Wedge Pillow Versus Evening Proton Pump Inhibitor for Nocturnal Reflux Symptoms: A Noninferiority Randomized Trial.

BACKGROUND: Certain patients still experience nocturnal reflux symptoms despite taking a standard morning PPI. This study compares the efficacy of adding a wedge pillow versus an evening PPI to the morning PPI for treating nocturnal reflux symptoms. METHODS: This noninferiority randomized trial included patients with ≥ 2/week nocturnal heartburn/regurgitation despite morning PPI use for ≥ 4 weeks. Participants were randomized to either a wedge pillow with morning or twice-daily PPI for 4 weeks. Nocturnal reflux symptoms were assessed using the nocturnal gastroesophageal reflux disease symptom

Evaluating the Frequency of Esophageal Motility Disorders in Patients With Upper Gastrointestinal Symptoms Based on the Chicago Classification Version 3.0.

INTRODUCTION: Patients with upper gastrointestinal (GI) symptoms may have esophageal motility disorders (EMDs). Esophageal high-resolution manometry (HRM) is often performed when no obvious abnormalities are observed on esophagogastroduodenoscopy (EGD). However, few reports from Japan have evaluated the relationship between symptoms and EMDs in a large number of patients from a single institution. This study aimed to elucidate the frequency of EMDs and the relationship between patient symptoms and EMDs by evaluating HRM findings based on the Chicago Classification version 3.0 (CC v3.0). METHO

A progression risk assessment model for pulmonary ground-glass nodules based on traditional Chinese medicine clinical symptoms combined with imaging.

OBJECTIVE: Progressive ground-glass nodules (GGNs) are associated with a risk of malignancy. Early identification and intervention are crucial for lung cancer prevention. Traditional Chinese medicine (TCM) has advantages of syndrome differentiation, holistic adjustment, and whole-process management. In this study, we built a risk assessment model based on the integration of TCM symptoms and imaging features to identify risks of GGN progression and to guide personalized TCM management and treatment. METHODS: This study enrolled 864 patients with GGNs from January 2015 to December 2023. Assessm

Diagnostic and Management Approach to Reflux-Related Cough.

Chronic cough, defined as cough persisting longer than 8 weeks, affects approximately 10% of the global population and significantly impairs quality of life. Gastroesophageal reflux disease is a recognized cause of chronic cough, yet the relationship remains complex and poorly understood. Up to 75% of patients with reflux-related cough lack classic gastroesophageal symptoms such as heartburn or regurgitation, making diagnosis particularly challenging. Although direct tissue irritation by refluxate has been proposed as a primary mechanism, evidence also supports a vagally mediated esophageal- t

Mid-esophageal mucosal impedance reflects mucosal integrity and phenotypes refractory reflux-like symptoms.

BACKGROUND: Refractory reflux-like symptoms (rRLS) are frequently encountered in clinical practice and often reflect heterogeneous underlying mechanisms beyond acid exposure alone. Esophageal mucosal impedance has emerged as a marker of mucosal integrity, yet its role in phenotyping rRLS remains incompletely defined. METHODS: In this retrospective study, adult patients with reflux-like symptoms who underwent multichannel intraluminal impedance-pH monitoring and high-resolution manometry were enrolled. Patients exhibiting persistent reflux-like symptoms despite ≥ 8 weeks of double-dose PPI the

Esophageal mucosal mast cell density and degranulation are increased in gastroesophageal reflux disease.

Gastroesophageal reflux disease (GERD) is a chronic condition encompassing visceral pain-experienced as heartburn and, in some cases, inflammation of the esophageal mucosa. Although mast cell dysregulation has been described in disorders of gut-brain interaction, it has been understudied in GERD. We aimed to characterize mast cell localization and degranulation in GERD esophageal mucosa. Distal esophageal biopsies were collected from healthy control subjects (HC; n = 10) and subjects with functional heartburn (FH; n = 9), nonerosive reflux disease (NERD; n = 13), and erosive reflux disease (ER

Proton Pump Inhibitors Are More Cost-Effective Than Potassium Competitive Acid Blockers for Gastroesophageal Reflux Disease.

INTRODUCTION: Potassium competitive acid blockers (PCABs) are superior to proton pump inhibitors (PPIs) for healing of Los Angeles (LA) class C/D erosive esophagitis and are approved for nonerosive gastroesophageal reflux disease (GERD) given their efficacy measured by heartburn-free days. However, they are more expensive than PPIs. We estimated the cost-effectiveness of PCABs compared with PPIs for the management of GERD. METHODS: A decision tree was constructed for the base case of a patient with GERD. We tested scenarios in which nonerosive reflux disease, LA A/B, or LA C/D esophagitis was

Pseudoachalasia due to Distal Esophageal Squamous Cell Carcinoma in an Elderly Patient: Clinical Insights.

