Re-modified thoracoabdominal nerves block through the perichondrial approach at the 9th intercostal space provides analgesia in the upper abdomen: a volunteer study.

We previously reported that administering local anesthetic into the space between the endothoracic fascia, diaphragm, and costodiaphragmatic recess (SEDIC) at the 10th intercostal space via the re-modified thoracoabdominal nerves block through the perichondrial approach (RM-TAPA) effectively anesthetizes the lower abdomen (T9-T12). However, the analgesic area obtained by RM-TAPA was distinct from prior reports of M-TAPA, which achieved broader analgesic effects from T4 to L1. The aim of this study was to verify the range of analgesic effects when local anesthetics are administered into the SED

Isolated Splenic Infarction: An Atypical Presentation Of Infectious Mononucleosis.

Splenic infarction, being one of the rare and serious complications of infectious mononucleosis, occurs due to splenomegaly and local vascular congestion, sometimes accompanied by transient hypercoagulability. However, it is usually not the presenting complaint. We present the case of a 24-year-old obese male with a past medical history of sleep apnea who presented to our ED with a complaint of moderate to severe left upper quadrant pain. His abdominal pain had continued intermittently for two weeks and progressively worsened, prompting him to come to the hospital. Left upper quadrant tenderne

A Case Report of Falciform Ligament Appendagitis.

Falciform ligament appendagitis is an extremely rare form of intra-abdominal focal fat infarction. It usually presents with vigorous abdominal pain and mimics other more common acute abdominal pain-associated diseases. Better recognition of this entity avoids misdiagnoses and unnecessary surgical treatment. We present the case of a 73-year-old woman admitted to the emergency department for abdominal pain, nausea, and vomiting. She had a fever and a diffuse tender abdomen with upper right quadrant pain. Laboratory investigation showed leukocytosis and high C-reactive protein. CT revealed a hete

Pedunculated focal nodular hyperplasia of the liver in a healthy child born following in vitro fertilization: a case report and review of the literature.

BACKGROUND: Focal nodular hyperplasia is a common nonmalignant liver mass. This nonvascular lesion is an uncommon mass in children, especially those with no predisposing factors, namely radiation, chemotherapy, and hematopoietic stem cell therapy. Exophytic growth of the lesion further than the liver margins is not common and can complicate the diagnosis of the lesion. This report observes a focal nodular hyperplasia as a pedunculated lesion in a healthy child. CASE PRESENTATION: We describe a 9-year-old healthy Persian child who was born following in vitro fertilization complaining of abdomi

Port-Site Metastasis After Laparoscopic Gastrectomy Extending to the Thigh: A Case Report.

Port-site metastasis (PSM) is rare following laparoscopic gastrectomy for gastric cancer. Previous reports focused on localized lesions treated with excision; contrastingly, case reports describing extensive invasion into the lower extremity skeletal muscles causing deterioration in activities of daily living are nonexistent. A 55-year-old male underwent a laparoscopic distal gastrectomy for gastric cancer. The pathological findings revealed a stage IIIA tumor. Two years later, skin hardening was observed on the left upper abdominal wall. Computed tomography displayed a 13-cm-long, flat tumor

Total Iliocaval Reconstruction in a Complex Palliative Patient with Malignant Inferior Vena Cava Syndrome.

Inferior vena cava (IVC) compression secondary to mass effect is accompanied by edema, ascites, back and abdominal pain, and central nervous system symptoms. Most IVC syndrome cases described in the literature focus on the focal treatment of IVC lesions, and reports of complete iliocaval reconstructions secondary to malignant IVC syndrome in the palliative context are limited. In this case report, we describe the clinical presentation, technical approach, and symptomatic outcomes of a patient with extensive malignant compression and invasion of the iliofemoral venous system. Postoperative Analgesia With Modified Thoracoabdominal Nerve Block Through Perichondrial Approach in Neonatal and Infantile Abdominal Surgery.

