneal dissemination of gynecologic or gastrointestinal malignancies. Disseminated peritoneal leiomyomatosis has been predominantly reported in patients with a history of gynecological treatment or during pregnancy. Herein, we present a case of disseminated peritoneal leiomyomatosis diagnosed incidentally following the torsion of a uterine fibroid in a patie

Ultrasonography for Band-Induced Small-Bowel Obstruction Associated With an Omphalomesenteric Duct Cyst in a Child: A Case Report.

Omphalomesenteric duct cysts are uncommon congenital anomalies and may present with nonspecific abdominal symptoms in children. We report a 6-year-old girl who presented with acute periumbilical pain and recurrent vomiting. Ultrasonography revealed an infraumbilical cystic lesion, dilated small-bowel loops, and a band-like echogenic structure that appeared to extend from the inferior aspect of the cyst toward an adjacent dilated bowel loop. Computed tomography confirmed small-bowel obstruction and provided a broader cross-sectional view of the infraumbilical cyst and the posterior course of th

The Dilemma of Stump Appendicitis: A Case Report.

Stump appendicitis is an uncommon but clinically significant delayed complication of appendectomy, arising from inflammation of remnant appendiceal tissue after the initial procedure. Because most clinicians do not consider appendicitis in a post-appendectomy patient, stump appendicitis frequently results in diagnostic delay and increased morbidity. We report the case of a 27-year-old man from a northern state of India who presented with features of generalized peritonitis one year after undergoing open appendectomy for acute appendicitis. Radiological evaluation suggested inflammation but fai

Occult fish bone ingestion inducing cross-site injury: a rare case report of gastric bleeding and jejunal perforation.

BACKGROUND: Fish bone ingestion is a common clinical problem. However, occult ingestion causing concurrent gastric bleeding and jejunal perforation is extremely rare and easily misdiagnosed. CASE PRESENTATION: A 72-year-old male presented with upper abdominal pain and hematemesis with no clear history of foreign body ingestion. Abdominal CT revealed a high-density shadow in the stomach, localized gas-fluid accumulation in the small intestine, and minimal surrounding exudation. Emergency gastroscopy failed to identify the source of bleeding. The patient subsequently developed peritonitis, and