Pseudoachalasia is a rare condition that clinically and radiologically mimics primary achalasia, most commonly secondary to an underlying malignancy. We report the case of a 79-year-old man presenting with a two-month history of progressive dysphagia to both solids and liquids, accompanied by regurgitation, retrosternal discomfort, heartburn, and marked weight loss. Physical examination revealed cachexia and severe muscle wasting. A barium swallow demonstrated esophageal dilatation with a bird-beak appearance, suggestive of achalasia. Upper endoscopy showed nodular, friable mucosa in the dista

Radiofrequency ablation reduces esophageal hypersensitivity in refractory non-erosive reflux disease.

Refractory non-erosive gastroesophageal reflux disease (rNERD) involves complex interactions between esophageal sensitivity receptors (PAR2/TRPV1) and persistent proton pump inhibitor (PPI)-refractory symptoms. This study exclusively employed radiofrequency ablation (RFA) as the uniform endoscopic intervention. All 20 enrolled rNERD patients underwent standardized RFA procedures; no participants underwent mucosal resection or ligation procedures. Post-intervention analysis demonstrated significant reductions in PAR2/TRPV1 expression (PAR2: 81.8% → 35.5%; TRPV1: 1.6 → 0.9 by Western blot) and i

The prevalence and risk factors of gastroesophageal reflux disease among medical students in southwestern Saudi Arabia.

BACKGROUND: Gastroesophageal reflux disease (GERD) is one of the most common gastrointestinal disorders. Medical students experience unique dietary and lifestyle changes due to their demanding coursework and stressful examinations. Our aim was to investigate the prevalence and risk factors of GERD among medical students in the southwestern part of Saudi Arabia. METHODS: A web-based validated GERD questionnaire (GerdQ) was used to diagnose GERD based on a score of >8. The questionnaire was distributed electronically to male and female medical students at different educational levels through a

Differences in Endoscopy Characteristics Between Providers With the Highest and Lowest Post Endoscopy Upper Gastrointestinal Cancer Rates in England.

BACKGROUND: Post-endoscopy upper gastrointestinal cancer (PEUGIC) rates vary over threefold between endoscopy providers in England. To determine if variations in endoscopy characteristics contribute, providers with the lowest and highest PEUGIC rates were compared. METHODS: Endoscopy providers were categorized into quartiles based on PEUGIC rates and those in the highest and lowest quartiles studied. Data for diagnostic upper gastrointestinal (UGI) endoscopy performed between January 2019 and February 2020 were extracted from the National Endoscopy Database. Multivariable regression analysis

Guidelines on diagnosis and management of gastroesophageal reflux disease in infants, children and adolescents: a joint consensus from Italian pediatric societies (SIP and SIGENP) -Part II: management.

BACKGROUND: Suspected gastroesophageal reflux disease (GERD) is one of the most common reasons for referral to pediatric and gastroenterology clinics, and proton pump inhibitors (PPIs) are frequently prescribed. Currently, there is still wide heterogeneity in treatment, especially in neonates and extraesophageal presentations. This article aimed to provide guidelines and a consensus of Italian experts on the management of GERD in infants, children, and adolescents, in order to improve the care and treatment of these patients. METHODS: A multidisciplinary pediatric panel identified four key cli

Development of a national root cause analysis system for post-endoscopy upper gastrointestinal cancer.

BACKGROUND AND STUDY AIMS: Post-endoscopy upper gastrointestinal cancer (PEUGIC) is a key performance indicator for endoscopy. PEUGIC represents a delay in diagnosis: a patient has an endoscopy that did not diagnose cancer, and another investigation, usually endoscopy, 3 to 36 months later that diagnoses upper gastrointestinal (UGI) cancer. We describe a national system to identify PEUGIC and undertake root cause analysis. METHODS: This retrospective national study was undertaken in the English National Health Service (NHS) and consisted of: 1) identification of PEUGIC; 2) development of an o

Effect of endoscopic pre-aponeurotic repair of diastasis recti on gastroesophageal reflux symptoms: An observational retrospective study.

BACKGROUND: Diastasis recti (DR) is a common condition, particularly in post-partum women, which may impair core function and alter intra-abdominal pressure (IAP). Gastro-esophageal reflux disease (GERD) is frequently associated with IAP imbalance, to which DR may be a contributing factor. This study aims to evaluate whether surgical correction of DR using pre-aponeurotic endoscopic repair (REPA) improves symptoms of GERD. METHODS: Symptom changes were assessed in 115 REPA patients using a validated questionnaire (modified Italian GERD Health-Related Quality of Life, MI-GERD-HRQL) administere

Functional Esophageal Disorders.

Functional esophageal disorders manifest a complex interplay between subtle physiomechanical perturbations and altered peripheral and central neuropsychological processing. These disorders present with heartburn, chest pain, globus and/or dysphagia, in the absence of structural abnormalities, esophageal motility disorders, or overt gastroesophageal reflux disease. Visceral hypersensitivity from peripheral or central neural dysfunction is thought to contribute significantly to symptom generation, potentially triggered by low-grade inflammation or compensatory responses to repetitive noxious sti
