BACKGROUND: Emergency General Surgery (EGS) patients are clinically complex and frequently underreported, particularly in Latin America, which hampers benchmarking and quality-improvement efforts. We implemented a hospital-based structured data collection system to characterize EGS patients and outcomes in a tertiary university hospital.
METHODS: We conducted a retrospective cross-sectional study of a prospectively collected database including all patients aged ≥ 13 years who required emergency general surgery between May and November 2020 at a high-complexity public university hospital in Co

INTRODUCTION: Spontaneous mesenteric hemorrhage (SMH) is a rare and potentially life-threatening cause of acute abdominal pain. It is defined as bleeding into the mesentery in the absence of trauma or iatrogenic injury. Diagnosis is challenging due to nonspecific clinical presentation and imaging findings that may mimic bowel ischemia or neoplasia.
CASE PRESENTATION: A 45-year-old man, known for heavy smoking and untreated hypertension, presented with acute diffuse abdominal pain following a forceful sneezing episode. Physical examination revealed localized peritoneal signs. Contrast-enhanced

The purpose of this study is to develop and retrospectively validate a deep learning system for the classification and anatomically interpretable localization of six acute abdominal emergencies on CT using multi-window Hounsfield Unit (HU) encoding. A publicly available national teleradiology dataset of 1274 patients (42,922 bounding box annotations) was used for training and internal validation (896/189/189 patient-level split). Each CT slice was encoded into three diagnostic HU windows (soft tissue, bone/stone, angio/liver). A YOLOv11-Large model with a stride-4 (P2) head was trained at 1280

Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of non-Hodgkin lymphoma; however, primary involvement of the small intestine is uncommon. Acute bowel perforation is a rare and life-threatening presentation that may mimic more common causes of acute abdomen. We report the case of a 78-year-old woman who presented with worsening abdominal pain, nausea, vomiting, and obstipation. Computed tomography demonstrated pneumoperitoneum concerning for hollow viscus perforation. Exploratory laparotomy revealed a perforated terminal ileal mass, and segmental small bowel resection was perfo

BACKGROUND: The role of immunometabolic dysregulation in reflux esophagitis (RE), particularly the metabolic drivers of pathogenic T helper 17 (Th17) cell differentiation, is not fully understood. Although Xuanfu Daizhe Decoction (XDD) is a clinically effective Traditional Chinese Medicine (TCM) formula for RE, its potential to modulate these immunometabolic pathways remains unelucidated.
OBJECTIVE: This study aimed to investigate the mechanism by which tryptophan metabolic dysregulation in RE influences Th17 cell differentiation and to evaluate whether XDD exerts therapeutic effects by targe

BACKGROUND: Uterine torsion during pregnancy is a relatively rare obstetric emergency that can occur in all stages of pregnancy. Among all kinds of acute abdomen during pregnancy, uterine torsion during pregnancy is a special type that is rare and poses a significant threat to the safety of mothers and infants. Failure to recognize clinical signs promptly and the absence of established diagnostic and treatment protocols represent persistent difficulties in managing obstetric emergencies.
CASE PRESENTATION: This study presents a case of uterine torsion caused by a giant uterine leiomyoma durin

Acute appendicitis is one of the most common causes of an acute abdomen. Early recognition of the condition and prompt operation has been the most important factors in reducing morbidity and mortality. On the other hand, removing a normal appendix is a burden both on patients and health resources. Several scoring systems have been used to aid in early diagnosis of acute appendicitis and its prompt management. Recently, the Alvarado, the acute inflammatory response (AIR) and the Raja Isteri Pengiran Anak Saleha Appendicitis (RIPASA) scoring systems have been the more common clinical diagnostic

Acute pancreatitis (AP), among the most common causes of acute abdomen, is characterized by persistent left upper abdominal pain and vomiting, without pain relief after vomiting. Its pathological features include abnormal activation of pancreatic enzymes and induction of pancreatic autodigestion by various etiologies. Emerging evidence indicates a strong association between the gut microbiota and AP progression, primarily mediated by intestinal barrier disruption, bacterial translocation, and immune dysregulation. Alterations in the gut microbiota, including overgrowth of pathogenic bacteria (

