Pancreatic Tuberculosis Mimicking Pancreatic Cancer in a Young Immunocompetent Man: A Case Report and Literature Review.

BACKGROUND: Pancreatic TB is extremely rare, even in endemic areas. Clinically and radiographically, it may resemble a malignant process. Herein, we present a case of pancreatic TB who underwent surgery with a preoperative diagnosis of pancreatic malignancy. CASE PRESENTATION: A 34-year-old Afghan man presented with a one-year history of intermittent epigastric pain, accompanied by nausea and vomiting. Physical examination showed stable vital signs with mild epigastric tenderness. Laboratory examinations revealed mild anemia with mildly increased liver enzymes and lipase. The CA19-9 level in

Opioid-free versus opioid-based anesthesia for robot-assisted radical prostatectomy: a single-center randomized controlled trial.

Multimodal, opioid-sparing analgesia is standard in urological surgery, and there is additional interest in eliminating intraoperative opioids in prostate cancer surgery. Randomized evidence for opioid-free anesthesia (OFA) in robot-assisted radical prostatectomy (RARP) is lacking. In this single-center randomized controlled trial, adult men undergoing RARP were allocated 1:1 to OFA (dexmedetomidine plus ketamine, no intraoperative opioids) or a remifentanil-based technique. Both groups received identical multimodal premedication, a bilateral quadratus lumborum block, sevoflurane maintenance a

Safety and Recovery Profile of Propofol-Based Intravenous Sedation for Endoscopic Retrograde Cholangiopancreatography in Adults: A Prospective Single-Center Observational Study.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) frequently requires deep sedation, but prone positioning, restricted airway access, and variable procedural duration create important safety concerns. We evaluated respiratory and hemodynamic events, recovery, and satisfaction during propofol-based intravenous sedation. METHODS: This prospective, single-center observational study included 100 adults with American Society of Anesthesiologists physical status I or II who underwent elective ERCP for common bile duct stones at King Khalid Hospital in Saudi Arabia from June 2025 to

EEG-Derived Pain Threshold Index-Guided Remifentanil Titration During Laparoscopic Abdominal Surgery: A Randomized Trial.

OBJECTIVE: To determine whether remifentanil titration guided by the pain threshold index (PTi), an electroencephalography-derived proxy for nociceptive tolerance rather than subjective pain, reduces intraoperative remifentanil infusion during laparoscopic abdominal surgery. METHODS: In this prospective, single-center, assessor-blinded randomized trial, adults undergoing elective laparoscopic abdominal surgery with propofol-remifentanil target-controlled infusion were assigned 1:1 to PTi-guided or conventional hemodynamic-guided remifentanil titration. The primary endpoint was intraoperative

A rare case report: atypical presentation of Klinefelter syndrome in a 24-year-old male at suburban tertiary care hospital in Nepal.

Klinefelter syndrome (KS) is the most common chromosomal condition affecting males, usually caused by the presence of an extra X chromosome (47, XXY). However, it often remains undiagnosed until adulthood. We report a unique case of a 24-year-old man whose primary concern was persistent nausea and vomiting, an atypical presenting complaint of Klinefelter syndrome. Further evaluation revealed typical features of KS, including infertility despite normal sexual desire, gradual enlargement of the breasts, tall stature, sparse facial and body hair, small testes, and learning difficulties. An uncomm

Assessing the Effectiveness of Bupivacaine for Managing Pain Post-rhinoplasty: A Triple-blinded Controlled Clinical Trial.

BACKGROUND: Rhinoplasty is often accompanied by significant postoperative pain, making effective pain management crucial for enhancing patient satisfaction and facilitating recovery. This study evaluates the effectiveness of bupivacaine, a long-acting local anesthetic, in managing postoperative pain for patients undergoing rhinoplasty. METHODS: A randomized triple-blind controlled clinical trial was conducted involving 220 participants aged 17 to 50 years, classified as ASA I. Participants were randomly assigned to one of four groups: control (no bupivacaine), injection (4 ml of bupivacaine v

GLP-1 Receptor Agonists and Insulin Versus Insulin for Perioperative Glycemic Control in Patients with Type 2 Diabetes Undergoing Cardiac Surgery: A Systematic Review.

Perioperative hyperglycemia is common in patients with type 2 diabetes undergoing cardiac surgery and is linked to adverse outcomes. Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) offer glucose-dependent insulinotropic effects and potential cardiac benefits, but their perioperative role remains uncertain. This review evaluated the efficacy and safety of GLP-1 RA therapy as an adjunct to insulin in this setting. Three electronic databases (PubMed, Cochrane, and Google Scholar) were searched for relevant articles up to October 2025. The primary endpoint was blood glucose levels during the

CLDN18 and FGFR2 splicing variants are emerging tumor-associated antigens for cancer therapy.

Alternative splicing and alternative promoters generate splicing variants to expand proteomic and transcriptional diversity to orchestrate physiological activities and dictate tumorigenesis. Splicing variants are attractive tumor-associated antigens (TAAs) for the clinical development of cancer therapies. Here, recent progress in claudin-18 isoform 2 (CLDN18.2)- and fibroblast growth factor receptor 2 isoform b (FGFR2b)-targeted therapeutics is summarized, and perspectives in this field are discussed. Zolbetuximab and bemarituzumab are first-generation monoclonal antibodies (mAbs) targeting CL

Contained transverse colonic perforation caused by an ingested fish bone presenting as chronic abdominal pain: a case report.

