Potassium-competitive Acid Blockers in Gastroesophageal Reflux Disease and Functional Dyspepsia: A Korean Expert Review With Original Meta-analyses.

Potassium-competitive acid blockers (P-CABs) have emerged as a potent alternative to proton pump inhibitors (PPIs) for the treatment of gastroesophageal reflux disease and functional dyspepsia (FD). This narrative review incorporating original meta-analyses summarizes the latest evidence from randomized controlled trials and meta-analyses, emphasizing the efficacy, safety, and clinical applications of P-CABs such as vonoprazan, tegoprazan, fexuprazan, keverprazan, and zastaprazan. P-CABs demonstrate rapid onset, sustained acid suppression, and superior healing rates in erosive esophagitis (EE)

Combined double-endoscopic surgery for a giant left-sided supradiaphragmatic esophageal diverticulum: a case report.

INTRODUCTION AND IMPORTANCE: Esophageal diverticulum is a rare condition, with supradiaphragmatic esophageal diverticula being even rarer, occurring in only approximately 0.06%-4% of cases, and most of these are located on the right side of the esophagus. This article reports a rare case of a giant left-sided supradiaphragmatic esophageal diverticulum, aiming to explore its diagnostic methods, surgical strategies, and therapeutic outcomes, thereby providing a reference for similar complex cases. CASE PRESENTATION: A 58-year-old male patient was found to have an esophageal diverticulum during

Diaphragmatic Breathing as an Adjunctive Approach in Gastroesophageal Reflux Disease: Narrative Review of Its Mechanistic Basis and Clinical Evidence.

BACKGROUND: Gastroesophageal reflux disease (GERD) is a chronic gastrointestinal condition defined by reflux of stomach contents into the esophagus, leading to cardinal symptoms such as heartburn and regurgitation, thereby negatively impacting patients' quality of life. Several underlying mechanisms contribute to GERD, with dysfunction of the anti-reflux barrier playing a key role in reflux. Even though proton pump inhibitors are commonly prescribed, a considerable proportion of patients do not achieve symptom relief. Diaphragmatic breathing (DB) has emerged as an adjunct non-pharmacological t

MRI-Based Gastric Function Assessment in Patients With Functional Dyspepsia.

BACKGROUND: Magnetic resonance imaging (MRI) enables non-invasive, radiation-free assessment of gastric emptying, intragastric liquid distribution, and motility. However, characterization of the stomach function in patients with functional dyspepsia (FD) remains limited. We aimed to apply the Virtual Gaster (VIGA) MRI platform to characterize gastric geometry, emptying dynamics, and motility patterns in patients with FD compared with healthy controls (HCs). METHODS: Twenty-six adults with Rome IV-defined FD and twenty HCs underwent static and dynamic gastric MRI during fasting, gastric fillin

Piecemeal submucosal tunneling endoscopic resection of a 4-cm esophageal leiomyoma.

BACKGROUND AND AIMS: Leiomyomas are the most common benign neoplasm of the esophagus. Although typically asymptomatic, they can cause retrosternal discomfort and dysphagia. Submucosal tunneling endoscopic resection (STER) has been increasingly used for resecting esophageal subepithelial lesions. We present this case of STER for resection of a 4-cm esophageal leiomyoma. METHODS: A 44-year-old man presenting with retrosternal discomfort, dysphagia, and heartburn was found on EGD to have a 4-cm subepithelial lesion in the mid esophagus. Subsequent endoscopic ultrasound and fine-needle biopsy con

Community based countrywide analysis of lactase persistence related genetic variants and their correlation with digestive symptoms in Libya.

Lactose intolerance is a complex symptomatic response to lactose maldigestion that is influenced by, but not strictly determined by, the down- regulation of the enzyme lactase. Several nucleotide polymorphisms - 13910C > T (rs4988235), - 13907C > G (rs41525747), - 13915T > G (rs41380347), - 14010G > C (rs145946881) and -14009T > G (rs869051967) upstream of the LCT gene have been associated with lactase persistence in European, African and Middle Eastern populations. We aimed to study the prevalence of lactase-persistence associated variants of the LCT gene in the Libyan population and its corr

A Rare Coexistence of Gastric Fundal False Diverticulum and Sliding Hiatal Hernia: Technical Considerations and Literature Review.

Gastric fundal false diverticula are exceedingly rare, and to our knowledge, this is the first reported case occurring in association with a sliding hiatal hernia. We describe a 49-year-old woman who presented with persistent dysphagia, regurgitation, heartburn, and halitosis refractory to medical therapy. Imaging revealed a 5.0 × 3.6 × 4.7 cm false diverticulum at the posteromedial fundus and a sliding hiatal hernia. Esophageal manometry demonstrated normal motility with ineffective bolus clearance. Laparoscopic management was employed, combining intraoperative endoscopy for diverticulum loca

From Childhood Ingestion to Adult Obstruction: A Long-Retained Synthetic Gastric Foreign Body.