BACKGROUND: Sepsis-induced acute lung injury (ALI) is characterized by edema resulting from increased vascular permeability. Transient receptor potential vanilloid 4 (TRPV4) interacts with inositol 1,4,5-trisphosphate receptor type 1 (IP3R-1) through calmodulin-binding domains and regulates vascular permeability. However, the specific mechanisms underlying the roles of TRPV4 and IP3R-1 in endothelial pyroptosis and vascular permeability remain unclear.
METHODS: EMPs were measured in septic patients and controls, and co-cultured with human pulmonary microvascular endothelial cells (HPMECs). LP

Acute abdomen is a potentially life-threatening condition requiring prompt evaluation and surgical intervention. Symptomatic Meckel's diverticulum in adults is uncommon and may mimic other intra-abdominal pathologies. We report a 25-year-old man presenting with diffuse abdominal pain, nausea, diarrhea, and signs of peritoneal irritation suggestive of complicated appendicitis. Exploratory laparotomy revealed axial torsion of a Meckel's diverticulum with irreversible vascular compromise caused by a fibrous band. Diverticulectomy with wedge resection was performed, resulting in complete recovery.

BACKGROUND: Gastrointestinal bleeding after coronary artery bypass grafting (GIBCG) is a serious postoperative complication linked to increased mortality. The stress hyperglycemia ratio (SHR) has been recognized as a reliable predictor of adverse clinical outcomes, particularly in patients with cardiovascular disease. This study aimed to investigate the association between SHR and both GIBCG and in-hospital mortality.
METHODS: Patients were recruited from four medical centers and the Medical Information Mart for Intensive Care IV, with a total of 16,440 individuals included in the final analy

Urachal cysts are rare congenital anomalies in adults and are often identified incidentally during imaging studies or surgery because most remain asymptomatic. They represent the most frequent subtype of urachal anomaly. The simultaneous occurrence of acute appendicitis and a urachal cyst is exceptionally rare, with only a handful of cases reported in the literature. We describe the case of a 49-year-old man with a history of heart failure and substance use who presented with acute abdominal pain. Computed tomography (CT) demonstrated acute appendicitis and incidentally revealed a large cystic

BACKGROUND: Acute appendicitis remains one of the most common causes of acute abdomen and the leading indication for emergency abdominal surgery worldwide. Despite advances in imaging and clinical scoring systems, predicting disease severity and postoperative outcomes at the time of admission remains challenging. Early identification of patients at risk for complicated appendicitis or prolonged hospitalization may improve treatment planning and optimize resource utilization. This study evaluated the predictive value of routinely available inflammatory markers for complicated appendicitis and p

Point of care ultrasonography (POCUS) is being increasingly employed in the emergency room as well as in intensive care units (ICUs) to guide rapid management of sick children while avoiding shifting them away from their primary treating physician. The examination must be targeted, with a checklist approach based on clinical signs, to enable both speed and accuracy. This review brings forth some less explored applications of POCUS, in order to expand its scope. The first section deals with thoracic applications, including detailed evaluation of pleural collections and diaphragmatic motility. T

Supraumbilical hernias are an uncommon cause of pediatric small bowel obstruction and may remain clinically occult until acute incarceration occurs. The absence of a visible or palpable abdominal wall defect can delay diagnosis, making early recognition of bowel obstruction essential to prevent ischemic complications. We report the case of a 27-month-old previously healthy girl who presented with a one-day history of progressively worsening abdominal pain and eight episodes of non-bilious vomiting following a recent parainfluenza infection. Physical examination demonstrated mild abdominal dist

Jejunal diverticulitis is a rare and often under-recognized cause of acute abdominal pain, frequently presenting with non-specific clinical, biochemical, and radiological findings. A 52-year-old female presented with acute abdominal pain and elevated inflammatory markers, but non-diagnostic imaging. Despite biochemical improvement with conservative management, her symptoms persisted. Diagnostic laparoscopy revealed jejunal diverticulitis with an incidental Meckel's diverticulum requiring dual segmental small bowel resection. The postoperative course was uneventful. This case highlights the lim
