Respiratory effects of rectus sheath block in patients undergoing major upper abdominal surgery: a randomized controlled trial.

BACKGROUND: Postoperative pulmonary complications remain a major concern after upper abdominal surgery and are often exacerbated by either inadequate pain control or opioid-based analgesia. We evaluated the analgesic efficacy and respiratory effects of ultrasound-guided RSB in patients undergoing upper abdominal surgery. METHODS: In this prospective, randomized, controlled trial, 60 patients aged 18–62 years, with ASA I or II status, who underwent upper abdominal surgery under general anesthesia, were enrolled and divided into two groups (30 patients each). RSB Group received ultrasound-guided

Central sensitization in patients with chronic pelvic pain resistant to hormonal therapy: A cross-sectional study of prevalence and clinical correlates in a tertiary referral cohort.

INTRODUCTION: Chronic pelvic pain (CPP) affects over 25% of women, with endometriosis being a predominant cause. Central sensitization, particularly pelvic and perineal pain central sensitization (PPCS), is increasingly recognized as a key mechanism in CPP, yet its prevalence and clinical implications across the broader CPP population are underexplored. MATERIAL AND METHODS: The aim was to determine the prevalence of PPCS among patients with CPP unresponsive to hormonal therapy and to assess its impact on symptomatology, analgesic consumption, and quality of life. This retrospective study inc

Pudendal Nerve Entrapment Within Alcock's Canal as a Compartment-Like Syndrome: A Systematic Review.

Pudendal nerve entrapment (PNE) within Alcock's canal is an under-recognized cause of chronic pelvic and perineal pain, increasingly conceptualized as a compartment-like syndrome due to sustained neurovascular compression within a confined fascial space. This systematic review evaluates the anatomical, pathophysiological, diagnostic, and therapeutic evidence supporting this hypothesis. A comprehensive literature search was conducted across PubMed, Embase, Scopus, and the Cochrane Library from April 2026 to May 1, 2026, in accordance with Preferred Reporting Items for Systematic Reviews and Met

Partial left hepatectomy in a young patient with ruptured hepatic adenoma: A case report.

BACKGROUND: Although hepatic adenomas (HA) account for less than 0.04% of all liver neoplasms, their occurrence in young women is rare but strongly associated with oral contraceptive use, particularly in the 20-40 years age group, with an annual incidence estimated at approximately 3 to 4 cases per 100000 population. CASE SUMMARY: We report the case of a previously healthy 30-year-old female patient, a regular user of combined oral contraceptives, who presented with acute hemorrhagic abdomen due to spontaneous rupture of a HA. The clinical presentation occurred after physical exertion and was

Surgical Correction of Severe Dystrophic Cervical Kyphosis With Intraoperative Motor-Evoked Potential Changes in a Pediatric Patient With Neurofibromatosis Type 1.

AIM: Uterine rupture during pregnancy is a rare but life threatening event in Obstetrics, with potentially catastrophic consequences for both the fetus and the mother. There are few published case reports that investigate the possible association between long-term steroid treatment and uterine rupture during the antenatal period. CASE REPORT: A 33-year-old G2P1 woman with obstetrical history of one previous transverse low-segment caesarean section presented at the 30th week of gestation with severe abdominal pain which started spontaneously one hour before. She had medical history of pemphigu

Acute Abdominal Pain in Pregnancy Revealing a Wandering Spleen With Infarction: A Case Report.

INTRODUCTION AND IMPORTANCE: Wandering spleen (WS) is a rare condition caused by the absence or laxity of splenic suspensory ligaments, predisposing the spleen to displacement and potential complications. While torsion is the most commonly reported issue, infarction due to vascular compromise is a serious and rare complication, particularly in pregnancy. We present a case of splenic infarction in a WS during pregnancy, emphasizing diagnostic challenges and management strategies. CASE PRESENTATION: A 19-year-old primigravida at 5 months of gestation presented with progressively worsening abdom

Ruptured Ovarian Endometrioma During Pregnancy: A Case Report and Review Highlighting Clinically Occult Risk Factors.

The rupture of an ovarian endometrioma during pregnancy is a rare cause of acute abdominal pain, and its underlying risk factors are incompletely understood. We report a case of ovarian endometrioma rupture during pregnancy complicated by dense intra-abdominal adhesions and review previously reported cases to clarify potential risk factors. Twenty cases, including our case, were analyzed. Among cases with available data, 78.5% involved cysts measuring ≥6 cm, consistent with previous reports identifying large cyst size as a major risk factor. Adhesions were reported in seven cases, although mos

Ultrasonographic Diagnosis of Acute Gallbladder Torsion in a Child: A Case Report.

Gallbladder torsion is an extremely rare condition in the pediatric population. A 32-month-old boy presented with paroxysmal abdominal pain and vomiting. An initial dedicated intestinal ultrasonography yielded false-negative results, and conservative treatment failed to relieve symptoms. Repeat examinations revealed elevated inflammatory markers; extended abdominal ultrasonography identified an enlarged and twisted gallbladder with wall thickening and absent mural vascularity. Emergency surgery confirmed 360° gallbladder torsion with ischemic necrosis, and laparoscopic cholecystectomy was perf

Promoting intergroup inclusion through literary discussions in fourth and fifth grades.

