Brainstem and vagal dysfunction in pharyngoesophageal and esophageal dysphagia.

Dysphagia affects approximately 16% of the US population and is classified as pharyngoesophageal and esophageal, the former being more clinically urgent because loss of airway protection may cause pulmonary aspiration and death. Safe swallowing depends on a brainstem central pattern generator, modulated by cortical and subcortical input and executed through the cranial nerves, above all the vagus. The latter supplies the pharynx, larynx, upper esophageal sphincter, esophagus and lower esophageal sphincter. Because afferent and efferent pathways converge in the medulla, small lesions in medulla

Thyroid Ectopia in the Euthyroid Patient: A Rare Case Discussion.

Base-of-tongue masses are difficult to differentiate based on imaging alone. The vast majority of masses in this region are squamous cell carcinomas, necessitating prompt and thorough evaluation. However, particularly in deep, midline, submucosal distributions, congenital masses are an important differential to consider due to the embryological correlates of aberrant structures in this location. For patients in whom ectopic thyroid is suspected, it is imperative to discern whether functional orthotopic thyroid tissue is present. Most patients with thyroid ectopia have some extent of thyroid dy

Development of 3D printed pea protein isolate-based composite spinach as dysphagia diets.

Driven by accelerating global population aging and muscle strength decline in elderly individuals, the demand for dysphagia diets is substantially increasing. 3D printing is equipped to process unpalatable pureed or thickened dysphagia diets into soft-textured and appealing foods tailored to personalized sensory preferences and nutritional requirements. This work explored the feasibility to develop 3D printed the texture-modified pea protein isolate (PPI)-based food as dysphagia diets, with incorporation of the spinach powder (SP) (12.5%, 25.0%, 37.5%, 50.0%, w/w). Results showed that SP incor

Orofacial Strength in a Typically Developing Pediatric Population: A Comparison of the Iowa Oral Performance Instrument and Tongueometer.

PURPOSE: Pressure-based assessments of tongue strength are becoming increasingly common in dysphagia management. However, whether different manometric devices produce comparable measurements for the same tasks, particularly in pediatric populations, remains insufficiently clarified. The purposes of this study were to establish age- and sex-specific reference values for tongue, buccal, and lip strength in a pediatric population using the Iowa Oral Performance Instrument (IOPI) and the Tongueometer (TO) and to systematically examine the effects of device, age, and sex on these measurements, as w

Predicting drooling severity after surgery in youth with neurodevelopmental disabilities: development of a proof-of-principle multivariable prognostic model.

INTRODUCTION: Predicting outcomes of surgery for drooling could support candidate selection and shape expectations for patients and caregivers. This study describes the development of a prognostic prediction model for the expected drooling Visual Analog Scale (VAS) at 32-weeks post-surgery in youth with a neurodevelopmental disability. METHODS: A multivariable linear model was developed with data from a prospectively gathered pseudonymized database of drooling patients from the Saliva Control Clinic of a Dutch tertiary care center. The population consisted of drooling patients with a non-prog

Obstructive 12 cm Esophageal Liposarcoma Removed by Endoscopic Submucosal Dissection and Tissue Fragmentation.

Well-differentiated esophageal liposarcomas are exceptionally rare tumors that are incompletely characterized in the literature. We report a case of an 80-year-old man presenting with an obstructive esophageal liposarcoma that was resected using endoscopic submucosal dissection (ESD) and extracted in fragments due to its large size. At 6-month follow-up, the patient reported resolution of presenting dysphagia with accompanying esophagogastroduodenoscopy notable for no tumor recurrence. This case highlights the efficacy of ESD in the resection of a giant esophageal liposarcoma and the diagnosti

Exploring G-Tube Timing and Clinical Outcomes in Patients With Locally Advanced Squamous Cell Carcinoma of the Head and Neck Receiving Chemoradiation.

BACKGROUND: G-tube placement timing remains poorly characterized in patients with localized head and neck cancer (HNSCC) undergoing definitive chemoradiation (dCRT). METHODS: We retrospectively analyzed 144 patients with HNSCC who received dCRT (2019-2024), categorized by "upfront" or "as-needed" G-tube strategy, to explore factors associated with timing and outcomes using propensity score weighting. RESULTS: Most patients were White (85%) and male (83%); median age was 63 years. Upfront G-tube placement occurred in 60 patients (42%) and was more common among women (64% vs. 37%, p = 0.015) a

Stroke-associated pneumonia: from immunodepression to bedside management and future research directions.

Stroke-associated pneumonia (SAP) is one of the most frequent complications after acute stroke, affecting approximately one in eight stroke patients. Besides prolonged hospitalisation and increased healthcare costs, it is also a major risk factor for mortality and poor functional recovery. The pathophysiology of SAP is driven by two converging processes: impaired local airway defence due to dysphagia as well as reduced airway clearance reflexes, and systemic stroke-induced immunodepression driven by dysfunction of the autonomic nervous system. While post-stroke immunodepression is a major risk

[Gastrointestinal manifestations in idiopathic inflammatory myopathies : Dysphagia, intestinal vasculopathy and pneumatosis cystoides intestinalis].

In idiopathic inflammatory myopathies (IIM), gastrointestinal manifestations are rare and poorly recognized apart from the relatively common dysphagia. Oropharyngeal or esophageal dysphagia in IIM is associated with aspiration and malnutrition and responds well to conventional immunomodulation except for inclusion body myositis. The rare intestinal vasculopathy in nuclear matrix protein 2 (NXP2)-associated dermatomyositis affects both children and adults, causing life-threatening perforation and bleeding from the esophagus to the colon with a high mortality. It responds poorly to standard immu

Giant 90-mm Ossified Stylohyoid Chain Associated with Restricted Mouth Opening.

A 62-year-old man was referred with a six-month history of right submandibular pain followed by dysphagia. At presentation, maximal interincisal opening was 25 mm. Computed tomography angiography demonstrated an ossified right stylohyoid chain extending from the skull base to the hyoid region. It measured 90 mm in length and 17.5 × 10.5 mm at maximal cross-section. The chain contacted the external carotid artery, lay 1.1 mm from the internal carotid artery, and was associated with focal narrowing of the internal jugular vein between the chain and the transverse process of the atlas, with prese

EXPRESS: Letter to the Editor Regarding "Much ado about eating: Intermittent fasting and post-stroke neuroprotection".

Vemuganti and colleagues highlight intermittent fasting (IF)'s neuroprotective potential post-stroke via autophagy and mitochondrial biogenesis. However, clinical translation faces hurdles in metabolically vulnerable patients. This letter critiques IF's bedside applicability, emphasizing post-stroke catabolic stress, dysphagia (~50% cases), sarcopenia risk, and cardiovascular concerns. Recent evidence links short eating windows to higher mortality, countering IF benefits. The obesity paradox suggests nutritional reserves aid survival. Strategies: protein-prioritized planning, dysphagia screeni