Modified thoracoabdominal nerve block through the perichondrial approach (M-TAPA) is a novel strategy for peripheral nerve block in the abdomen. Its usefulness has been highlighted in adults, but no literature is currently available regarding its efficacy in infants. This report describes the cases of a one-day-old neonate in open abdominal surgery and a one-month-old infant in laparoscopic surgery who received M-TAPA. The postoperative condition of the infants was assessed through a neonate pain scale and the Face, Legs, Activity, Cry, and Consolability behavioral scale, respectively; both sc

Fatal abdominal hemorrhage following surgery to remove a retroperitoneal MPNST associated with NF1: A case report.

RATIONALE: Individuals diagnosed with neurofibromatosis type I (NF1) commonly present with neurofibromas, and a subset may progress to develop malignant peripheral nerve sheath tumors (MPNST) during their lifetime. Diagnosing and treating MPNST, typically linked to NF1, poses challenges for clinicians due to its histopathological complexity. PATIENT CONCERNS: A 25-year-old male presented with postprandial discomfort and vomiting, subsequently developing left mid-abdominal pain. DIAGNOSES: The patient was admitted to the hospital, where a low-density retroperitoneal mass was detected via comp

Wandering spleen with torsion causing an acute abdomen: A case report.

INTRODUCTION AND IMPORTANCE: Wandering spleen, also known as ectopic spleen, is an uncommon disorder in which the spleen's anatomical location differs from its fixed position in the abdomen's left upper quadrant. The etiology is either congenital or acquired, possibly leading to torsion and splenic infarction. It affects children and young adults, especially childbearing-age women. Patients affected by this condition may present with nonspecific symptoms requiring a high index of suspicion. Given the nonspecific clinical symptoms and the potential complications associated with wandering spleen

A bird-eye view of diaphragmatic endometriosis: current practices and future perspectives.

Diaphragmatic endometriosis is one of the most common localization of extra-pelvic endometriosis and may cause debilitating symptoms such as cyclic shoulder pain, right upper abdominal pain, and right-sided chest pain. Diaphragmatic endometriosis may also be asymptomatic. The exact mechanisms by which diaphragmatic endometriosis originates are unknown. The high correlation between severe pelvic endometriosis and diaphragmatic endometriosis suggests that the latter originates from endometriotic cells that reach the upper abdomen by circulating with the peritoneal fluid current. Robust evidence

Percutaneous Ultrasound Guided Celiac Plexus Approach: Results in a Pig Cadaveric Model.

Celiac plexus (CP) block (CPB) and neurolysis (CPN) are interventional techniques employed in human analgesia to control visceral pain originating from the upper abdomen. Visceral pain is common in animals and its treatment is challenging. A percutaneous ultrasound (US)-guided approach to the CP has been reported in people but not in veterinary species. The objective of this study is to describe a US-guided percutaneous approach to the CP in a porcine cadaveric model. Cadavers were positioned in right lateral recumbency. The vertebral body of the last thoracic vertebra (T15) was identified (in

Modified thoracoabdominal nerves block through perichondrial approach for surgical patients: a scoping review.

BACKGROUND: Modified thoracoabdominal nerves block through perichondrial approach (M-TAPA) is a trunk block that has been gaining attention for managing postoperative pain following abdominal surgeries since its first report in 2019. We conducted a scoping review on M-TAPA, aiming to comprehensively evaluate existing research, identify the gaps in knowledge, and understand the implications of M-TAPA. METHODS: This scoping review was conducted using databases including PubMed, Embase, Cochrane, and CINAHL to evaluate the clinical efficacy of M-TAPA on April 19, 2024. Background and outcomes in

A Diagnostic Dilemma of Emphysematous Liver Abscess with Gas under the Diaphragm: A Case Report and Review of Literature.

A 53-year-old woman presented to the surgical emergency with complaints of high-grade fever accompanied by chills for 15 days and pain in the right upper abdomen for 10 days. X-ray of the abdomen and chest X-ray revealed free gas under the right hemidiaphragm. As there were no signs of generalised or localised peritonitis, emergency laparotomy was postponed. Subsequently, a contrast-enhanced computerised tomography scan of the abdomen suggested an emphysematous liver abscess, which was managed by ultrasonography-guided drainage of the abscess cavity.