UNLABELLED: The study investigated the effects of a one-year literary program on children's intergroup attitudes toward peers with a migration background and on their perceptions of inclusive classroom norms. Fourth- and fifth-grade students and their teachers discussed four literary fiction books in small groups, with children taking the lead by asking their own questions, engaging in dialogue, and challenging one another. The books addressed topics such as intergroup exclusion, inclusive group norms, and civil courage. A total of 744 children participated in pre- and posttest assessments. Th

Spontaneous splenic rupture: a sporadic presentation of tuberculosis-a case report.

Atraumatic or spontaneous splenic rupture is a rare but life-threatening emergency. Common causes include hematologic malignancies and infections, but tuberculous involvement of the spleen is exceedingly uncommon. To our knowledge, this represents the third reported case of spontaneous rupture of a tuberculous spleen. We report a 50-year-old human immunodeficiency virus (HIV) -positive woman who presented with cough, fever, and abdominal distension, with abdominal examination suggesting peritoneal irritation and laboratory tests showing severe anemia. Emergency laparotomy revealed a ruptured s

Multifocal small bowel gastrointestinal stromal tumor causing perforation peritonitis: A rare cause of acute abdomen.

A female patient in late 40s presented with severe abdominal pain, distension and fever for a week. Ultrasound abdomen revealed ascites and a large indeterminate mass in the pelvis, but bowel loops were not dilated. Contrast-enhanced CT (CECT) abdomen revealed a large, exophytic, heterogeneously enhancing intraperitoneal mass, arising from a jejunal loop in the pelvis; showing fistulous communication with an adjacent small bowel loop. Multiple smaller enhancing rounded, serosal masses were also seen in the jejunum and ileum along with pneumoperitoneum, moderate ascites and peritoneal enhanceme

Isolated Ovarian Cyst Rupture Causing Hemoperitoneum After High-Energy Blunt Abdominal Trauma: A Case Report.

Hemoperitoneum after high-energy blunt abdominal trauma is most commonly caused by solid organ injuries, particularly involving the liver and spleen, while isolated gynecological sources are rare and may be overlooked during initial evaluation. We report the case of a 23-year-old woman who presented after a fall from the fourth floor with hemodynamic instability and signs of an acute abdomen. Imaging revealed massive hemoperitoneum without evidence of solid organ injury, along with extensive pelvic fractures. Emergency exploratory laparotomy was performed due to persistent instability, and no

Solid-Pseudo papillary Tumor of the Pancreas (Frantz Tumor): Case Report of a Rare Pancreatic Tumor in Ethiopia.

Solid-pseudopapillary tumor (SPT) of the pancreas is an extremely rare neoplasm in children, with an unknown origin. It accounts for 8% to 12% of primary pancreatic tumors and predominantly affects young females (91%), with very few cases reported in males. Malignant potential is minimal, particularly in adolescents. We report a 12-year-old child presenting with abdominal distension, nausea, vomiting, appetite loss, weight loss, pruritus, jaundice, dark urine, and back pain. Examination revealed a distended abdomen moving with respiration and a firm, tender 15 cm × 20 cm mass in the right uppe

Atraumatic Splenic Rupture: A Case Report.

Atraumatic splenic rupture (ASR) is a rare but potentially fatal emergency, accounting for less than 0.5% of all splenic ruptures. Its nonspecific presentation often complicates diagnosis and management. True idiopathic ASR, occurring without an underlying splenic disease, is extremely rare. We report the case of a 61-year-old woman with diffuse large B-cell lymphoma in complete metabolic remission who developed sudden, severe epigastric and left upper quadrant pain 7 days after receiving high-dose methotrexate for central nervous system prophylaxis. There was no evidence of splenic involvemen

Upper arm administration is associated with higher injection site pain with romosozumab: a randomized controlled trial and a self-controlled study.

These 2 prospective studies investigated whether the injection site influences pain and local reactions with romosozumab treatment for osteoporosis. In Study 1, 169 patients were randomly assigned to receive monthly romosozumab 210 mg (2 vials) injections for 12 mo in either the abdomen (Abd group) or upper arm (Arm group). In Study 2, 55 patients received 105 mg (1 vial) injections in both sites to directly compare pain at each location. Pain was evaluated using a Visual Analog Scale (VAS, mm), and secondary outcomes included BMD, bone turnover markers (BTMs) (total P1NP and TRACP-5b), and in

Laparoscopic Management of Jejunal Perforation Caused by Toothpick Ingestion: A Case Report.

Foreign body ingestion is a frequent clinical problem, although most ingested objects pass spontaneously through the gastrointestinal tract. Sharp and elongated foreign bodies, such as toothpicks, are associated with a higher risk of perforation and may present diagnostic challenges due to their radiolucent nature. We report the case of a 62-year-old man who presented with a four-day history of abdominal pain, fever, nausea, and signs of peritoneal irritation. Laboratory tests revealed leukocytosis and elevated inflammatory markers. Contrast-enhanced abdominal computed tomography demonstrated

Dysgerminoma of the ovary presenting as acute abdomen with intratumoural gas image: a diagnostic challenge suspected as a gastrointestinal stromal tumour.

A woman in her 20s presented with acute abdomen, manifesting as a palpable abdominal mass, left lower quadrant pain and vomiting. Imaging revealed a large pelvic mass containing multiple linear air pockets, which complicated the differentiation from gastrointestinal tumours (such as gastrointestinal stromal tumour) or abscesses. Emergency surgery revealed a left ovarian tumour with 360-degree torsion and associated congested change. Pathological diagnosis confirmed dysgerminoma, characterised by positive immunohistochemistry for Sal-like protein 4, placental alkaline phosphatase and octamer-bi