INTRODUCTION AND IMPORTANCE: Foreign body ingestion is common; however, gastrointestinal perforation is rare and often poses a diagnostic challenge, particularly when symptoms are chronic and nonspecific. Colonic perforation due to ingested foreign bodies is uncommon, with the transverse colon being an especially rare site due to its mobility and wide lumen. CASE PRESENTATION: We report the case of a 19-year-old male who presented with central abdominal pain and intermittent diarrhea for 1 month, followed by acute worsening of abdominal pain, fever, nausea, and vomiting over the proceeding 3

Pregnancy Complicated by Citrin Deficiency and Gestational Diabetes Mellitus: Perioperative Metabolic Management and Postoperative Analgesic Challenges Following Caesarean Delivery.

Citrin deficiency is a rare autosomal recessive metabolic disorder caused by pathogenic variants in the SLC25A13 gene. Pregnancy in affected women is uncommon and presents unique challenges because conventional obstetric management may conflict with metabolic requirements. The coexistence of gestational diabetes mellitus (GDM) introduces additional complexity. A 25-year-old, gravida 2 para 0+1 woman with genetically confirmed citrin deficiency (homozygous SLC25A13 c.1763G>A mutation) was managed jointly by obstetric and inherited metabolic disease (IMD) teams. Her pregnancy was complicated by

General Anesthesia vs Monitored Anesthesia Care With Local Anesthetic Infiltration (MAC-VL) for Ankle Fracture Open Reduction and Internal Fixation: Impact on Recovery Metrics and Safety.

BACKGROUND: Monitored anesthesia care (MAC) combined with regional anesthetic techniques is commonly used for open reduction and internal fixation (ORIF) of unstable ankle fractures. When compared to general anesthesia (GA), MAC with regional anesthesia has been associated with reduced systemic adverse effects and shorter recovery times, although regional anesthesia carries additional costs and potential complications of its own. A new technique involving the infiltration of high-volume, low-concentration (VL) local anesthetic was therefore developed. When used in conjunction with MAC, we refe

Resection of a Large Intra-Abdominal Desmoid Tumor with Preoperative Superior Mesenteric Artery Balloon Catheter Placement: A Case Report.

INTRODUCTION: Large tumors occupying the abdominal cavity can hinder surgical exposure, particularly of the posterior aspect, increasing the risk of arterial bleeding. Because arterial hemostasis is difficult, hemodynamic instability may occur. This report describes the safe use of preoperative balloon catheter placement in the superior mesenteric artery (SMA), the tumor's primary feeding vessel, to control arterial bleeding through on-demand intraoperative occlusion. CASE PRESENTATION: A 40-year-old woman with a history of 2 cesarean sections presented with abdominal distension. An intra-abd

C. butyricum Combined With Resmetirom Provides Therapeutic Benefits in MASH Associated With Gut-Liver-Brain Axis Modulation.

BACKGROUND AND AIMS: Resmetirom has emerged as a major advance in MASH pharmacotherapy as the first approved liver-targeted THR-β agonist; however, variable therapeutic efficacy and adverse responses may limit its clinical benefits. This study investigated whether C. butyricum could provide additional therapeutic benefits when combined with resmetirom and attenuate nausea/vomiting-like responses in association with changes across the gut-liver-brain axis. METHODS: In this study, we established a high-fat diet-induced MASH mouse model and administered C. butyricum, resmetirom, or their combina

The relative contribution of anaesthetists and post-anaesthesia care unit nurses to patient-reported postoperative quality improvement indices.

Patient-reported outcomes in the post-anaesthesia care unit (PACU), such as pain, postoperative nausea and vomiting (PONV) and hypothermia, are often used for quality improvement (QI) purposes and to benchmark hospitals and anaesthetists. However, it is unclear whether variations in these measures reflect differences between clinicians or are driven by factors beyond clinician control. The aim of this study was to quantify how much of the variation in patient outcomes of severe pain, PONV and hypothermia in PACU, was associated with individual anaesthetists and PACU nurses, after adjusting for

Seasickness or Postoperative Nausea and Vomiting? A Retrospective Review of Patients Receiving Anesthesia at Sea Onboard the USNS Comfort.

INTRODUCTION: The USNS Comfort is a 1,000-bed hospital ship that provides full perioperative and surgical capabilities in a maritime environment. Patients may undergo surgery while the ship is pierside, anchored, or underway at sea. Postoperative nausea and vomiting (PONV) is a common anesthetic complication, but ship motion and sea state introduce an additional vestibular stimulus that is not captured by traditional PONV risk models. This study evaluated whether operating environment was associated with PONV after elective surgery aboard the USNS Comfort during a humanitarian mission. MATERI

Bilateral external oblique intercostal block combined with intravenous patient-controlled analgesia for multimodal analgesia after laparoscopic cholecystectomy: a single-center randomized controlled trial.

BACKGROUND: Postoperative pain after laparoscopic cholecystectomy (LC) remains common and peaks within 24 h. Opioid-based analgesia improves pain but can delay recovery, prompting opioid-sparing regional techniques within ERAS pathways. OBJECTIVE: To evaluate whether adding bilateral ultrasound-guided external oblique intercostal plane block (EOIB) to standard intravenous patient-controlled analgesia (PCA) improves early analgesia and recovery after elective LC. METHODS: In a prospective, single-centre, randomized controlled parallel trial, 56 patients were randomized (1:1) to receive bilate

Case Report: Diagnostic pitfalls and laparoscopic repair of a delayed large right-sided traumatic diaphragmatic hernia.

BACKGROUND: Delayed right-sided traumatic diaphragmatic hernia (TDH) may be overlooked during polytrauma assessment because concomitant injuries dominate attention and the liver can obscure diaphragmatic discontinuity. Subsequent respiratory and gastrointestinal symptoms may remain nonspecific until obstruction or visceral compromise develops. CASE DESCRIPTION: A 50-year-old man presented with more than 20 h of nausea and vomiting 19 months after severe blunt trauma. Since the injury, he had experienced intermittent dyspnea, chest tightness, postprandial fullness, abdominal distension, and ab
