Examination of pulses of the upper part of the body is important not only in localizing disease but also in indicating diffuse disease. Although described separatley in this article, the examination of the arterial system can be integrated in a complete physical examination when the examiner is assessing the extremities, the abdomen, and the cardiovascular system.

8 patients with carcinoma of the hepatic duct are presented. Despite symptoms during the long anicteric period (2-13 months, x 15 months) the diagnosis was never established until the patients became jaundiced. Early in the course of the diseases all of them reported upper abdominal pain and intermittend diarrhea, and the test for occult blood in the stool was positive. Initially there may be no biochemical evidence of cholestasis. Liver biopsy may at an early stage reveal pseudoxanthoma cells as an index of long-lived, local mechanical cholestasis.

From 1948 to 1973, intercostal nerve block was used 10,941 times or a total of approximately 100,000 individual nerves were blocked. Junior staff (residents) performed 95% of the blocks. The local anaesthetic solution of choice used 0.25 or 0.5% bupivacaine (Marcaine) with adrenaline. Total milligram doses of bupivacaine as high as 400 mg were employed. The duration of the blocks was 9-18 hr. No severe systemic toxic reactions occurred, e.g. disorientation, convulsions, etc. The incidence of pneumothorax was 0.073%.

During a 23 year period at Memorial Hospital, the diagnosis of liver cell carcinoma was made in 42 patients who were 11 to 40 years old. Ninety per cent were Caucasian, mostly born in the United states. No occupational hazard was detected. Serum hepatitis antigen was demonstrated in only one patient. Alpha fetoprotein was found in the serum of 55 per cent of nine patients tested. Eight-three per cent were Rh positive, 43 per cent were ABO groups, A or O, respectively.