Foreign body ingestion occurs predominantly in children, and most objects pass spontaneously; only a minority require endoscopic or surgical intervention. Prolonged gastric retention of synthetic material with presentation in adulthood is exceptionally uncommon. A 26-year-old man, who recalled accidental ingestion of expanding construction foam at four to five years of age, was first evaluated by gastroenterology on initial presentation for long-standing epigastric pain, nocturnal discomfort, heartburn, acid regurgitation, reduced appetite, and intermittent vomiting. He had no significant rece

Impact of meal frequency on gastroesophageal reflux disease following laparoscopic sleeve gastrectomy.

BACKGROUND: Saudi Arabia has a high obesity rate (> 35%), increasing the risk of gastroesophageal reflux disease (GERD) and presents major health risks. Laparoscopic sleeve gastrectomy (LSG) is an effective bariatric surgery that reduces mortality. Controversies exist regarding the recommended meal frequency following bariatric surgery, as some patients experience increased GERD symptoms postoperatively. In this study, we describe the association between meal frequency and GERD at least one year after LSG. METHODS: We conducted a retrospective cohort study based on data collected from electro

Reduction of Proton Pump Inhibitor Dependence in Gastroesophageal Reflux Disease Following Isometric Diaphragmatic Resistance Training With a Novel Respiratory Muscle Training Device: An Autobiographical Case Report.

Gastroesophageal reflux disease (GERD) has been associated with transient lower esophageal sphincter relaxations (TLESRs), which are considered a predominant mechanism of reflux and are modulated in part by the crural diaphragm. Long-term proton pump inhibitor (PPI) therapy remains the standard of care but is associated with drug dependency and systemic adverse effects. Strengthening the crural diaphragm through resistive respiratory training represents a mechanistically plausible non-pharmacological approach. We report a case in which a 14-day isometric diaphragmatic resistance training proto

Predictors of Non-union in Subtrochanteric Femur Fractures: A Multivariate Logistic Regression Analysis.

Over the last 20 years, the role of adjuvant pharmacotherapy in optimising outcome in rehabilitation programmes for alcohol-dependent patients has become increasingly evident. New avenues for rational drug treatment have arisen from better understanding of the neurobiological substrates of alcohol dependence, including adaptive changes in amino acid neurotransmitter systems, stimulation of dopamine and opioid peptide systems, and, possibly, changes in serotonergic activity. Disulfiram, naltrexone and acamprosate are currently the only treatments approved for the management of alcohol dependenc

Acute hepatitis B infection escaping vaccination: A case report.

RATIONALE: Hepatitis B virus (HBV) is an important cause of acute and chronic infectious hepatitis globally. Vaccines against HBV are widely available and generally lead to lifelong protection against chronic infection, disease, and infectivity. Vaccine failure, in which a person fails to form sufficiently high antibody titers, occurs in 5% to 10% of the population. Vaccine failure due to mutations in the virus, enabling vaccine escape, on the other hand, is very rare. PATIENT CONCERNS: A 72-year-old man presented with heartburn complaints. In clinical examination, an enlarged liver was found

Pictures certainly are worth a thousand words in the case of the structure of the connective tissue skeleton of normal and diseased myocardium. This report reviews the connective tissue matrix of the normal human myocardial tissue and the pathological myocardial fibrosis in left ventricular hypertrophy due to chronic arterial hypertension in humans and in human chronic chagasic myocarditis. The myocardial connective tissue matrix was studied employing a cell-maceration method that removes the myocardial tissue non-fibrous elements, and leaves behind a non-collapsed matrix, thus allowing a bett

INTRODUCTION: Essential precondition for the development of cervical cancer is a persistent human papillomavirus (HPV) infection. The majority - approximately 70% - of cervical carcinomas is caused by two high-risk HPV types (16 and 18). Recently, two vaccines have been approved to the German market with the potential to induce protection against HPV 16 and HPV 18 among additional low-risk virus types. OBJECTIVES: To analyse whether HPV vaccination is effective with regard to the reduction of cervical cancer and precursors of cervical carcinoma (CIN), respectively? Does HPV vaccination repres

Antimicrobial resistance and vaccine-preventable infections: concurrent health security challenges after the double earthquake in Venezuela.

SCIENTIFIC BACKGROUND: Meningiomas are the most common benign intracranial neoplasms with a slow growth presented as the intracranial lesion. These tumors are without any symptoms for a long time. At the time of diagnosis it is frequently an asymptomatic tumor. In that case the therapist may well suggest a wait-and-see strategy. The therapy of meningiomas focuses firstly on the microsurgical treatment. Volume reduction can be achieved immediately after treatment. Stereotactic radiosurgery is an important non-invasive treatment option for recurrent tumors or meningiomas with partial resection.
