Four immune-compromised children who were receiving antineoplastic chemotherapy (three for leukemia), presented with recurrent episodes of fever and left upper abdominal pain. Chlamydia trachomatis infection in the lower part of the genital tract of young girls and women may ascend to produce endometritis, salpingitis, perihepatitis, and other localized or generalized abdominal diseases. The slipping rib syndrome is characterized by upper abdominal pain due to irritation of the intercostal nerve by incomplete dislocation of the costal cartilage of the 8th, 9th, or 10th ribs. Twenty-three new cases are reported and the literature is reviewed. Diagnosis is only clinical. The pain is precipitated by movement and certain postures; it is faithfully reproduced by pressure on one particular point of the costal margin and is relieved by local injection of an anaesthetic. Pain from a slipping rib is usually attributed to visceral causes, which is a source of diagnostic errors.

The effects of intraperitoneal administration of bupivacaine on pain and the sympathoadrenal response to surgery were studied in a double-blind randomized trial in 19 patients undergoing cholecystectomy. Bupivacaine (2 mg/kg) was dissolved in 300 ml isotonic saline and administered into the peritoneal cavity 10 min before the operation (n = 9). A variety of benign liver tumors associated with the use of oral contraceptives has been described. However, there is controversy regarding the possible relation of focal nodular hyperplasia of the liver to oral contraceptive therapy. Neurolytic celiac plexus block is the therapy of choice for visceral upper abdominal pain that is resistant to therapy. In order to ensure that the treatment is indicated a temporary block for diagnostic purposes (application of a local anesthetic) has to be carried out. Ultrasonographic (US) examinations of the upper abdomen were performed in 80 patients with Gaucher disease. Of the 49 patients that had not undergone splenectomy, 47 had splenic enlargement. Twelve cases of liver abscess in children and adolescents presenting at Children's Hospital at Los Angeles from 1974 to 1983 were reviewed. Most occurred in children younger than 5 years of age. Upper gastrointestinal fiberendoscopy in pediatric patients is done safely and under local anesthesia in most instances. This study of 47 children confirmed the value of fiberendoscopy in establishing the etiology of upper gastrointestinal hemorrhage and the presence of esophageal varices. It also contributed significantly to the management of patients with disphagia, pyrosis, epigastric pain, and ingestion of foreign bodies. No significant morbidity was caused.

Over a period of 3 years acute perihepatitis has been diagnosed in 7 women. Clinically the pain was localized to the right upper abdomen in 4, whereas 1 patient complained of bilateral upper quadrant pain; 2 patients presented with exclusively left-sided upper abdominal symptoms. In the literature perihepatitis is characterized clinically as a syndrome limited to the right upper abdominal quadrant. However, our own experience shows that this disease entity must also be considered in the differential diagnosis of left-sided upper abdominal pain in young female patients.

A survey of 46 patients with the slipping-rib syndrome shows that it is a condition which affects mainly middle-aged people. It is equally common in men and women. The main symptom is upper abdominal pain, equally common on either side and occasionally bilateral. The pain is precipitated by movement and certain postures and is faithfully reproduced by pressure at one or more points on the costal margin. The cause of the "slipping rib" is not known. Symptoms are relieved by reassurance and in some cases by injection of local anaesthetic into painful sites.

Five cases of retroperitoneal iliac fossa abscess are presented. Abnormal excretory urogram and plain films, coupled with a history of either hip pain, limp, or pelvic mass should suggest the diagnosis. The excretory urogram is the most sensitive means of detecting iliopsoas abscess; all patients had medial displacement on the distal ureter. The upper urinary tract was normal in all cases. Study of the gastrointestinal tract may be of additional help in localizing the abscess and ruling out other pathology. Fiberoptic gastrointestinal endoscopy was performed on 52 patients between the ages of 2 months and 16 years. The procedures were safely and easily done with premedications consisting of meperidine, promethazine, and chlorpromazine, and atropine, with diazepam given at the time of the procedure. We present a 14 year old girl with a two years' history of colicky abdominal pain associated with the palpation of a tumor in the left upper quadrant of the abdomen.
